Purpose:Periarthritis of shoulder is a common clinical disease,often manifested as shoulder and surrounding pain,range of joint movement is limited,and then affect daily life.At present,many clinical conservative treatments are given priority,its contains manual therapy,it has a unique effect on the periarthritis of shoulder,the patient easy to accept,and easy to implement,there are differences between different treatment,discovery of posture technique is a new manual technique for the treatment of musculoskeletal pain.In this research to observe the clinical effect of discovery of posture technique on periarthritis of shoulder in adhesion stage.Methods:Thirty patients with periarthritis of shoulder in adhesion stage were selected from the outpatient department of rehabilitation medicine of Affiliated Hospital of Qingdao University from August 2020 to May 2021 and met the inclusion criteria.According to the random number table method,they were divided into observation group(15 cases)and control group(15 cases).Patients in the observation group were treated with discovery of posture technique,while patients in the control group were treated with traditional massage technique.Each treatment lasted for 30 minutes,twice a week,for a total of 2 weeks.Visual analog scale(VAS),pressure pain threshold(PPT),range of motion(ROM),constant murley score(CMS),and absolute vertical distance difference between the upper Angle of scapula and the second spinous process of thoracic vertebra were measured before treatment,after once treatment,after four treatments and one month after treatment,the acromio humeral interval(AHI)was assessed before treatment and after 4treatments.Results:There were no significant differences between VAS score,CMS,ROM,muscle tenderness threshold(smaller pectoral muscle,supraspinatus muscle,deltoideus triangularis,subscapularis),absolute vertical distance difference between the upper angle of scapula and the second spinous process of thoracic vertebra and AHI values(P> 0.05).Comparison within groups,compared with before treatment,VAS scores in 2 groups were decreased after once treatment,after four treatments,and one month after treatment(P <0.05),CMS,ROM,smaller pectoral muscle,supraspinatus muscle,deltoideus triangularis tenderness threshold were increased compared with before treatment(P < 0.05).The subscapularis muscle tenderness threshold of the observation group at each time point was increased compared with before treatment,there was no statistical significant in the control group compared with before treatment(P > 0.05).The absolute vertical distance difference between the upper angle of scapula and the second spinous process of thoracic vertebra of the observation group at each time point was decreased compared with before treatment(P<0.05),there was no statistical significant in the control group compared with before treatment(P > 0.05).After four treatments,the AHI value in the observation group was increased compared with before treatment(P < 0.05),there was no statistical significant in the control group compared with before treatment(P > 0.05).Comparison between groups,there were no statistical significant in VAS score,CMS,smaller pectoral muscle,supraspinatus muscle,deltoideus triangularis tenderness threshold between the two groups after once treatment(P > 0.05).After four treatments and one month after treatment,the VAS score of the observation group was decreased compared with control group(P < 0.05),CMS,smaller pectoral muscle,supraspinatus muscle,deltoideus triangularis tenderness threshold were increased compared with control group(P < 0.05).The ROM of the observation group at each time point was increased compared with control group(P<0.05).The subscapularis muscle tenderness threshold of the observation group at each time point was increased compared with control group(P < 0.05).The absolute vertical distance difference between the upper angle of scapula and the second spinous process of thoracic vertebra of the observation group at each time point was decreased compared with control group(P<0.05).After four treatments,the AHI value in the observation group was increased compared with control group(P<0.05).Conclusion:Compared with the traditional massage,discovery of posture technique can effectively change adhesion periarthritis of shoulder symptoms,relieve muscle pain,improve joint mobility,restoring shoulder brachial rhythm,make the scapula better return to neutral,lasting treatment effect.Therefore this new manual technique should be widely used in clinical treatment of musculoskeletal pain.
