Purpose:Stroke is a common disease with high morbidity,recurrence rate,disability and fatality rate.In recent years,the age of onset tends to be younger.After stroke,patients often have different degrees of dysfunction,among which the degree of dependence on walking assistance and whether they can walk independently affect the psychology of patients,directly determine whether patients can return to family,work and society,affect the quality of life of patients and the happiness index of the whole family.After stroke patients with hemiplegia actively cooperate with rehabilitation treatment can effectively improve their walking function.Currently,most of the rehabilitation therapy techniques applied in clinical practice are based on the functional disorders of patients.Through the principle of neuroplasticity,neuromuscular manipulation is applied to induce the appearance of patients’ functions.Most of them focus on rehabilitation training of trunk and limb functions,and lack of overall posture evaluation.The discovery of posture secret(DPS)is a new rehabilitation therapy technology that starts from the patient’s posture,corrects the joint that causes the patient’s abnormal posture,and on the basis of the corresponding manipulation correction,carries out strength training and stretching on the relevant muscles,and then supplemented by sensory stability and sensory control training,in order to restore the patient’s normal neuromuscular control.In addition,the discovery of posture secret has achieved good therapeutic effect in the fields of neck,shoulder,low back and leg pain,but it is rarely applied in the research of walking function in stroke.In this study,the therapeutic effect of the discovery of posture secret on walking function in patients with hemiplegia after stroke was observed.Methods:Forty stroke patients with hemiplegia who were hospitalized in rehabilitation medicine department of affiliated hospital of Qingdao university from august 2022 to november 2022 and met the inclusion criteria of this study were selected and divided into treatment group and control group according to random number table method,with 20 cases in each group.The control group received routine rehabilitation treatment for 50 minutes a day,and the treatment group received routine rehabilitation treatment for 40 minutes a day,combined with the application of the discovery of posture secret for 10 minutes.The two groups received treatment once a day,5 times a week,for a total of 3 weeks.Before treatment and 3weeks after treatment,The absolute value of the vertical distance between the inner upper angle of the scapula and the second spinous process of the thoracic spine(shoulder height),the angle of pelvic incline(the angle between the anterior superior iliac spine and posterior superior iliac spine,that is the angle between ASIS and PSIS),the lower limb Fug l-Meyer motor function assessment(FMA-LE),the "stand up and walk" timing test(TUGT),and the stride length(affected side)and step frequency of the patients in the gait parameters were evaluated in this 2 groups.Results:Before treatment,there were no statistical differences in shoulder height,the angle between the anterior superior iliac spine and posterior superior iliac spine(the angle between ASIS and PSIS),FMA-LE score,TUGT,step length(affected side)and step frequency in gait parameters between the two groups before treatment(P>0.05),which was comparable.After 3 weeks of treatment,shoulder height,the angle between ASIS and PSIS,FMA-LE score,TUGT and step length(affected side)of patients in 2 groups were improved compared with before treatment.The improvement of above evaluation contents in the treatment group was better than that in the control group at the same period,with statistical significance between 2 groups(P<0.05).In addition,after treatment,the step frequency in both groups was significantly increased(P<0.01),but there was no statistical significance between the two groups(P>0.05).After 3 weeks of treatment,the conversion rate of ASIS and PSIS in the 2 groups was higher in the treatment group,that is,the improvement was more obvious in the treatment group,with statistical significance(P<0.05)..Conclusion:The application of conventional rehabilitation therapy combined with DPS can more effectively improve the walking function of stroke patients with hemiplegia.Therefore,manual therapy based on the discovery of posture secret should be actively promoted in the walking function rehabilitation of stroke patients with hemiplegia.
