Chronic gastritis(CS)is a disease which is very common among the out-patient, for the present time the treatment for CS is not satisfied. There is a broad horizon for the treatment of CS by means of traditonal Chinese medicine. The relationship between the expression of apoptosis gene and TCM pattern type of Chronic gastritis was researched literaturely and clinically in this study. Part One: literature researchChronic gastrit is belonged to the range of "gastritis pain", apoptosis is a sort of process in which cell kill itself according to it's program in a certain physiological or pathologic condition. Fas belonged to apoptosis gene, whereas bcl-2 belonged to antiapoptosis gene. Part Two:Experiment research1. ObjectiveThe purpose of this study was to determine whether there is a relationship between the presence of apoptosis and TCM pattern type. The substance Background of Pattern Identification of chronic gastritis is to be revealed in gene stratification, a system of pattern identification for chronic gastritis is to be established in molecul and gene.2. MethodsExpession of Fas and bcl-2 of 41 Liver-Stomach Disharmony and 50 Sleen-Stomach Damp-heat in chronic gastritic were investigated with 10 health persons as control by the methods of immunology and histochemistry. meanwhile,an insitu apoptotic cell detection method was adopted to detect the apoptotic cells in the selesions, and the number of apoptotic cells was comared with the Fas gene expression in eachcase.3. Result3. IComparison of the Positive rat of HP among Liver-Stomach Disharmony, Sleen-Stomach Damp-heat and ControlThe positive rates of HP is of 22%, 52% and 10% respectively in Liver-Stomach Disharmony Sleen-Stomach Damp-heat and control, the positive rates of HP were significant higher in Sleen-Stomach Damp-heatm than in Liver-Stomach Disharmony and control ,there is a significant correlation between the pattern type and HP infection. 3.2comparison of the positive rat of bcl-2 among Liver-Stomach Disharmony, Sleen-Stomach Damp-heat and controlThe expression rate of bcl-2 antigen was 24.2%, 76.0% and 30.0% respectively in Liver-Stomach Disharmony Sleen-Stomach Damp-heat and control. The expression rate of bcl-2 antigen were significant higher in Sleen-Stomach Damp-heatm than in Liver-Stomach Disharmony and control (P<0. 01). there was no significant difference in Liver-Stomach Disharmony and control(P>0.05). there is a significant correlation between the pattern type and expression rate of bcl-2 antigen(P<0. 01). 3. 3comparison of the positive rat of Fas among Liver-Stomach Disharmony, Sleen-Stomach Damp-heat and control sleenThe expression rate of Fas antigen was 70. 7% and 46% respectively in Liver-Stomach Disharmony Sleen-Stomach Damp-heat, the expression rate of Fas antigen were significant higher in Liver-Stomach Disharmony than in Sleen-Stomach Damp-heat(P<0.01), there is a significant correlation between the pattern type and expression rate of Fas antigen(P<0.01). 3.4Comparison of the Positive rat of Bcl-2 between hp Infected and Uninfected of Liver-Stomach DisharmonyThe expression rate of bcl-2 antigen was 33.3%, 21.9% respectively in Hp infected and uninfected of Liver-Stomach Disharmony, there is no significant correlation between the expression rate of bcl-2 antigen and HP infection, in Liver-Stomach Disharmony (P>0. 05). 3.5Comparison of bcl-2 of the HP Infected and the Uninfected of Sleen-Stomach Damp-heatThe expression rate of bcl-2 antigen was 30. 8%, 79. 2% respectively in Hp infected and uninfected of Sleen-Stomach Damp-heat, the expression rate of bcl-2 antigen were significant higher in Hp uninfected than that of infected (P<0.01). there is a significant correlation between theexpression rate of bcl-2 antigen and HP infection, in Sleen-Stomach Damp-heat (P<0.01).3.6Comparison of Fas of the HP Infected and the Uninfected of Liver-Stomach DisharmonyThe expression rate of Fas antigen was 77. 