| 1> Academic heritageProfessor Shen Chunti, is one of the Xu Dihua’s disciples who is the heir of the Menghe medical sect, and was taught by professor Tu Kuixian who is the first batch of national famous aged traditional Chinese medicine physicians and awarded the State Council Special Allowance, and is the heir of Feijia that one of the Menghe medical sects, and is the Chief Physician in Changzhou TCM hospital affiliated to Nanjing university of traditional Chinese medicine, is a professor and the fifth batch of national academic inheritance instructor of TCM. Professor Shen Chunti has engaged in clinical practice, scientific research and education for 45 years, with noble medical ethics, excellent medical skills and rigorous in educating, who focus on combining theories and clinical practice and good at diagnosis and treatment of internal diseases, especially on lung diseases research, having worked on the research of "syndrome" of TCM for a long time, being well-known in the country.Through long-term clinical practice and scientific research, and on the basis of inheriting academic thoughts of the Menghe medical sect, Professor Shen Chunti claimed the application of combination of syndrome differentiation and disease differentiation in clinic, by constant innovation and development, has formed the clinical thoughts of combination of the disease and syndrome, which provide a traditional and modern combination method of TCM for clinical diagnosis and treatment, to improve the clinical curative effect.2、Academic research overviewOver the years, Professor Shen Chunti has been dedicated to academic research and clinical practice on lung diseases. Through clinical practice and scientific research, she has formed unique academic views and thoughts on the diagnosis and treatment of lung diseases, by developing the traditional Chinese medicine theories in the inheritance.2.1 Inheritance of the essence of curing cough from Menghe medical sectThe selection principle of drug from the Menghe medical sect on curing cough tend to be bland and "fewer but better ", which was perfectly inherited by Professor Shen Chunti, who made good use of "linght herbs", such as herba schizonepetae and saposhnicovia divaricata to expel wind and relieve cough, at the same time, according to the important pathogenesis of cough that phlegm-heat storing in lung, adding grilled ephedra for dispersing the lung to relieve cough and asthma; and using cortex mori radicis, trichosanthes peel and folium eriobotryae to clear away heat and remove phlegm; and using pericarpium citri reticulatae and pinellia to strengthen the spleen for regulating transportation and transformation and eliminate dampness and phlegm; using radix scutellariae to clear and discharge lung heat, using adenophora stricta to nourish the yin and moisturize the lung, using almonds and fritillaria to clear up the phlegm to stop coughing; using radix asteris and flos farfarae to moisten the lung to stop cough. All the prescriptions made by Professor Shen Chunti reflected her medication rule is that she thought highly of curing the cough by releasing the lung to stop cough, clearing heat to eliminate phlegm, nourish the yin and moisturize the lung and strengthen the spleen to regulate transportation and transformation.2.2Combination of disease and syndrome and prescription corresponding to syndrome The characteristic and advantage of traditional Chinese medicine (TCM) is the syndrome differentiation, which is beneficial in understanding the essence of diseases and the similarities from different patients to grasp the specific characteristics of every single patient better, and improve the clinical curative effect. Professor Shen Chunti advocated combination of disease and syndrome based on syndrome differentiation, which offer the breakthrough point of reasonably combining TCM theories and research results of the symptoms and syndromes of TCM and correlation between the syndrome and testing indexes in modern medical diseases, have explored some inherent laws between diseases and syndromes.Prescription corresponding to syndrome is also called prescription-syndrome correspondence and prescription related to syndrome, which means prescriptions links to syndromes. The prescription corresponding to syndrome refers to prescriptions corresponding the main syndromes, which represents the location of disease, the characters of the disease belongs to asthenia or sthenia syndrome, cold or hot syndrome and external or internal syndrome, and the most important pathology. Only the targeted therapy for main syndromes can attain the result with half effort. The accompanied symptoms, secondary symptoms and deteriorated syndromes would be reduced or even disappear by eliminating the main syndromes. And the addition and subtraction of prescriptions were on the basis of the "syndrome", changing prescriptions according to the changing symptoms. The prescription corresponding to syndrome is an extension of the combination of disease and syndrome, under the premise of clear diagnosis of disease and the accurate syndrome differentiation to treat diseases by adjusting already being verified prescriptions. In a word, the medicine disease diagnosis is the basis, the accurate syndrome differentiation is the guarantee, and the proper prescription is the key to the treatment. So according to many years of clinical experience, the Professor Shen Chunti has summed up the treatment method of "combination of disease and syndrome and prescription corresponding to syndrome" which echo the mode of disease-syndrome-prescription.2.3Strengthening