ObjectivesTo study the distributing rule of CM(Chinese Medicine) syndromes, and CMpathologic characteristic of the patients with BPH(benign prostate hyperplasia).To study the relationship between syndromes of BPH and the indexes; To research theessences of the syndrome and pathophysiology about BPH; To research the efficientmethods in treating and preventing the BPH with acupuncture and moxibustion; Toevaluate the syndrome-element integral method using in acupuncture and moxibustion;To study the mechanism of treating and preventing BPH with acupuncture andmoxibustion.MethodsIn order to study rules of syndrome, 120 patients with BPH were chose and dividedinto 6 types. The 6 types were deficiency of kidney Qi, yin deficiency with fire excess,deficiency of kidney yang, blood stasis in lower-jiao, damp-heat in the urinary bladder,and heat obstructing lungs. The relevant was analyzed during the syndromes-element,age, I-PSS (international prostate symptom score), the prostate volume, the urinaryflow rate, the sex hormone, and the prostate specificity antigen.40 patients were chose and treated by reinforcing and reducing technique accordingto syndrome differentiation, in order to compare the difference during syndromeselements integral, IPSS, QOL, the urinary bladder remaining amount of urine, theprostate volume, maximum urinary flow rate and sex hormone.Kidney deficiency is the essential syndrome in BPH, including deficiency of kidneyyang, deficiency of kidney qi, and yin deficiency with fire excess. Blood stasis inlower-jiao is secondly.According syndromes elements integral, locations of BPH is mainly at kidney,spleen and urinary bladder secondly; Damp, Qi stagnation, blood stasis, heat, phlegmand mass are in turn in excessive syndrome. Qi deficiency, yang deficiency, blooddeficiency and yin deficiency are in turn in deficient syndromes.Pathologic changes of syndromes elements exist in various degrees in everysyndrome. T descends obviously, E2 ascends and T/E2 descends in every syndrome ofBPH. There are no difference during FPSA,TPSA,FPSA/TPSA in every syndrome(P>0.05), There are significant difference of them comparing with control group.There are relationships during ages, E2 and T/E2 according syndromes elementsintegral of kidney. The syndromes elements influence each other in cases of BPH. PVrelates to deficient syndromes elements. The change of the prostate specificity antigenis nothing with syndromes elements integral, and evolvement of BPH.The acupuncture and moxibustion would regulate the syndromes-element integral.E2 changes significantly and T doesn't changes significantly after treatment withacupuncture and moxibustion. All the IPSS, QOL, RU, and Q max improvesignificantly in mild and moderate cases after treatment with acupuncture and moxibustion, which suggest acupuncture and moxibustion could improve pathologicalchange as well as objective index.All the IPSS, QOL, RU, Q max and PV did not change significantly in severe casesafter treatment with acupuncture and moxibustion.ConclusionsKidneys, spleen, the urinary bladder are main locations for BPH. Deficiency of thekidney is the essential in BPH. Deficiency of spleen also appears in every type ofBPH. The symptoms of lower urinary tract usually are due to disorder of urinarybladder function.Coincidence of deficiency and excess is the pathologic feature for BPH. Pathogenicdamp, qi stagnation and blood stasis are common in excess types, and deficiency of qiand deficiency of yang commonly exist in deficient types.Pathophysiologic foundation of BPH: Age, E2 and T/E2 influence pathogenicchange of kidney. T and E2 influence syndrome. Coincidence of deficiency and excessis due to every pathogenesis in BPH. The prostate specificity antigen would not be afactor of pathophysiologic foundation of BPH, as it does not affect thesyndrome-element integral, and does not affect the BPH syndrome evolution.Kidney, spleen, deficiency, damp and stasis are key points for the symptom lowerurinary tract. Invigorating kidney yang, reinforcing spleen, dispelling damp andresolving blood stasis are basic mothers in treating BPH.Acupuncture and moxibustion has better effect in mild and moderate cases of BPH.It is feasible for syndrome-element integral as CM clinical index.It is suggested in the study that invigorating the kidney may adjust the hormonehorizontal disorder, and prevent prostate hyperplasia. Benefiting yin, Qi and bloodwould affect the prostatic volume, and delay the prostate gland proliferation progress.It would get better clinical result by efficient combination of them.