8%-. 68. 8% respectively in Hp infected and uninfected of Liver-Stomach Disharmony, there is no significant correlation between the expression rate of Fas antigen and HP infection in Liver-Stomach Disharmony (P>0. 05).3. 7Comparison of the Positive Rat of Fas between the HP Infected and the Uninfected of Sleen-Stomach Damp-heatThe expression rate of Fas antigen was 69. 2*){n 12. 5% respectively in Hp infected and uninfected of Sleen-Stomach Damp-heat, the expression rate of Fas antigen were significant higher in Hp infected than that of uninfected (P<0.01). there is a significant correlation between the expression rate of Fas antigen and HP infection in Sleen-Stomach Damp-heat (P<0. 01).3. 8Comparison of the Positive Rat of bcl-2 between the HP Infected and the Uninfected of CSG and ControlThe expression rate of bcl-2 antigen was 66. 1%> 31.4% and 30.0% respectively in Hp uninfected , infected of chronical superficial gastritis and control, the expression rate of bcl-2 antigen were significant higher in Hp uninfected of chronical superficial gastritis than that of Hp infectedin and control. (P<0.01). there is a significant correlation between the expression rate of bcl-2 antigen and HP infection. 3. 9Comparison of the Positive Rat of Fas between the HP Infected and the Uninfected of CSGThe expression rate of Fas antigen was 71. 4% and 48. 2% respectively in Hp uninfected^ infected of chronical superficial gastritis, the expression rate of Fas antigen were significant higher in Hp infected of chronical superficial gastritis than that of Hp uninfectedin (P<0.05). there is a significant correlation between the expression rate of bcl-2 antigen and HP infection(P<0.05).3. lOComparison of the Positive Rat of Fas between the bcl-2 Infected and the Uninfected of CSGThe expression rate of Fas antigen was 20. 0% and 87. 1% respectivelyin bcl-2 expressed and that of unexpressed of Liver-Stomach Disharmony, the expression rate of Fas antigen were significant higher in bcl-2 unexpressed of Liver-Stomach Disharmony than that of expression. HComparison of the Positive Rat of Fas between the bcl-2 Expression and Unexpression of Sleen-Stomach Damp-heatThe expression rate of Fas antigen was 31.6% and 91.7% respectively in bcl-2 expressed and that of unexpressed of Liver-Stomach Disharmony, the expression rate of Fas antigen were significant higher in bcl-2 unexpressed of Sleen-Stomach Damp-heat than that of expression. (P<0. 01). 3. 12Comparison of the Positive Rat of Fas Between the bcl-2 Expression and Unexpression of CSGThe expression rate of Fas antigen was 29.2% and 88.4% respectively in bcl-2 expressed and that of unexpressed of chronical superficial gastritis, the expression rate of Fas antigen were significant higher in bcl-2 unexpressed of chronical superficial gastritis than that of expression. (P<0.01), there is a significant correlation between the expression rate of bcl-2 antigen and that of Fas antigen (P<0. 01). 3. 13Comparison of the Apoptosis Index Between Liver-Stomach Disharmony and Sleen-Stomach Damp-heatThe apoptotic index were 12.60 + 4.87755 and 10.07 ± 2.94836 respectively in Liver-Stomach Disharmony and Sleen-Stomach Damp-heat, the apoptotic index in Liver-Stomach Disharmony group was higher than that in Sleen-Stomach Damp-heat group (P<0. 05).3.14Comparison of the Apoptosis Index Between HP Infected and Uninfected in Liver-Stomach DisharmonyThe apoptotic index were 15.27 ± 3.86879 and 10.82 ± 3.9315 respectively in Hp infected and un infected of Liver-Stomach Disharmony, the apoptotic index of Hp infected were significant higher than that of Hp uninfected in Liver-Stomach Disharmony (P<0.05).3. 