ObjectiveBy investigating the burden level of home caregivers for patients with traumatic amputees,the influencing factors of the burden on family caregivers of traumatic amputees are explored,and corresponding measures and solutions are proposed in order to improve the caregiver's self-care ability and to reduce trauma.The family caregiver burdens the amputee and provides scientific evidence.MethodsThis study selected the home caregivers of 120 patients with amputees from Department of Orthopedics and Hand and Foot Surgery at the First Hospital of Jilin University from July 2016 to December 2017.Self-made scales were used to investigate general data of amputees and their family caregivers.The Chinese version of the ZBI scale and caregiver response assessment scale(CRA scale)were used to assess the burden on home caregivers of patients with traumatic amputees.The(SOC-13)scale assesses the level of psychological cohesion among home caregivers of patients with traumatic amputees.Use SPSS21.0 for statistical analysis.Descriptive analysis and frequency statistics were used for general data description;Measurement data were expressed as(?x±s);Differences in self-care ability,amputation sites,and psychological status were compared using variance analysis and multiple test;Pearson correlation analysis was conducted to analyze the correlation between variables;the family caregiver burden score was used as the dependent variable,and the amputee's self-care ability,amputation site,and psychological status were used as independent variables for multiple linear regression analysis.Results1?Of the 122 questionnaires issued in this survey,120 valid questionnaires were recovered and the effective rate was 98.36%.2?The overall score of family caregivers of amputated patients is 51.68±17.93,all family caregivers have burden,35.8% is at a mild level,24.2% is at a moderate level,40% is at a severe level.3?There are significant differences in the scores of family caregivers among different amputation sites,medical cost sources,psychological status and self-care ability(P<0.05).4?The Pearson correlation analysis showed that family caregivers of patients with amputation reaction score and family caregiver burden score significantly correlated(r=0.598,P<0.01);family caregivers of patients with amputation of sense of coherence scores and family caregiver burden score had negative correlation(r=-0.393,P<0.01).5?Multiple linear regression analysis showed that there was a linear regression relationship between the psychological state and the burden of family caregivers(?=8.201,t=3.156,P<0.05).There was a linear regression relationship between self-care ability and family caregiver burden(?=4.516,t=2.612,P<0.05).Multiple stepwise regression analysis showed that "psychological state" and "self-care ability" entered the regression model of family caregivers' burden on amputated patients.Conclusions1?Family caregivers of amputees have varying degrees of burden.2?The influencing factors of amputee family caregiver burden include: amputation site,source of medical expenses,mental health,self-care ability of amputee,caregiver response and psychological coherence.3?The psychological coherence of amputee family caregivers was negatively correlated with the burden of family caregivers.Home caregivers can reduce psychological stress and burden by improving comprehension,control,and sense of meaning.4?The relevant social departments and medical workers should take relevant countermeasures and nursing measures to reduce the burden on home caretakers of amputees.
