Research BackgroundMedication Overuse Headache (MOH) is a worldwide problem closely related with the public health, of which the global prevalence rate is about 1%-2%. MOH has not only brought serious influence to people’s living quality, but also caused serious social and economic burden. The latest published Eurolight Project results show that the average annual cost of dealing with MOH is three times of that with Migraine and ten times of that with Tension Headache. Preventing MOH attack and improving the living quality of the patients is a common responsibility of the Chinese and Western medical communities.In the recent years, the literature and clinical research found that Chinese medicine has obtained a more outstanding curative effect on the treatment of headache, but there has not been much document literature or clinical research report on the treatment of MOH. According to our clinical treatment research results, associate Professor Xuhong Yang’s experience prescription of Chai xiong Acesodyne Decoction has got a marked curative effect in the treatment of MOH.ObjectiveThis research observes and compares the clinical curative effects between the program group of ChaixiongAcesodyne Decoctionand the one of Flunarizine Hydrochloride in the treatment of MOH, in order to provide the clinical basis for the Chinese traditional treatment of MOH.MethodsChoose 62 cases of the MOH patients that have the migraine as the primary headache type who came to the Neurology ClinicofThe Chengdu University of Traditional Chinese Medicineto see a doctor from the August of 2012 to the October of 2014, and do research on the general situations, the characteristics of the headaches, thefamily disease histories and the anodyne taking histories of the cases chosen. Fill the Migraine Disability Assessment questionnaire (MIDAS) and the Medication Dependence Questionnaire in Headache Patients (MDQ-H) and then divide them into the treatment group and the control group in accordance with the method of random digits table. Provide the treatment group with the treatment program of the suggestion of drug withdrawal, the MOH propaganda and education, taking the ChaixiongAcesodyne Decoction, supplying symptomatic treatment and taking Difene or naproxenforpain relieving, while providing the control group with the treatment program of the suggestion of drug withdrawal, the MOH propaganda and education, taking Flunarizine, supplying symptomatic treatment and taking Difene or naproxen for pain relieving.The treatment cycle is a time of three months. Make the comprehensive curative effectevaluations of the two patient groups on the indexes of the headache degrees, the number of days that the headaches last and the scores of MIDAS separately on the third and sixth months. treatment time was 3 months, were used to evaluate the degree of headache, migraine days, MIDAS grading index of curative effect of two groups of patients at third months and sixth months.Results(1) theDegree of Headache (VAS):The situations of the degrees of the headaches of the two groups are both improved at the end of treatment and in the follow-up processes. There is no difference between the comparative results of the two groups are the same at the end of the treatment, while there is obvious superiority of the result of the treatment group compared with that of the control group in the courses of the follow-up processes. (2) the headache days:The results of the two groups are both improved at the end of the treatment and in the follow-up processes. There is no significant difference between the two groups. (3)the reaction rate of headache days:The reaction rates of headache days of the treatment group and the control group are respectively 76.7% and 72.4%. (4) MIDAS scores:The MIDAS scores of the two groups both fell in the follow-up processes, but the comparison between the two groups has no significant difference. (5)TCM syndrome score:The TCM syndrome scores of wind syndrome and syndrome of blood stasis of the two groups are both improved at the end of the treatment and in the follow-up processes, compared with those before the treatments, while there is little differencein the comparation between the two groups.Thesituations of melancholia of the two groups are both better at the end of the treatments, while the amelioration of the treatment group is more significant than that of the control group. The situations of melancholia of the two groups are both better in the follow-up processes, while there is no significant difference in the comparation between the two groups.ConclusionChaixiongAcesodyne Decoction has certain clinical curative effect in the treatment of MOH, and can significantly improve the living qualities of the patients.The effect ofChaixiongAcesodyne Decoctionis not inferior to that of the flunarizine hydrochloride.ChaixiongAcesodyne Decoction makes obvious improvement of melancholia of the patients during the treatment, while also making a long-term curative effectonthe headache degree in the follow-up processes after the treatment.
