Objectives(1)Based on the theoretical model of stress and coping model,this study analyzes the current situation of disability acceptance of traumatic amputees;(2)To explore the effect of social psychological factors on the disability acceptance of traumatic amputees;(3)The structural equation model of post-traumatic stress disorder,social support,family care,self feeling burden,patient coping style and patient disability acceptance was constructed to provide scientific basis for improving patient disability acceptance and improving patient prognosis.Methods(1)Set variables:the patient’s disability acceptance is the outcome variable,the pre variable is social support,family care,self perception and individual stress response,and the intermediary variable is coping style.(2)Determine the theoretical framework:This study is based on the theoretical framework of stress and coping model of American psychologist Richard S.Lazarus.(3)Research object and questionnaire:Using the convenient sampling method,we selected the traumatic amputation patients admitted from December 2019 to December 2021 in three Grade A hospitals in Jinan,Shandong Province as the research objects.The questionnaire is divided into 8 parts,including 6 scales,including questionnaire description,general information and disability acceptance scale(ADS),post-traumatic stress disorder scale(PCL-C),medical coping style scale(MCMQ),Chinese version of self perceived burden scale(SPBS),social support rating scale and family care index(PAGAR),which are collected online through questionnaire stars.(4)Statistical methods:①Statistical analysis using SPSS 23.0 software;②Cronbach coefficient was used as the reliability test index to test the reliability of each dimension of the scale;③ K-S test method is used to test the normality of measurement data.Measurement data conforming to normal distribution are expressed in the form of "mean ± standard deviation",and counting data are expressed in the form of "percentage(%)";④ Pearson correlation analysis is adopted for the correlation analysis of measurement data with normal distribution α=0.05 is the inspection level;⑤The Bootstrap method in SPSS Process plug-in was used to sample 5000 times to test the intermediary effect of intermediary variables,and AMOS 24.0 software was used to construct and test the structural equation model.Results(1)A total of 246 patients were enrolled in this study,and the total score of disability acceptance was(80±6.43).The disability acceptance was negatively correlated with stress disorder,self perceived burden and surrender,and positively correlated with social support,family care,facing and avoidance.(2)The results of linear regression analysis of uncorrected confounding factors showed that the total scores of face,avoidance,social support and family care had significant positive effects on disability acceptance,while the total scores of yield,stress and feeling burden had significant negative effects on disability acceptance,and the effects were statistically significant(P<0.05).(3)The results of linear regression analysis after correcting the confounding factors showed that:facing,avoiding,yielding,the total score of stress disorder,the total score of feeling burden,the total score of social support,and the total score of family care all had significant effects on disability acceptance,The model was established as:Y=64.977+0.283*Facing+0.443*Avoiding+0.166*Yielding+(-0.143)*Total Score of Stress Disorder+(-0.37)*Total Score of Perceived Burden+0.362*Total Score of Social Support+1.036*Total Score of Family Care.(4)① Face and avoidance play a significant mediating role in the influence of stress disorder and perceived burden on disability acceptance;② There was no intermediary effect of yielding on the influence of stress disorder and perceived burden on disability acceptance;③ Face and avoidance play a significant intermediary role in the influence of social support and family care on disability acceptance,while surrender plays a significant intermediary role in the influence of social support on disability acceptance;④ Yield has no intermediary effect on the influence of family care on disability acceptance.(5)In path analysis:① Beneficial factors(i.e.social support and family care)have negative effects on negative factors(i.e.stress disorder and perceived burden);② Beneficial factors have a positive effect on coping styles,with the effect of 0.55;③ Negative factors have negative effects on coping styles,with the effect of-0.86;④ Coping style has a positive impact on disability acceptance,with an impact of 0.76.The analysis results of each path of the model were significant(P<0.05),and each fitting index reached the ideal value.The final model fitting results were good,indicating that the model was in good agreement with the observation data,and the model was established.Conclusion(1)The disability acceptance of traumatic amputation patients is in the middle level;(2)The disability acceptance of traumatic amputees was negatively correlated with the negative factors in this study,namely,stress disorder and self perceived burden;(3)The disability acceptance is positively related to the beneficial factors in this study,namely,social support and family care,and social support and family care can directly significantly improve the disability acceptance of trauma amputees;(4)The beneficial factors in this study can also significantly reduce the negative factors of patients,so as to indirectly promote patients to take positive coping measures such as face to face equivalence,and ultimately improve patients’ disability acceptance. |
