Objective:To abserve the clinical efficacy of treating early swelling and pain of middle-aged and elderly patients with calcaneal fracture(qi and blood stasis syndrome)based on conventional detumescence treatment with Traditional Chinese medicine for Regulating qi and promoting blood circulation,to further provide some treatment evidence in clinical practice.Methods : 60 paients with calcaneal fracture in this research were hospitalized in the department of orthopedics of Weihai central hospital during February 2021 to December 2021,including 42 males and 18 females.Patients in control group were only treated by the conventional treatment,including:1.local immobilization,evelation of affected limb,strict bed rest.2.Local ice was appilied every 6 hours after admission,treated with injection of Mannitol 125 ml every 12 hours,and ketorolac ambutritol injection was given intravenously for analgesic treatment.Patients in the treatment group were treated with conventional treatment and Traditional Chinese medicine as the core of Regulating qi and promoting blood circulation theory.Pain socre and heel/ankle circumference were recorded before treatment and after treatment respectively,and the ratio of swelling regression was recorded.At the same time,the day of wrinkle test is positive,tension blisters and skin ecchymosis were recorded.All clinical date were analysis by SPSS 25.0.Results:1.This data of the patients in this research before treatment,such as gender,age,fracture type,showed no statistical difference(P>0.05),and the comparison was statistically significant;2.Swelling degree of the affected limb : There was no significant difference in detumescence effect between the experimental group and the control group before treatment and 1-3 days after treatment(P > 0.05).There was significant difference in detumescence effect between the experimental group and the control group after 5 days and 7 days of treatment(P <0.05).Intra-group comparison,there were significant differences in the degree of limb swelling between the two groups before and after treatment(P< 0.05).The results showed that both the experimental group and the control group can play a detumescent effect,but the experimental group is better than the control group;3.Affected limb pain VAS score:There was no significant difference in pain scores between the experimental group and the control group before and after one days of treatment(P > 0.05).There were significant differences in pain relief effect between the experimental group and the control group after3 days,5 days and 7 days of treatment(P < 0.05).Intra-group comparison,there were significant differences in pain scores between the two groups before and after treatment(P < 0.05).The results showed that both the experimental group and the control group could relieve pain,but the experimental group had better effect than the control group;4.Comparison of waiting time for surgery: Compared with the control group,the first time of skin wrinkly appearance and time of operation conditions in the experimental group were earlier than those in the control group,with statistically significant differences(P > 0.05);5.Perioperative complications: The incidence of tension blisters and phlebothrombosis in the experimental group was less than that in the control group,with statistical significance(P < 0.05).No deep vein thrombosis and obvious adverse reactions occurred in both groups.Conclusion:This research confirmed that the self-designed Liqi Huoxue decoction under the guidance of the treatment principle of Regulating qi and promoting blood circulation can reduce the calcaneal fracture early swelling and pain,shorten the preoperative detumescence period,reduce the hospitalization period and relieve the pain of patients,and the clinical curative effect is distinct and safe. |
