| Objective: Through clinical observation and questionnaire survey,this study explored the related factors of intermenstrual bleeding(intermenstrual bleeding,IMB)of liver depression and kidney deficiency from the aspects of social demography and eating habits,and obtained a scientific guidance plan to reduce the risk of disease as much as possible;By observing the clinical efficacy of the randomized controlled trial of“ Zishen Dingjing Decoction ” and “ Baogong Zhixue granule ”,the therapeutic effect of Zishen Dingjing Decoction was preliminarily discussed,which provides a practical and reliable basis for the clinical application and popularization of Zishen Dingjing Decoction,and expands a new train of thought and direction for the treatment of intermenstrual bleeding of liver depression and kidney deficiency.Methods: According to TCM syndrome differentiation,115 patients with intermenstrual bleeding of liver depression and kidney deficiency and 115 healthy female volunteers without intermenstrual bleeding were selected.The general situation,age of menarche,reproductive history,living and eating habits were collected,and the related factors were investigated and analyzed;70 patients with intermenstrual bleeding of liver depression and kidney deficiency were randomly divided into experimental group(n=35)and control group(n=35).Observe the clinical comprehensive effect and hemostatic effect,bleeding time,total syndrome score of TCM,main and secondary symptoms score of each single TCM,and basic body temperature of the test group and the control group,statistically analyze the differences between the two groups before and after treatment,evaluate the treatment effect,and analyze the therapeutic effect of Zishen Dingjing Decoction.Results: 1.Univariate difference analysis: The pressure of study and work,smoking secondhand smoke,physical exercise,sleep quality,weight loss mode,adequate daily drinking water,eating bean products,taste preference,mental mood,oral emergency contraceptives were correlated with intermenstrual bleeding of liver depression and kidney deficiency type.2.Multivariate Logistic regression analysis showed that the protective factors of intermenstrual bleeding of liver depression and kidney deficiency type(OR<1,P<0.05)were regular physical exercise,good sleep quality,drinking enough water every day,and the risk factors of intermenstrual bleeding of liver depression and kidney deficiency type(OR>1,P<0.05)were: high pressure of study and work,often smoking secondhand smoke,dieting to lose weight,spicy taste,mental tension and anxiety,and often taking emergency contraceptive.3.Analysis of curative effect: The total effective rate of the clinical comprehensive efficacy of the test group and the control group were 91.43%and 71.43% respectively,and the total effective rate of the hemostatic efficacy were 91.43% and 77.14% respectively.The difference in the total effective rate between the two groups after treatment was statistically significant(P<0.05).The two groups had a good effect in improving the bleeding time and the total TCM syndrome scores,and the bleeding days and scores were decreased(P<0.05).Compared with the control group,the experimental group was more dominant in all aspects(P<0.05).After treatment,the scores of main TCM symptoms and secondary symptoms of each single item in the two groups were decreased(P<0.05),and the relief of chest tightness,sore waist and knees,five-heart fever,irritability or depression,dry stool,short yellow urine in the test group was more obvious than that in the control group(P<0.05).The two groups were similar in the treatment of dizziness and tinnitus,breast pain and abdominal pain(P>0.05).Both the experimental group and the control group could regulate the change of basal body temperature,and the curative effect of the experimental group was more significant(P<0.05).4.During the follow-up after drug withdrawal,the recurrence rates of the test group and the control group were 9.38% and 44% respectively,and there were significant differences between the two groups(P<0.05).Conclusions: 1.In the prevention and treatment of intermenstrual bleeding of liver depression and kidney deficiency type,We should pay attention to strengthening physical exercise,maintaining good sleep quality,drinking enough water every day,reasonably planning the study and work arrangement,dispersing the study and work pressure,trying to avoid smoking second-hand cigarettes and dieting to lose weight,correcting the habit of preferring spicy food,adjusting the mental mood of tension and anxiety,and reducing the use frequency of oral emergency contraceptives.2.Zishen Dingjing Decoction has a good effect in treating patients with intermenstrual bleeding of liver depression and kidney deficiency type,which can obviously relieve TCM symptoms,improve bleeding,improve luteal function and regulate basic body temperature.Various clinical indexes are more advantageous than Baogong Zhixue granule,and the recurrence rate is low after treatment,and the long-term clinical effect is significant. |
