BackgroundAcne is a common disease in dermatology, high incidence rate, involving before and after puberty by all kinds of age stage. The disease often repeated attacks, It has been proved by clinical practice that compound traditional Chinese medicine can comprehensive adjustment of body function, so as to achieve good results. Qingre Chushi decoction is often used for treating acute and damp heat of skin disease. For the treatment of damp heat type acne,it also achieved good clinical efficacy.The cause of acne is complex, involved in the pathogenesis of genetic, immune, endocrine, microbial infection, hair follicle keratosis of the sebaceous gland ducts and other various factors. There is still a lack of Chinese medicine treatment in the repair of skin barrier function and skin physiological indicators improved in the aspect of research. This study based on improving the skin micro environment by this kind of traditional Chinese Medicine.Research objectives(1) to observe improvement in acne lesion and TCM syndromes,evaluation of its clinical efficacy, and promote clinical application.(2) According to the results of physiological indexes of skin of acne patients, providing data support for the treatment of acne with this kind of traditional Chinese Medicine.Research method(1) clinical efficacy and safety evaluationThis study adopts randomized, controlled trial design, observation of the clinical efficacy and safety of Qingre Chushi Decoction in the treatment of damp heat type acne. To ratio of 2:1 were randomly divided into 72 cases of all patients, the test group 48 cases, given Qingre Chushi decoction treatment,24 cases in the control group, treated with Danshentong capsule. Course of treatment was 4 weeks. After treatment, by comparing the acne lesions (including the extent of lesions, quantity, color, swelling and pain) and syndrome (including lesions score and syndromes of TCM). Evaluated the side-effects in the two groups.(2) determination of skin physiologyUsing VISIA facial image analysis system and SOFT5.5 skin detector measured by facial red zone, brown spots, pores, porphyrin and water content of the skin, skin oils, pH value, to reveal the correlation between the improvement of specific skin physiological indexes and the treatment of traditional Chinese Medicine.(3) Study on antibacterial activity of TCMA variety of microorganisms exist in hair follicles, sebaceous glands of acne patients. inhibit its growth can make acne remission.To study the inhibitory effect of different concentrations of Qingre Chushi decoction on P.acne, and to provide laboratory basis for clinical application.Research results(1) clinical efficacy and safety evaluation resultsRecovery rate of the experimental group is 16.28%, explicit efficiency is30.23%, effective rate is 39.53%, non efficiency is 13.95%, total effective rate is 86.04%. Recovery rate of the control group is 9.52%, explicit efficiency is 19.05%, effective rate is28.57%, non efficiency is 42.86%, the total efficiency is 57.14%. The results showed that, Qingre Chushi decoction is better than the control group in improving the skin lesions.By statistical analysis, the skin lesions in the experimental group, the regression rate and syndromes treatment efficiency respectively 62.86% and 62.41%, higher than the control group, skin lesions subsided rate and syndromes treatment efficiency, with statistical significance, the results show that Qingre Chushi Decoction is better than that of control group in improving skin lesions and syndromes.(2) the results of skin physiological indexesThe experimental group of red and brown spots were less than the control group, the results were statistically significant (P< 0.05). Before and after the treatment of porphyrins and the pores of the scores decrease was not statistically significant, indicating that two groups of drugs is not obvious in the improvement of porphyrin and pores effect. After treatment,The skin fat content of experimental group is 18.74+10.263, compared with the control group (23.90+12.498) values to reduce.(3) the results of the study on the antibacterial activity of TCMExperimental study on inhibition by Qingre Chushi decoction. Results showed that: after 72h anaerobic culture, there is bacterial growth in the control tube. The test tubes of Qingre Chushi Decoction from 1:2 times dilution, the results showed that the growth of bacteria was observed in the concentration of 500mg/ml. MIC of Qingre Chushi decoction is 500mg/ml. The colony count is 1500CFU/ml.Conclusion(1) Qingre Chushi decoction has good effect in treating damp heat type acne. It is better than the control group in improving the condition of skin lesion and syndrome, which is worthy of clinical application.(2) Qingre Chushi decoction has a certain effect on improving the skin moisture, oil concent, red area and brown spot of acne patients. It is suggested that we should pay attention to the protection and repair of skin barrier in the course of clinical work.(3) At a concentration of 500 mg/ml,we see the results of bacterial growth, indicating that the weak effect of the drug for the inhibition of P.acnes. By further tests,there is inhibitory effect on the P. acnes by Qingre Chushi decoction. The colony count is 1500CFU/ml, which is obviously less than the number of bacteria in the negative control. The reason may be related to the drug’s overall adjustment, there is a little inhibitory effect to the pathogenic bacteria on the surface of the skin by oral medication simply.