the spleen and stomach for treating lung diseasesThe spleen and stomach function were often affected in patients with chronic lung disease. The spleen and stomach, belongs to earth in the five elements, is the foundation of acquired constitution. Spleen and stomach health is the important guarantee of the source of qi, blood and body fluid, nourishing limbs and cultivating shen, so the stomach qi is the key to cure disease. Treatise on the spleen and stomach, has put forward all illness was caused by the internal injuries of the spleen and stomach. What worse, the weak spleen and stomach will lead to poor absorption of medicine and food to healthy qi can be reinforced. On the other hand, lung disease causing deficient result in fresh air shall not be spread to shortage of the spleen qi. May also due to turbid phlegm obstructing the lung, the transportation and transformation of the spleen and stomach would be influenced, causing endogenous phlegm, and blocking ventilatory function of lung further.So in the clinic, the Professor Shen Chunti attached importance to protect the spleen and stomach, commonly using pericarpium citri reticulatae, endothelium corneum gigeriae galli and yam to fortify the spleen and supplement qi. Shenling baizhu powder is basic formula to strengthen the spleen and tonify the lung for patients of lung diseases with spleen deficient.2.4Personal inheritance and innovationInspired by theory of "curing sthenia syndrome by less dose medication with light character" from the Menghe medical sect and the Professor Shen Chunti lung disease, I have added herbs with characteristic of light and dispelling wind-evil, such as fineleaf schizonepeta herb, saposhnicovia divaricata, periostracum cicadae and platycodon grandiflorum, which have received good curative effect. And under the mentor’s inspiration, the law of promoting blood circulation to remove blood stasis and invigorating the spleen and supplementing qi were applied at the same time to treat patients with cough and asthma for a long time, belonging to the type of blood stasis and spleen deficient syndrome, which also obtain good curative effect.3.By sorting and summarizing academic thoughts and clinical experience of the Professor Shen Chunti on treatment of lung disease, this paper expounds the individual comprehension:During the process of learning, I have summarized the therapeutic experience from the Professor Shen Chunti on the treatment of chronic bronchitis, COPD, cor pulmonale and bronchial asthma, extracted the diagnosis and treatment thoughts and concluded theories-methods-formulae-medicines of these diseases. At the same time, I have elaborated my own comprehension.4. Clinical curative effect observation of chronic bronchitis with the kidney deficiency and blood stasis syndrome treated by Professor Shen Chunti’s empirical prescription-Xian Qiong decoctionObjective:To further study the clinical curative effect of chronic bronchitis with the kidney deficiency and blood stasis syndrome treated by Professor Shen Chunti’s empirical prescription-Xian Qiong decoction.Methods:Xian Qiong decoction was used as the main formula and the chronic bronchitis patients with kidney deficiency blood stasis were selected as the research object to observe the clinical curative effect. All of 100 cases in accordance with the inclusive criteria were divided into the treatment group and control group (n=50) according to the principle of 1:1 comparison. Both of the groups were accepted respective treatment for a course (2 weeks), to observe related symptoms integral change before and after treatment for evaluation of clinical curative effect.Results:The total effective number of cases in the treatment group was 48 (50 cases in all, 36 cases showing marked effect and 12 cases developing effect). The total effective number of cases in the control group was 42 (50 cases in all,31 cases showing marked effective,11 cases developing effect). There was a significant difference of the total effective rate between both groups, which meant the clinical curative effect of the treatment group is better than that of control group (P<0.05). Before and after treatment, the comparison of TCM symptoms curative effect evaluation indexes in the two groups: Before treatment there was no significant differences of the scores between the two groups. But after treatment the TCM syndrome integral in two groups were reduced. Compared with the control group, TCM syndromes curative effect in the treatment group more was more obvious (P<0.05).Conclusions:Xian Qiong decoction was made by Professor Shen Chunti’s teachers on the basis of experience of Menghe medical sect, combined with years of clinical practice, which was made up with herba epimedii, ligusticum wallichii, salvia miltiorrhiza, semen persicae, grilled ephedra, bitter almond, cortex mori radicis, trichosanthes peel, bryozoatum, radix asteris, flos farfarae, amethyst, citrus chachiensis hortorum and liquorice, to attain the effect of dredging and reinforcing, dispersing and descending, tonifying kidney and promoting blood circulation to remove blood stasis, and regulating the flow of lung qi to clear up the phlegm to stop coughing, being confirmed by clinical practice to be effective. This study found that the treatment effect of chronic bronchitis is better by using Xian Qiong decoction with conventional western medicine, and the clinical safety also has been proved in a certain degree, which can be carried on further popularization and application on the basis of combination of disease and syndrome. |
