Objective:In China, the folable intraocular lens is being developed.This study is to detect the cellular toxicityof a new hydrophilic acrylic soft lens in vitro made by Tianjin TEDA biological company, a company of IOL manufactory, and observe its biocompatibility in rabbit eyes.Methods:1. In vitro, we performed cellular adhesion experiment inclunding platelets, granulocytes, macrophages of mouse as well as fibroblast cell in the conjunctiva capsule to compare the compatibility of three soft lens materials consisting of hydrophobic acrylic, silicone and hydrophilic acrylic.2. Staphylococci epidermidis adhesion of many types of IOLs was also observed.3. In vivo,26 rabbits were divided into four groups. Group 1 was implanted with test IOL in one eye and control IOL (a type of imported hydrophilic acrylic IOL) in counterlateral eye after phacoemulsification in 16 rabbits. Group 2 was of test IOL in one eye and no implantation in counterlateral eye in 4 rabbits. Group 3 was of control IOL in one eye and no implantation in counterlateral eye in 4 rabbits. Group 4 was normal control. In all, we implanted 20 test hydrophilic acrylic IOL and control IOL repectively in rabits'eyes after phacoemulsification.4. The rabbits were observed by slitlamp microscope at 1 week and 1,3 months respectively after operation and paracentesis to count white blood cell was also performed simultaneously. The eyeballs and IOLs were excised at 1,3 months postoperatively to make the IOL scanning electronic microscopy and tissue pathology.Results:1. In vitro, the platelets and granulocytes on hydrophilic acrylic IOLs are less than that of hydrophobic acrylic IOLs and silicone IOLs which had significant differences.The adhesion of macrophages of mouse on hydrophilic acrylic IOLs is significantly lower than hydrophobic IOLs and similar to silicon IOLs. The tested hydrophilic acrylic IOLs shows no toxicity to the growth of human fibroblast. The hydrophlic acrylic is better than the silicon and inferior to the hydrophobic acrylic.2. In the adhesion of staphylococci epidernidis, the hydrophilic acrylic IOLs are the least ones which is similar to heparin modified IOLs.3. In rabbit eyes, the white cells counting in aqueous is significantly lower in control hydrophilic acrylic IOLs at 1 wek postoperatively. But the differences were not significant at 1,3 months postoperatively. The corneal edema, exudates in the anterior chamber and cells on the IOL surface between groups are no significant differences.4. There are a few proteineous membrane and macrophages gathered on the surface of tested hydrophilic acrylic IOLs under scanning electronic microscopy at 1 month postoperatively. At 3 month postoperatively, the proteineous membrane reduces and the macrophages scatter.There are no significant differences between the two hydrophilic IOLs.5. Under microscopy of anterior segment tissue there are no significant differences between two groups, less synachae is founded in anterior chamber angle and no edema in cornea. The iris and ciliary body shows no injection and synachae. But there are less lens epithelium cells proliferations on the posterior capsule in the control IOLs because of its square edge design.Conclusios:1. There are less cellular and bacterior adhesion on the hydrophilic acrylic IOL made in china. It has no cellular toxicity.2. The biocompatibility in rabbit eyes is mimic in the test and control hydrophliclic acrylic IOLs.