15Comparison of the Apoptosis Index between HP Infected and Uninfected in Sleen-Stomach Damp-heatThe apoptotic index were 9.34 ± 2.72502 and 7.09 ± 2.71900 respectively in Hp infected and un infected of Sleen-Stomach Damp-heat, the apoptotic index of Hp infected were significant higher than that ofHp uninfected in Liver-Stomach Disharmony (P<0. 01).3.16Comparison of the Apoptosis Index between Hp Infected and UninfectedThe apoptotic index were 10. 7 + 3. 59921and 8. 06 + 2. 85386 respectively in Hp infected and uninfected of chronical superficial gastritis, the apoptotic index of Hp infected were significant higher than that of Hp uninfected (P<0. 05).3.17Comparison of the Apoptosis Index between Fas Expression and UnexpressionThe apoptotic index were 9. 96 + 3. 19449and 9. 78 + 2. 31749 respectively in Fas expressed and unexpressed, there is no significant difference between Fas expressed and unexpressed (P>0. 05).3. 18 The result of Multivariate logistic regression analysis for Sleen-Stomach Damp-heat3.18. llf take Sleen-Stomach Damp-heat as dependent variable, three independent variable bcl-2, HP and Fas enter the logistic equation, there is a positive correlation between bcl-2, HP and Sleen-Stomach Damp-heat. It means the possibility of leen-Stomach Damp-heat get higher with the positive rat of HP and bcl-2. there is a negative correlation between Fas and Sleen-Stomach Damp-heat. It means the possibility of leen-Stomach Damp-heat get lower with the positive rat of Fas. when there is a higher positive rat of HP and bcl-2 and a lower positive rat of Fas the possibility of leen-Stomach Damp-heat get higher.3.18.2 If take Liver-Stomach Disharmony as dependent variable, three independent variable bcl-2, HP and Fas enter the logistic equation, there is a negative correlation between bcl-2, HP and Sleen-Stomach Damp-heat. It means the possibility of leen-Stomach Damp-heat get lower with the positive rat of HP and bcl-2. there is a positive correlation between Fas and Sleen-Stomach Damp-heat. It means the possibility of leen-Stomach Damp-heat get higher with the positive rat of Fas. when there is a higher positive rat of Fas and a lower positive rat of HP and bcl-2 the possibility of Liver-Stomach Disharmony get higher. 4.ConclusionThe result suggest that there is a certain corelationship between hp infection and pattern type, the positive rates of HP is higher in Sleen-Stomach Damp-heat compared with Liver-Stomach Disharmony. There isa certain corelationship between the expression of bcl-2 and Fas in Sleen-Stomach Damp-heat, Liver-Stomach Disharmony may probably promote apoptosis in gastric, whereas, HP infection performs a main role in apoptosis in Sleen-Stomach Damp-heat. Three independent variable bcl-2, HP and Fas enter the logistic equation, there is a positive correlation between bcl-2, HP and Sleen-Stomach Damp-heat. It means the possibility of leen-Stomach Damp-heat get higher with the positive rat of HP and bcl-2. There is a negative correlation between Fas and Sleen-Stomach Damp-heat. It means the possibility of leen-Stomach Damp-heat get lower with the positive rat of Fas. when there is a higher positive rat of HP and bcl-2 and a lower positive rat of Fas the possibility of leen-Stomach Damp-heat get higher, three independent variable bcl-2, HP and Fas enter the logistic equation, there is a negative correlation between bcl-2, HP and Sleen-Stomach Damp-heat. It means the possibility of leen-Stomach Damp-heat get lower with the positive rat of HP and bcl-2. there is a positive correlation between Fas and Sleen-Stomach Damp-heat.It means the possibility of leen-Stomach Damp-heat get higher with the positive rat of Fas. when there is a higher positive rat of Fas and a lower positive rat of HP and bcl~2 the possibility of Liver-Stomach Disharmony get higher.
