Investigation On Female Premenstrual Syndrome Of Chinese Syndrome (Deficient Syndrome) Distribution And Relevant Constitution

Premenstrual Syndrome (abbreviated as PMS), occurs repeatedly during the luteal phase of the menstrual cycle, and refers to disorder of patients' body,spirit and behavior, influences woman's daily life and work. The major manifestation includes symptoms of restlessness and easy tantrum,insomnia,stress,depress,anxiety,headache,spargosis and puffiness of face, etc. Judged from clinical symptoms of PMS, the disease happens highly among reproductive life women. Disease incidence reported abroad was about 30%~40%. In Zhao Gengli's investigation of 454 reproductive life women showed the incidence was appropriately 30.4% and Qiao Mingqi's epidemiology survey indicated the incidence was 41.9% among adult women. The review of Li Yanqin reported the incidence of PMS among reproductive life women accounted for 30%~79%. Zhang Deli even reported most of reproductive life women had PMS, thought all not seriously, yet the incidence might be highly as 96%. As the high incidence and influence on women'life and work, research of PMS have been attracting concerns by scholars more and more.ObjectiveIn this study, we try to gain the message of Chinese Medicine Syndrome distribution rule of the PMS patients with deficient syndrome, and evaluate the relationship between patients'constitution type and premenstrual syndrome, additionally explore and analyze relative factors for epidemiology of female premenstrual syndrome with deficient syndrome in Tai Wan. Besides, the research would provide with strict scientific information and essential evaluation system for new drugs'exploitation and clinical study of prevention and therapy for patients with PMS deficient syndrome in Chinese Medicine'sMethodsThe study used the research methods of epidemiology, adapted synchronization observation and analysis of multi-subjects and applied two kinds of questionnaires, namely Chinese Medicine symptom questionnaire and Chinese Medicine constitution type questionnaire. Our criteria standardization used diagnosis standardization of integrated medicine of Chinese medicine and Western medicine, referring to new century textbook of gynecology in Chinese Medicine,diagnosis and treatment standardization of diseases and symptoms in Chinese Medicine, and the sixth edition obstetrics and gynecology, etc. In the research, we recruited 512 cases with PMS deficient syndrome.Based on the research thesis, our questionnaire included four parts. The first part was the basic information of individual; the second one included menstruation condition and history of chronic diseases; the third part was questionnaire of premenstrual syndrome; and the fourth one was questionnaire of constitution type. There were eighty three questions among four parts including Chinese Medicine symptoms and signs such as spargosis,low emotion,insomnia and dreaminess,restlessness and tantrum,fine pulse,headache,white fur,pale and fat tongue,moist fur,increased weight,sinking pulse,dizziness,pale and white tongue,string pulse,fate pulse,etiolate complexion,few fur,soreness and weakness of waist and knees,distension and fullness of gastric cavity and abdomen,poor appetite, etc. Distribution of Chinese Medicine symptoms,distribution of Chinese Medicine syndrome type and distribution of Chinese Medicine constitution type were analyzed by descriptive statistics. Through observing their frequency, we tried to explore the distribution information of major Chinese Medicine symptoms,tongue manifestation and pulse of PMS with deficient syndrome. Chi square test was used to analyze difference of symptoms and signs among diverse syndrome type and difference of syndrome type and Chinese Medicine constitution type; and correlation analysis was applied to explore the correlation between four syndrome types and diverse kinds of Chinese Medicine constitution type.ResultsResults of 512 female PMS patients with deficient syndrome in Chinese Medicine were showed below: 1. From many to few, syndrome distribution of Chinese deficient syndrome was syndrome of stagnation of liver and spleen deficiency(223 cases, accounting for 43.55%),syndrome of deficiency of heart and spleen (112 cases, accounting for 21.88%),syndrome of yang deficiency of heart and kidney (10lcases, accounting for 19.73%),syndrome of yin deficiency of liver and kidney (76 cases, accounting for 14.84%). disease severity of most PMS patients was slight or moderate, totally 443 cases accounting for about 86.5%. All serious patients have symptoms before menstruation; appropriately accounting for 5.1%.37.6% patients have body symptoms as major manifestation and 35.7% patients have symptoms of spirit and body both. There are 148 cases, or 28.9% patients manifested spirit symptoms mainly.2. Symptoms and signs of PMS Chinese Medicine deficient syndrome concentrates in syndrome of liver and spleen meridian.3. Totally nineteen Chinese Medicine symptoms and signs of four syndrome types have significance statistically (P<0.05). There are irregularity of menstrual cycle,low emotion,melancholy and unhappiness,unfocused thinking,low efficiency,thoughtfulness and anxiety,restlessness and easy tantrum,forgetfulness,fatigue in mind and lack of power,headache,palpation,sprgosis,diarrhea,edema of face and extremity,softness and weakness of waist and knee,fear of cold and cold of extremity,dysphoria with feverish sensation in chest,palms and soles,tidal fever,sweating. Through comparison of positive rate of unity symptom, there is relation between single symptom and syndrome type statistically.4. Spirit symptoms and body symptoms are quite prominent among syndrome of liver stagnation and spleen deficiency and syndrome of yang deficiency of spleen and kidney. Syndrome of deficiency of heart and spleen manifest mainly body symptoms and syndrome of yin deficiency of liver and kidney manifest mainly spiritual symptoms. Slight and moderate severity patients account about for 89.2% and 89.3% relatively in syndrome of liver stagnation and spleen deficiency and syndrome of deficiency of heart and spleen. There are more moderate and serious patients among syndrome of yang deficiency of spleen and kidney, totally about 60.4% and 67.1%. all serious patients have anxiety before menstruation to some extend. Therefore, there are more PMDD patients among syndrome of yang deficiency of spleen and kidney and syndrome of yin deficiency of liver and kidney, all results above have significance statistically (P<0.05)5. Among Chinese Medicine constitution type, the proportion of qi depression type is higher, about 28.52%; then static blood type, dampness-heat type, accounting for 13.09%,12.89% relatively; the proportion of qi deficiency type,yang deficiency type and yin deficiency type is lower, about 10% relatively.6. Correlation analysis between Chinese Medicine syndrome type and Chinese Medicine constitution type indicates:syndrome of liver stagnation and spleen deficiency is correlative to constitution type of qi depression; syndrome of deficiency of heart and spleen is relative to constitution type of qi deficiency and constitution of static blood; syndrome of yang deficiency of spleen and kidney is correlative to constitution type of yang deficiency; and syndrome of yin deficiency of liver and kidney to constitution type of yin deficiency.ConclusionFrom investigation results of clinical symptoms in Chinese Medicine, though Chinese symptoms of premenstrual syndrome is very complicate, yet pathological changes of liver are main, dealing with heart,spleen,lung and kidney. Correlation analysis suggests, among Chinese Medicine constitution types, constitution type of qi depression is correlative tightly most to syndrome of liver stagnation and spleen deficiency, reflecting the important stimulation of liver; syndrome of deficiency of heart and spleen is correlative most to constitution type of qi deficiency and constitution type of static blood, reflecting the function of heart and spleen to stimulate the qi and blood; syndrome of yang deficiency of spleen and kidney is relative most to constitution type of yang deficiency, and syndrome of yin deficiency of liver and kidney is relative most to constitution type of yin deficiency.
