Epidemiological Investigations Of Gout TCM Symptoms And Experimental Study On Treating Acute Gouty Arthritis With Danshao Erdi Simiao Yin

Objective: To explore epidemiological distribution and factors of gout TCMsymptoms; and to further study and reveal efficacy and mechanism of Danshao ErdiSimiao Yin treating acute gouty arthritis.Methods:1. Following principles and methods of epidemiology and evidence-basedmedicine, we adopted literature review, expert consulting, questionnaire developed,clinical epidemiological survey, data mining and medical analysis, to discuss factors,common symptoms and signs as well as basic symptoms of gout and diagnostic criteria ofTCM,etc..2. Mice models of acute gouty arthritis were made by intraperitoneal injectionof acetic acid to observe influences of Danshao Erdi Simiao Yin on writhing incubationperiod and writhing frequency; while rat models of acute gouty arthritis were made byankle articular injection of sodium urate to observe influences of Danshao Erdi Simiao Yinon joint volume and level changes of inflammatory mediators48hr. later.Results:1.Gender, educational background, diet preference, drinking history,profession, exercises, social activities as well as medical history were main factors, amongwhich improper diet was the primary one. Joint pain, swelling, deformity, limitation ofmovement, local skin redness, red tongue, pale tongue, dark tongue, large tongue, threadpulse, uneven pulse, deep pulse etc. could be seen, while red swelling and painful jointsand deformity were the main symptoms. It had four common syndromes: blockage due todamp-heat, wind-cold-dampness arthralgia, phlegm and blood stasis blocking and yangdeficiency of spleen and kidney. We established Logistic Regression mathematic modeland the review showed every model was some predictive.2. When the dosage of DanshaoErdi Simiao Yin was between6.175and24.7g/kg, the writhing incubation period of mice extended and the mice writhed less with the dosage increased, which showed a kind ofdosage trends. At the time of4hr,6hr, and8hr after modeled, joint swelling of rats inColchicine group, Danshao Erdi Simiao Yin medium dosage group and high dosage groupwere all reduced significantly (P<0.05). At8h after modeled, TCM low dosage group alsoshowed significant effect, while it was less than that of high dosage group. IL-1βandTNF-αof joint tissue of rats in Colchicine group, Danshao Erdi Simiao Yin low, mediumand high dosage group decreased with significantly statistical differences (P<0.05). Andcompared to those of the medium dosage group and low dosage group, IL-1βand TNF-αof TCM high dosage group decreased more significantly (P<0.05).Conclusion:1. Improper diet is primary factor of gout and red, swelling and painfuljoints as well as deformity are its main clinical manifestations.We also have establishedand reviewed Logistic Regression mathematic model of symptoms.2. Danshao ErdiSimiao Yin has significant efficacy on relieving pain, swelling and inflammation and sucheffects as well as the effect on reducing levels of IL-1βand TNF-α show dosage trends.
