The Ziyin Yiqihuoxue The Anti-diabetic Patients With Cerebral Infarction Clinical And Experimental Research

Cerebral infarction is one of main complications of diabetes,and one of main lethal and maimed reasons impatients with diabetes. Of the same age range, cerebral infarction incidence of diabetes patients is significantly higher than that of patients without diabetes. Diabetic cerebral infarction belongs to "emaciation thirst and (ischemic)apoplexy" category in TCM. It is considered that the deficiencies of yin and qi and the disorder accumulation of blood circulation were basic pothogenesis of diabetic cerebral infarction (emaciation-thirst and ischemic apoplexy) according to the ancient documents and mordern research achievement. Nourishing yin and supplementing qi and improving blood circulation (ZYYQHX) may be the basic therapeutic principle of diabetic cerebral infarction.To study the clinical effect of ZYYQHX on diabetic cerebral infarction, 42 cases with this disease were observed. The total effective rate of the treated group with ZYYQHX recipe was86.4%, and that of the control group with XinNaoShuTong was 75%. It is indicated that the efficacy of ZYYQHX recipe is superior to that of XinNaoShuTong. ZYYQIIX recipe could improve Patients?symptoms, and reduced blood-glucose and glycation products, reduced triglycende cholesterol, high density Iipoprotein and lipoprotein(a),and effectively reduced the intracranial thrombus.To explain the mechanism of actions of ZYYQHX, ZYYQHX recipewas used to rat models with high blood-glucose and ischemic cereral injury. The results showed that:ZYYQLIX recipe could significantly reduce rats?fasting blood-glucose and restrain continue elevation of blood-glucose after cerebral ischemic reperfusion injury.ZYYOHX recipe could prevent elevation of thromboxane B2(TXBjof model rats?brain tissue, regulate I/K ratio. ZYYQIIX recipe could also prevent elevation of endothelin (ET) in plasma ,and elevation of nitric oxide (NO) and exreb-rospinal amino acid (glutamate and asparate) content in brain tissue. Observed by optical microscope, ischemic brain injwy could prevented by ZYYQHX recipe.Altrastructure and apoptosis of cerebral cortex and hippocampal gyrus CA1 area in cerebral tissue were observed on gerbil model with high bloodglucose and cerebral ischemic reperfusion using transmission electron microscope and TUNEL method. The results showed that:on gerbil model with ischemie injury and two days reperfusion there were two forms of cell death in cerebral tissue, neuronal necrosis and apoptosis.In ZYYQHX recipe group ,there was no neuronal necrosis, neuronal ultrastructure was more improved than model group, neuronal apoptosis was slighter than that of model group. The results all above suggested that:ZYYQHX may play the role of antagonizing high blood-glucose and cerebral ischemia by reducing blood-glucose, inhibiting elevation of TXB2, ET, NO, and excessive release of exreb rospinal amino acid, antagonizing CA1 area neuronal necrosis and apoptosis.
