A Study On The Construction Of A Certification System For Online Nurses

ObjectiveThis study uses the certification process and the competency model as theoretical guidance,incorporates nurses’ views on online nurse certification,draws on family nurse competency indicators through a scoping review,and constructs an online nurse certification system after expert correspondence,with a view to providing a theoretical basis for online nurse certification.MethodA descriptive qualitative research method was used to conduct semi-structured interviews with 12 online nurses and 8 clinical nurses selected from three tertiary hospitals in Jinan,Shandong Province,to understand nurses’ perceptions of online nurse certification.In addition,a scoping review of the 32 included papers was conducted to draw on home nurse competency indicators,synthesis of research findings and group discussions were used to initially construct a pool of entries for the online nurse certification system,which was eventually established through two rounds of expert correspondence.Result1.This study analysed interview data from 12 online nurses and 8 clinical nurses and extracted 3 themes and 7 sub-themes.(1)positive utility of accreditation.1)protection and motivation of online nurses;2)improved quality of care and patient safety;(2)assessment elements of accreditation.1)qualification requirements for online nurses;2)competencies of online nurses;3)authority and scientific validity of accreditation bodies;(3)barriers to accreditation.1)the model is still in its infancy and lacks policy support;2)nurses are not sufficiently motivated and lack incentive mechanism.2.This study systematically searched Embase,Web of Science,The Cochrane Library,PubMed,VIP,Wanfang,China Knowledge Network and China Biomedical Database to conduct a scope review of family nurse competencies,and finally included 32 papers,extracting nursing procedure execution competencies,interpersonal and communication competencies,multidisciplinary 7 competencies such as nursing procedure execution competency,interpersonal and communication competency,multidisciplinary collaboration competency,resource management and allocation competency,career path planning competency,internet information literacy competency,and safety prevention competency.3.22 nursing experts were consulted by letter in this study,covering North,East,West,East and Central China,including both pilot and non-pilot areas for network nurses.The recall rates of the two rounds of expert consultation were 95.45%and 100%,respectively,with a high level of expert motivation,an authority coefficient of 0.881,a high level of expert authority,and coordination coefficients of 0.194,0.146 and 0.112,respectively,with a high level of coordination of expert opinion.The final draft of the online nurse qualification system,which contains 5 primary indicators,26 secondary indicators and 118 tertiary indicators,was formed.Conclusion1.The management should strengthen policy guidance and increase incentives to give full play to the effectiveness of certification in order to enhance nurses’ motivation.At the same time,the elements of certification should be improved and the qualifications of certification should be strictly carded in order to improve the competency of nurses and protect patient safety.2.Online nurses should have at least the ability to perform nursing procedures,interpersonal and communication skills,multidisciplinary collaboration skills,resource management and allocation skills,career path planning skills,network information literacy skills,and safety prevention skills.3.This study consults experts with high motivation,authority and coordination of opinions,and constructs a certification system that is scientific,comprehensive and practical.It can provide a theoretical basis for the certification of online nurses.
