ObjectiveTo understand the assessment of multi-health functional status and their influencing factors of the community's elderly population, and to analyze the impact of chronic diseases on ability of daily life of the elderly in communities. Also to investigate ADL function and their relation with the health service demand for city community-dwelling elderly, so as to provide an important basis for decision-making process of general community health services.MethodsA total of 648 residents aged 60 and above was selected among three community centers in Wuhan city. All participants were interviewed by using questionnaires of Chinese version of OARS for related information of multi-health functional status and their influencing factors of the community's elderly population, also and ADL function and their relation with the health service demand . Chi-square test and non-conditional Logistic regression model, and average Ridit scores and CIS scores were used in data analysis.Results1. Correlation analysis results of total integrated health functional status and the part of the health function showed:â‘ Overall functional health status of the elderly and parts of the health function of has a very close relationship, with the overall functional health status is most closely with the activities of daily living and status of physical health.â‘¡Three-parts of physical health, mental health and activities of daily living have close inter-relationship. Social health and economic conditions were positively correlated with these three parts, social health and economic situation are closed with each other.2. Of multi-dimensional assessment of health status: survey of the mental health of the elderly are the highest proportion of the fine, amounting to 88.7 %, followed by social health (83.3%), the economic situation (66.5 %), physical health (50.9%), survey of the activities of daily living are the lowest proportion of those good for 45.0 %. General health of the elderly who account for 36.7% good, in general accounted for 45.4%, those with poor accounting for 17.9%.3. Analysis of multidimensional impairment of elderly population showed: five parts were not damaged accounted for 25.5%, of which only one, or two, or three, or four parts of the damage accounted for 26.0%, 23.5%, 13.0%, 7.0%; among 5 single portion of all, the damage and only 5.2%. Also showed that five single function, the highest rate of damage for the daily activities of capacity, 648 elderly relative damage ratio was 54.1%, followed by physical health (49.3%), economic situation of 33.6 %, social Health 16.8%, the lowest damage rate for the mental health of just 11.3%. The damage of healthy function of elderly population are significantly increased with age; The health functional status of the elderly with university education level is the best, followed by secondary education, primary education, and illiterate education. The more income is lower, the damage of the health functional severer, with the income increase, the health functional status of the elderly has improved markedly. Marital condition is also a factor that affects health functional status of the elderly.4. Status of chronic illness is related with the functional status of daily activities. The prevalence of chronic diseases was 85.8% among the elderly, The top five diseases in turn were: hypertension, rheumatoid arthritis, disease of digestive system, chronic lung disease, myocardial disease. The rates of disability for Activities of Daily Living (ADL), Physical Activities of Daily Living(PADL) and Instrumental Activities of Daily Living (IADL)were 55.1%, 15.6% and 54.8% respectively. Chronic lung disease, disease of digestive system, rheumatoid arthritis, myocardial disease. Stroke, glaucoma showed more significant effect on disability for ADL. The numbers of chronic diseases the elderly got influence the severity of disability of ADL. Conclusion1. The Findings of this study validate the research hypothesis. Of the general health of the elderly function are not be optimistic; impact factors on the health of the elderly general functions are chronic disease and factors such as age, education, marriage, exercise, and income. The prevalence of chronic diseases is an important health problem among the elderly. Chronic diseases deteriorate the disability of ADL,PADL and IADL among the elderly. Elderly health care should be undertaken to improve the quality of life of older persons.2. This innovations of the study is:â‘ From the research methods, this study used number of functional health damage part as standard, setting five-part measure of health functional status of the elderly population's from best to worst, which lined up a healthy spectrum as a way to replace research of the traditional disease spectrum. This method of elderly Comprehensive health evaluation has an important application value either in theory or in practice.â‘¡The study made a major assumption based on biopsychosocial medical model, that factors such as physical health, psychological health status, activities of daily living, economic conditions and social relations of resources in five areas affect health of the elderly population, and the five aspects are closely related and influenced each other. Assessment of Multi-dimensional functions combined with each aspect can reflect the elderly population's health comprehensively. This research confirmed the above assumptions. Therefore, this study design is of a certain degree of theoretical innovation.â‘¢Of statistical analysis method, the average Ridit value and the CIS are two useful indicators for the evaluation of health functional status of population. The Ridit and the CIS analysis can distinguish the different characteristics of groups of the older persons in health, the pros and cons. To screening out the impact factors of population health, also contribute to further understanding of health is a "degree" of modern health concept.3. No existing scale to cover all of the old features information such as 0ARS Scale, whose contents were considered the most extensive, also did not involve the living environment, and nutritional risk factors that influencing the elderly. Further determination of the contents of overall health functional evaluation for the Elderly is future research direction.
