The Study Of Using Regulating The Pivot To Treat Polycystic Ovarian Syndrome

【Study purpose】Polycystic ovarian syndrome(PCOS)is a most common hormonal problem for women in their childbearing years.5%-10%of women in their childbearing years can get it.For those women who cannot get pregnant because of non-ovulate vibriosis,5%-21%are PCOS patients.Women have polycystic ovary disease, are likely to have an irregular or infrequent menstrual periods;an absent or more normal menstrual periods;vibriosis;hirsutism;acne that gets worse; weight gain and acanthosis nigricans;increasing the surface by the double side ovary to be smooth,the color ash shines,the white membrane accumulate hardens,under the membrane presents the pearl string type the not mature graafian follicle;it presents hyper-androgenemia,hyper-luteinizing hormoneand,insulin resistance and compensating hyper-insulinemia.Along with the Chinnese and the Western medicine observation,studies to the PCOS understanding is deeper,the date also demonstrates its complexity and the importance.It not only affects patient's reproduction health and the survival quality,moreover its unusual hormone environment causes the patient to the endometrial carcinoma,the hypertension,hyperlipidemia,the diabetes and disease incidence rate obvious increase and so on heart blood vessel of brain accident.But till now the cause of disease is not extremely clear yet.The modern medicine thought the cause of disease involved the heredity,the internal secretion,the immunity;the metabolism and so on is various.Although it presents hyper-androgenemia,hyper-luteinizing hormoneand and compensating hyper-insulinemia,the relationship between these three with PCOS is still not clear yet.There is no Chinese Medical terminology for PCOS.Systemic research and studies of PCOS only begins in the recent 20 years and until today we are still looking for a complete and standardize method to diagnose PCOS.In Chinese clinical assessment,PCOS is similar to "irregular of menstruation", "infertility","aggregation accumulation ",and "flooding and spotting", etc.Chinese Medical studies among these years also realized that PCOS may be associated with malfunctions of liver,kidney,spleen that causes deficient, sputum,ecchymosis and fire;the insufficient of thoroughfare and conception vessels without replacement;deficiency of yang in spleen and stomach that causes sputum infectious internally;the congestion of liver qi elevates liver fire uprising may also causes the illness.Professor Zhang leading a group of research specialist of PCOS,with clinical experience and investigation, they found out that an elevated level of testosterone is the key issue of the young PCOS patients with liver qi depression,liver fire,liver fire with dampness and liver fire depression.Lesser yang governs the pivot.Lesser yang leads qi movement in and out in a normal condition,up and down freely,open and close normally.PCOS is the ovaries in an abnormal condition.The ovary breeding ova maturely just like storage;the ova are mature after that ovulates,just like flows swiftly.The storage and flows swiftly is depended on the regulation of lesser yang qi movement.Former and famous Chinese Doctor Ran XueFeng had a publication regarding of Shanghanlun.He stated that "Yang governs the pivot,either internally or externally".Uplifting or lowering the body."Using Xiao Chai Hu Tang for externally,using Dachaihu Tang for lowering".Clinical observation in these years has discovered that most of the PCOS patients have the symptoms of dysfunction of the pivot.Under the supervision of Professor Zhang,we divide patients by characterized their symptoms.For those patients with dysfunction of pivot and qi staqnation,we tried to use Dachaihu Tang as the major formula and added with shichangpu,yuanzhi to coordinate the heart and kidney,to regulate the pivot and studied the changes in symptoms and hormonal secretion.【Method of Study】1.To accomplish the study,we divide PCOS patients into two groups.The first group has 42 patients and using Chinese Medication while the other group has 30 patients using Western Medication.Take detail records for each patient before treatment,such as correlation of all four examinations, menstrual condition,acne,hair grow,age,weight,height,combined calculation of BMI,analysis of menstruation and basic body temperature (B.B.T.)2.For the Chinese group,using medicine from Sun Ten Pharmaceutical Company that including 10.5g of Dachaihu Tang,1g of shichangpu,1g of yuanzhi and 0.5g of wugong.3-times a day before meal.Stop medication during menstruation period.Continuing for 3-6 months.Sun Ten Pharmaceutical Company' s 10.5g Dachaihu Tang including 8g of chaihu,3g of huangqin,3g of baishao,5g of ban xia,2g of zhishi,5g of sheng jiang,2g of da huang and 2 Chinese dates.For those kidney yin deficiencies,add 5 g of YouguiWan, for kidney yang deficiencies,add 5g of Zuogui Wan;for those blood stasis add 1g of Dan Sheng,for those spleen deficiencies,add 1g of shan yao.For the Western group,taking Metformin on the 5th day of every menstruation cycle. Those who have an absent menstrual period and ultrasonic waves shows there is no predominance ovarian follicle,use different medication together with LH,FSH,T,PAL tests.Take detail records for each patients before treatment, such as menstrual condition,acne,hair grow,BBT,menstrual cycle after treatment,changes and improvement of the aforementioned and the ability to get pregnant3.According to China' s "Chinese native medicine new medicine clinical guiding principle" giving a standard definition of treatment of an irregular menstruation problem.Using SPSS12.0 for statistic,X2 tests for data assessment,Rid for equalization.【Result】1.For the Chinese Medication group,among 42 PCOS patients,there are 13 patients being fully recovered(30.95%),12 patients with significant improvement(28.57%),6 patients with some improvement(14.28%),that make up a total of 73.80%.No improvement for 11 patients.7 patients getting pregnant during treatment.2.After treatment,patients with irregular menstruation cycle are under control.10 patients out of 42 have 21-40 days in one-cycle has increased to 34 out of 42.Those who has 41 days in one cycle drops from 31 out of 42 to 8 out of 42.3.For B.B.T.body temperature,before treatment there is 42 has unidirection,after treatment is 11. 4.There are significant improvements in Chinese Medication group than the Western group in areas of menstruation cycle,lowering of LH and LH/FSH comparison.【Conclusion】1.Dysfunction of the pivot is the most important mechanism of disease. From the ovaries' s ova' s growth,mature,the ovulation and the storage and flows swiftly,the ovaries will not open and close normally,normal storage and flows swiftly when the pivot dysfunction.It presents PCOS and irregular of menstruation.2.Dysfunction of the pivot' s PCOS patients show no ovulation,multi-ovarian follicle.Biochemistry indicated that hyper-androgenemia, hyper-luteinizing hormoneand,and presented hyper-androgenemia(irregular of menstruation,ache and hair grow)in the clinic.3.Using JiaWey Dachaihu Tang to regulate the pivot is the most effective way to treat PCOS.By using Dachaihu Tang(for those kidney yin deficiencies, add 5 g of YouguiWan,for kidney yang deficiencies,add 5g of Zuogui Wan;for those blood stasis add 1g of Dan Sheng,for those spleen deficiencies,add 1g of shan yao)on the PCOS patients can lower LH and T levels,it also can correct menstruation,recover ovulation and fertility.This is the first paper discuss PCOS from dysfunction of the pivot' s view. By regulating the pivot,can correct menstruation,recover ovulation and impove multi- ovarian follicle.The result shows that by using JiaWey Dachaihu Tang not only can treat PCOS but can prenent further diseases.
