Screening Of High-Risk Population For Stroke In The Working Physical Examination Population In Luzhou And Analysis Of Influencing Factors

Objective: By screening the high-risk stroke risk of the working population who participated in the physical examination in Luzhou,we can find out the distribution of related risk factors among the working population at high risk of stroke in this region,and analyze the influencing factors of the working population at high risk of stroke.We propose feasible interventions and management measures for the high-risk population,and provide scientific basis and theoretical support for subsequent health management and comprehensive interventions for the high-risk population.Methods:Through the convenience sampling method,three hospitals were selected in Luzhou city,Sichuan Province from June to November 2020,and the working population aged 40~60 years old who came to the hospital’s physical examination center for physical examination in the name of the unit and group were screened for high-risk stroke groups.With the consent of the respondents,the self-designed "Registration Form for Comprehensive Management of Chronic Diseases(Stroke Prevention and Treatment)" was used to conduct face-to-face questionnaires on the working population to collect relevant physical examination data.SPSS 26.0 software was used for data analysis and processing.Descriptive analysis was used for count data such as frequency and composition ratio,continuous data was expressed as SX ±,χ2 or χ2 trend test was used for univariate analysis,and multivariate logistic regression model was used for multivariate analysis.Results:(1)There are 2903 working people in this study,35 stroke patients,with a stroke prevalence rate of 1.2%.There were 1077 high-risk stroke populations screened,with a detection rate of 37.1%.(2)The risk factors of stroke screening in the high-risk population were dyslipidemia(86.1%),lack of exercise(80.4%),smoking(66.9%),hypertension(65.4%),and obesity.(28.4%),diabetes(22.4%),family history of stroke(6.1%),history of stroke(3.2%),atrial fibrillation or valvular heart disease(0.8%),and transient cerebral ischemia(0.7%).(3)After multivariate analysis,compared with men,the risk of stroke in high-risk groups is lower in women(OR=0.136,95%CI=0.101-0.182);OR=0.757,95%CI=0.637~0.915);Compared with the 40~age group,the50~ and 55~ age groups are higher(compared to 40~,50~:OR=1.439,95%CI=1.119~1.850,55~:OR=1.505,95%CI = 1.135~2.001).Compared with no family history of diabetes,those with a family history of diabetes have a higher risk of becoming a high-risk group of stroke(OR=1.576,95%CI=1.262~1.969);compared with no family history of hypertension,those with a family history of hypertension have a higher risk of becoming a high-risk group of stroke(OR=1.847,95%CI=1.539~2.217).Among different lifestyles intake and diets,compared with high salt intake,low and moderate taste intake were protective factors for high-risk stroke population(compared to high salt intake,moderate taste intake :OR=0.696,95%CI=0.562~0.861,low salt intake:OR=0.570,95%CI=0.431~0.755);compared with preference of meat,moderate meat and vegetarian food was a protective factor for high-risk stroke population(OR=0.740,95%CI=0.607~0.901).Compared with regular three meals,failure to eat three meals regularly is a risk factor for high-risk groups of stroke(OR=2.329,95%CI = 1.412~3.843);compared with the frequency of attending meals with guests <1 time/week,1-2 Times/week and ≥3times/week were risk factors for high-risk stroke population(compared to <1time/week,1-2 Times/week: OR=1.538,95%CI=1.249~1.895,≥3times/week : OR=1.490,95%CI=1.089~2.039);compared with the frequency of fruit intake ≥5 days/week,≤2 days/week,and 3~4 days/week were risk factors for high-risk stroke population(compared to ≥5 days/week,≤2days/week: OR=1.263,3~4 days/week :95%CI=1.010~1.579,OR=1.745,95%CI=1.424~2.139).Among stroke-related knowledge,compared with no knowledge of stroke aura,mastering the knowledge of stroke aura was a protective factor for high-risk stroke population(OR=0.826,95%CI = 0.687~0.994).Conclusion:(1)The detection rate of high-risk groups of stroke was high.(2)The detection rate of risk factors such as dyslipidemia,lack of exercise,and smoking in high-risk groups of stroke is relatively high.It is necessary to strengthen blood lipid detection,increase physical activity and advocate smoking cessation,so as to reduce the detection rate of high-risk groups.(3)Hypertension,obesity and diabetes are important risk factors affecting the incidence of stroke in the working population.It is necessary to control blood pressure,blood glucose and body weight through various methods such as lifestyle,diet and drugs.(4)The detection rate of the family history of stroke and atrial fibrillation or valvular heart disease are low in the high-risk population of working stroke,but it is still necessary to strengthen the screening and treatment of atrial fibrillation to reduce the risk of serious stroke.(5)It is necessary to pay attention to the male,elderly,low education level and family history groups of chronic diseases among the high-risk groups of stroke,advocate healthy eating methods such as regular meals,low salt,and increase the intake of fruits,and strengthen the publicity of stroke health knowledge,so as to reduce the risk of stroke.
