Due to the population aging and popularity of health-management concepts in latest years, it has become necessary to assess quantitatively the health status of junior citizens and realize the health management in a scientific way. As a special population of junior citizens in China, the cadres of health targets are the national elites. The transformation from cadre-health-care to health-management model has become important tendency, which is of extremely practical significance to national construction and development. By analyzing the targets of cadres'health management, this study established new quantitative assessment tools and methods to set up basis for the transformation from cadre-health-care to health-management model. This research is a project subject to special research projects sponsored by Central Healthcare Committee, that is, The Study on the Integrative System of Health Assessment, Early Screening and Comprehensive Intervention.In this study, the methods of Comprehensive Geriatric Assessment of Cadre (CGA-C) were first explored. In addition to referring to the popular Comprehensive Geriatric Assessment(CGA)in foreign studies,we employed such methods as systematic analysis and Delphi survey to establish a comprehensive assessment system for CGA-C including 4 dimensions and 8 indexes including physical health,activities of daily living,mental health as well as adaptive ability to society and the weight of each index and the assessment methods were also identified. Specially, for the Comprehensive Illness Condition(CIC),one of the indexes, we employed Cumulative Illness Rating Scale(CIRS), and furthermore, designed a new scale, the Combined Score of Cumulative Illness Rating Scale (CS-CIRS),to optimize the assessment results by analyzing the feature of cadres and referring to the Scorning Standards for the Popular Illness in Cadres which was complied by experts in health management. On the basis of this index system, by using the CHS, we also established the model for CGA-C and set forth the standards of comprehensive health grade so as to realize quantitative assessment and the comprehensive health grade. Secondly, on the basis of the above established index system, by using the method of equal proportion sampling, we chose at random 402 military cadres in a troop who had taken the physical examination of 2009 as subjects of this study and conduct an empirical research on the CGA-C by way of questionnaire, physical exam and functional assessment to test the feasibility. The survey showed that health status of each dimension of the cadres health are better than those of the common senior citizens in such respects as the diversity of illness structures, the control of hypertension and diabetes, general nutrition level, mood condition and social supports. However, other problems still remained, for example, 34.6% of the cadres suffered the Activities of Daily Living(ADL), 1/3 of them had decreased cognitive assessment and many suffered the sleeping problems as early weak and difficulties in falling sleep and excess combined medication. The comprehensive health scores of the cadres are generally higher than those of common with average 86.83±6.69. The comprehensive health grades of them are 12.4% A, 56.2%B, 25.1%C and 6.2% D. By comparing the subject and object assessment and a Prognostic Index for 4-Year Mortality in Older Adults(PIMO), and by the cohort study on readmission rates of the various grades within 5 years, the results from the object assessment of comprehensive health of the cadres conform highly with the subject quantitative assessment, which showed the precise predication of the readmissions within a short period time and Mortality. Therefore the CGA-C established in this empirical study is scientific and reliable.On the basis of the empirical study results, in the research a service program for in CGA-C is at last put forth and the definition and contents of the comprehensive health assessment is made clear. The strategy health management by grades of the assessment is also pointed out. Furth more, a design of a digital assessment software is made to make feasible for the successful practical Comprehensive Geriatric Assessment of Cadres.In conclusion, we believe that the Comprehensive Geriatric Assessment of Cadre can supplement and rich the present health assessment and enhance a comprehensive, systematic and scientific healthcare service of cadres. It also can lay a foundation for transformation from the healthcare to healthcare management which is of great practical significance to scientific health management and will be an important tendency of healthcare of cadres. The CGA-C established in this study showed great in scientific nature and feasibility in the testing result. It is a beneficial exploration in the assessment methods in Comprehensive Geriatric Assessment of Cadre and will play an instructive role in the healthcare work for cadres. In practice, in addition to the carryout of the CGA-C and making full use its roles, we also ought to change the ideas and enhance the healthcare concepts, to greatly improve the quality of the staff and the geriatric education, to strengthen leadership and form efficient multi-disciplinary teams, to innovate methods to establish scientific assessment system and intensify the management by way of assessment so as to set up a long term evaluation mechanism.
