| My mentor, Professor Shen Lin is Chief physician, Doctoral supervisor and professor of Union Hospital affiliated to Tongji Medical College of Huazhong University of Science and Technology. He is also one of the fifth batches of instructors for the old Chinese medicine experts’ academic experience and work. Professor Shen Lin was born into a medical family in 1952. He had read so much since childhood. As a Traditional Chinese medicine talent who received professional higher education in new China, Professor Shen Lin has been engaged in clinical teaching of traditional Chinese medicine and scientific researches for more than 30 years. He has successively presided over more than ten national and provincial scientific research subjects, won 13 second-class and third-class prizes of Science and Technology Progress Award at provincial level and ministerial level. He has been highly praised by fellows and patients.Professor Shen Lin attaches great importance to the study of TCM classics in the long-term clinical teaching practice. He seeks advice from old adage frequently and he is good at thinking. He learns tirelessly from predecessors’ experience from voluminous ancient books of traditional Chinese medicine. He learns many physicians’ academic thoughts as well. Professor Shen acquired rich clinical experience in TCM Internal Medicine and orthopedics disease treatment.On the basis of exploring the origin of Professor Shen Lin’s academic thought, this paper aims to sort out Professor Shen Lin’s academic thoughts and clinical experience in the diagnosis and treatment of renal osteopathy, and induce the TCM rules in renal osteopathy diagnosis and treatment of traditional Chinese medicine, and explore the mechanism of spleen invigorating and kidney nourishing and remove stasis drain turbidity therapy.This research includes two parts:In the traditional theory of TCM it think the kidney generating marrow and dominating bone,and recognize the bone disease root in kidney long ago.Professor Shen Lin think although no ofrenal osteopathy in TCM classics, but its clinical manifestations of academically breaks down. mainly in the bone impotent involving the bone consumptive article. He think the renal osteopathy should belong to the traditional Chinese medicine bone impotent involving the bone consumptive disease category. The mechanism of renal osteopathy in chinese medicine mainly lies in two aspects of deficiency in origin and enrichment in symptom.The pathogenesis of renal osteopathy is yang deficiency of spleen and kidney. The turbidity toxin and blood stasis blocking blood vessel is another pathogenesis of renal osteopathy, and it also can block joint or collaterals. In the middle of the disease, the disease is common with kidney deficiency bone impotent, The commonly used formula is jia wei qing e wan. It can strengthen spleen and nourish kidney. During the late in illness, wet, dirty, blood stasis, poison in Screased dramatic-ally. he divided into two type treatment in the period: the wet muddy resistance type and poison stasis mutual junction type. He treat the type of poison stasis mutual junction of renal osteopathy by ji xue pai du decoction. It can clear heat, remove dampness, and eliminating turbidity by purgation. He treat the pain of polioplus caused by stagnation of qi and blood stasis with yiqi huoxue Fang. It can tomify qi and blood, activate blood and remove stasis. In the process of clinicaltreatment, professor Shen Lin often combining syndrome differen-tiation of traditional Chinese medicine and western medicine disease. The research is aimed at exploring the mechanism of tonifying spleen and kidney and removing stasis drain turbidity method in treatment of renal osteopathy, so as to provide scientific basis for TCM prevention and treatment of renal osteopathy.This study selects 70 patients with maintenance hemodialysis and renal bone disease. They all have spleen and kidney Yang deficiency with wet muddy and blood stasis of the patients.They are randomly divided into treatment group that consists of 33 cases and control group of 37 cases. The control group is given conventional treatment and calcitriol; while the treatment group is given conventional treatment and calcitriol, combined with traditional Chinese medicine of of invigorating spleen and kidney and removing blood stasis and turbidity. The total course of treatment for both groups is 8 weeks. The observation targets include patients’ a single TCM symptoms, the symptom integral of TCM, the values of blood Ca2+, P3-, i PTH values before and after treatment, so as to evaluate the comprehensive curative effect of disease treatment. Results: Using traditional Chinese medicine of invigorating spleen and kidney and removing blood stasis and turbidity, combined with conventional western medicine treatment of renal osteopathy could significantly improve the clinical symptoms, Cold, lumbar debility and cold limbs, waist pain, joint swelling pain, limb edema trapped, body weight, bone pain and other clinical symptoms. The differences between treatment group and the control group difference were of statistical significance. The clinical symptoms of less gas lazy words, eat less, stay, nausea, vomiting, numbness symptoms are not improved. The comparison between the treamment group and control group is of no statistical significance. The blood calcium can be in Screased, and the blood phosphorus and blood i PTH reduced effectively by using traditional Chinese medicine of invigorating spleen and kidney and removing blood stasis and turbidity, combined with conventional western medicine treatment of renal osteopathy. The different results of before and after treatments are statistically significant. And the different results compared with control group are statistically significant. It is safe to use traditional Chinese medicine of invigorating spleen and kidney and removing blood stasis and turbidity, combined with conventional western medicine treatment of renal osteopathy. Conclusion: Using traditional Chinese medicine of invigorating spleen and kidney and removing blood stasis and turbidity, combined with conventional western medicine treatment of spleen and kidney Yang deficiency, blood stasis block and the renal bone disease, achieved good curative effect. It can in Screase the blood calcium, reduce the blood phosphorus and blood i PTH. It can improve the clinical symptoms significantly. The mechanism may be related to the fact that traditional Chinese medicine could improve kidney calcium phosphorus metabolism disorders in patients with renal osteopathy, and directly or indirectly suppress se Scretion of PTH. |
