| Objective:Vertigo has been one of the most seen diseases in urban cities in China and worldwide. The study is about the application of the combination of mainly the YunTong-three needles and Jing-three needles as dominance for treating Cervical Vertigo (CV). This is to investigate the clinical curative effect and clinical therapeutic evaluation in order to provide a foundation and standardize the acupoints indication of the remedy of CV.Method:80 subjects who were CV sufferers (patients) being selected to participate in the study. They were randomly picked to form two different categories: the Treatment Group (42 subjects) and the Control Group (38 subjects). The application of the dominant needles in the Treatment Group: the YunTong-three needles (acupoints: The Four Spirit Points + Yin Tang + Tai Yang) combine with the Jing-three needles (acupoints: Tian Zhu (BL10) + Bai Lao (Extra) + Da Zhu (BL11)), modified according to the symptoms of CV;whereas in the Control Group, traditional acupuncture manipulations were adopted and the acupoints are mainly Feng Chi (GB20) + Jing-Jia-Ji 2-7. All the 80 subjects in the two groups participated in this study for a duration of three courses of treatments. Six days a treatment with one day interval in between each course of treatment. Trans Cranial Doppler (TCD) Tests were adopted to the subjects and they were asked to fill in the "Cervical Vertigo Symptoms and Function Evaluation Scale (ESCV) " questionnaires for pre-initial treatment, post-initial treatment and post-3 course treatments;whereas curative effect evaluation was done based on 'The Standard of TCM Diseases and Symptoms Therapeutic Effect"(published in June, 1994 by the "State Administration of Traditional Chinese Medicine of People' s Republic of China" ).The data of TCD tests includes the "Systolic blood volume" (Vs), the "Mean of Blood flow volume" (Vm), "Diastolic blood volume" (Vd) as well as "Pulse index" (PI) in the Vertebral Artery (VA) and the Basilar Artery (BA). The ESCV includes 5 aspects: Vertigo, Shoulder pain and Cervicodynia, Cephalodynia, Daily life, Psychological ability and Social accommodating ability.Results:l.The result of the 80 subjects (patients) in the Treatment Group (42 subjects) treated by the application mainly of the Yun-Tong needles combine with Jing-Three needles for the initial treatment showed that 0 subjects were totally recovered, 14 subjects claimed to have significant improvement, 23 subjects claimed the treatment is effective and 5 subjects claimed the treatments were totally useless and remained the status quo, the total effective ratio was 88. 1%. Whereas the post-3 course treatments results showed that 20 subjects were totally recovered, 16 subjects claimed to have significant improvement, 6 subjects claimed the treatment is effective and 2 subjects claimed the treatments were totally useless and remained the status quo, the total effective ratio was 95. 24%. On the other hand in the Control Group with 38 patients, the results for the initial treatment showed that 0 subjects were totally recovered, 6 subjects claimed to have significant improvement, 8 subjects claimed the treatment is effective and 24 subjects claimed the treatments were totally useless and remained the status quo, the total effective ratio was 36.84%. By comparison, the results for the post-initial treatment of the Treatment Group has significant difference with the Control Group (P<0.01) in terms of total effective ratio. Whereas the post-3 course treatments results showed that 3 subjects were totally recovered, 10 subjects claimed to have significant improvement, 10 subjects claimed the treatment is effective and 15 subjects claimed the treatments were totally useless and remained the status quo, the total effective ratio was 60.53%. By comparison, the results for the post-3 course treatments of the Treatment Group has significant difference with the Control Group (P<0.01). Therefore the probability of the prolong prognosis of CV patients after 3 courses oftreatments seems to be significant higher in the Treatment Group than in the Control Group (P<0. 001).2.After the post-initial treatment and post-3 course treatments, all the 80 subjects participated in either the Treatment Group or the Control Group have shown improvements in certain extents and degree in the TCD results. However, the number of patients with normal blood flow speed after the post-initial and post-3 course treatments in the Treatment Group is extensively higher than those in the Control Group (P<0. 01). And the more and important fact is the changes among the CV patients in the Treatment Group is more enormous significant (P<0.001).3. From the study of the ESCV questionnaires, it pointed out that the 5 aspects no matter in Vertigo, Shoulder pain and Cervicodynia, Cephalodynia, Daily life, Psychological ability or Social accommodating ability, all indicated significant improvement among the CV patients by comparing the results gained for the pre-treatment, the post-initial and post-3 course treatments. Among these five aspects, vertigo improvement scores the highest. The results of the questionnaires showed: the most noticeable improvement in the CV patients in the Treatment group in every aspect and index after 3 courses of treatments (P<0.001);while the CV patients in the Control Group only found significant improvements in vertigo, Shoulder pain and Cervicodynia, psychological ability and Social accommodating ability aspects only (P<0.05). And the results of the scores indicated in the questionnaire in all index and aspects was much higher in the Treatment Group than in the Control group for the post-initial and post-3 course treatments (P<0. 01), and more importantly, all aspects and index after the 3 courses of treatments of the Treatment Group compared to the Control Group had even more significant difference (P<0. 001). We found out that the healthy status and clinical curative effectiveness of the CV patients could be evaluated more all-roundedly and objectively by adopting the ESCV questionnaires.Conclusion:l.From the studies above, the application of the combination of the acupoints dominant by the Yun-Tong needles and the Ting-Three needles revealed an immediate and short-term relives of the CV symptoms. Furthermore, the treatments could significantly improve the Vertigo, Shoulder pain andCervicodynia, Cephalodynia, Daily life, Psychological ability as well as Social accommodating ability of the CV patients, These results indicated that the combination of the acupoints dominant by the Yun-Tong needles and the Jing-Three needles is one of the reliable model remedy of the CV clinical treatments.2. The application of the combination of the acupoints dominant by the Yun-Tong needles and the Jing-Three needles showed improvement in the blood flow supply of the VA and VB of the CV patients after treatments.3. The study arouses the sense of recognition towards the acupuncture treatments of the CV patients. |
