Objective:This study combines the screening of high-risk population of stroke in community with the study of TCM syndromes to study the distribution of risk factors of high-risk population of stroke,and to analyze the distribution of TCM syndromes of high-risk population and the correlation between risk factors of stroke and TCM syndromes,so as to provide reference basis for the prevention of stroke by traditional Chinese medicine.Methods:The subjects came from three communities:Shiliulihe Community,Nanxinzhuang Community and Zhenxing Street Community in Jinan.The high risk of stroke screening and basic clinical information of community residents were investigated through the 2020 Community and Township population screening Table for risk factors of Cardiovascular and Cerebrovascular Diseases,and the screened high-risk groups were further studied through the Ischemic Stroke Syndrome Diagnostic scale.The level monitoring of stroke and its risk factors based on community population was carried out by the combination of questionnaire,physical examination and four diagnosis of traditional Chinese medicine,and discussed and analyzed to grasp the exposure level and distribution characteristics of the main risk factors.Record and study the TCM syndrome characteristics of the population.EXCEL is used to record and manage data,and SPSS23.0 software is used to analyze the data.Results:1.A total of 583 people at high risk of stroke were included in this study,including 274 males and 309 females,with an average age of63 years old.In the high-risk group of stroke,hypertension was the most common risk factor,with a detection rate of 72.0%,followed by dislipidemia with a detection rate of 69.1%,followed by lack of exercise(50.4%)and obvious overweight or obesity(48.7%).This was followed by smoking(34.6%),family history of stroke(29.5%),diabetes(28.3%),atrial fibrillation or valvular heart disease(11.8%).2.The most common syndrome in this screening is phlegm-dampness syndrome,accounting for 33.8%,followed by qi deficiency syndrome,internal fire syndrome,internal wind syndrome,yin deficiency syndrome,blood stasis syndrome,and 14.9% of the samples have no obvious syndrome.3.In the age distribution of stroke prevalence,the stroke prevalence rate is the highest in people aged 70-79,and there is a close positive correlation between stroke and age in the age range of 40-79 years old.4.According to binary Logistic regression analysis,advanced age is the risk factor of yin deficiency and protection of internal fire;dietary taste is the risk factor of internal wind;drinking is the risk factor of internal wind and internal fire;hypertension is the risk factor of internal fire;dislipidemia is the risk factor of phlegm-dampness;atrial fibrillation or valvular heart disease is the risk factor of qi deficiency and yin deficiency;smoking is the risk factor of internal wind and internal fire.Obvious obesity or overweight is the risk factor of phlegm-dampness syndrome,the protective factor of internal wind and internal fire,and the lack of exercise is the risk factor of qi deficiency.Family history of stroke is a risk factor without obvious syndrome;stroke is a risk factor of qi deficiency syndrome and a protective factor of internal fire;TIA is a risk factor of blood stasis and yin deficiency.Conclusion:1.In this survey,the high risk group of stroke is mainly middle-aged and elderly.Hypertension was the most common risk factor in the sample,with a detection rate of 72%.Secondly,dislipidemia,lack of exercise,overweight or obesity,smoking,family history of stroke,diabetes,atrial fibrillation or valvular heart disease,the constituent ratio of high risk factors is basically consistent with other surveys in the country and the province.the detection rate of hypertension is slightly higher than the national data,and there is a greater difference compared with the research in East and South China,which may be related to the habit of high-salt diet in Shandong.One of the important ways to prevent stroke is to strengthen the screening of stroke high-risk groups,establish a stroke prevention system at all levels,and actively control risk factors.2.The most common syndrome in this screening was phlegm-dampness,followed by qi deficiency,internal fire,internal wind,yin deficiency and blood stasis.14.9%of the samples had no obvious syndrome.Men are more likely to have internal fire than women due to physical fitness,diet and other reasons.Due to similar reasons,middle-aged people are more likely to have internal fire than the elderly,while the elderly are more likely to have qi deficiency.It can be seen that phlegm and dampness is the main pathogenic factor in the pathogenic process of stroke,and the syndrome characteristics of different groups are also different,so when using traditional Chinese medicine to prevent and treat stroke,we should fully consider the syndrome characteristics of high-risk groups.3.Lifestyle factors such as age,smoking,drinking and obesity are more likely to be related to TCM syndromes,and there are different correlations between different high risk factors and TCM syndromes.When managing and controlling relevant risk factors,grasping the objective influence of these related factors on TCM syndromes can carry out syndrome dialectics more quickly and accurately so as to carry out targeted guidance and TCM intervention according to the characteristics of TCM syndromes.The promotion of a healthy lifestyle is also a top priority.
