Objective:Investigation and analysis of secondary and tertiary hospital nurses in our country for the treatment of venous specialist nurse training and management of the relevant requirements,and through the Delphi method to construct our country's intravenous treatment specialist nurse training and certification management mode,including training objectives,training process,training base,access conditions,teaching conditions,training methods,training time,training content,reference materials,rotary department,practice requirements),evaluation,certification and recertification management continuously,for our country and the cultivation of the intravenous treatment health nurses recertification management to provide theoretical reference.Methods:1.Literature analysis was used to understand the current situation of training and management of intravenous therapy nurses at home and abroad.2.By means of convenience sampling method,questionnaire survey was conducted on 684 nurses in 20 hospitals in and outside Shanxi Province to analyze the overall demand of nurses in secondary and tertiary hospitals for the training of specialized nurses in intravenous therapy,as well as the difference in the demand of nurses in different conditions for the training content.3.With the Delphi method,31 experts in the field of intravenous therapy and nursing management from 12 regions in China were selected for expert consultation,and finally the mode of training and certification management of intravenous therapy nurses wasdetermined.Results:1.1.Results of the questionnaire survey: nurses' requirements for the six training modules are from high to low: specialized knowledge of intravenous therapy and nursing,vascular access technical operation,basic medical knowledge,nursing management,introduction to nursing,and evidence-based scientific research teaching.Forty-seven of the 48 second-level training items(97.92%)had a significance score higher than4.Nurses with different conditions have different requirements for each training module(P<0.01,P<0.05).64.77% of nurses considered it very necessary and 34.21% considered it necessary to carry out intravenous therapy specialist nurse training.The top five requirements for nurses' admission conditions are as follows: whether to obtain nurse practice certificate(80.99%),years of working in clinical nursing(69.01%),years of working in intravenous therapy(67.11%),education background(47.95%),and professional title(45.91%).Theoretical training should be 2 weeks(39.47%)and clinical practice 3 months or longer(36.40%).Training in full-time mode(35.96%),and the implementation of practice tutor one belt and one road(93.86%);The first teaching method is case teaching method(89.47%).The top four departments in rotation were vein catheter outpatient department(87.57%),oncology department(77.63%),intensive care unit(73.25%)and hematology department(67.84%).The assessment methods approved by more than 50% of nurses include operational assessment(89.77%),theoretical assessment(89.04%)and daily assessment(53.95%).The qualification is considered valid for 5 years(53.07%).2.Delphi expert consultation results: the consulting experts in this study are from12 regions of China,with a wide range of distribution.They all have bachelor's degree or above and intermediate or above professional titles,including experts in the field of intravenous therapy and nursing management.The consulting experts are very representative.The effective recovery rates of the two rounds of expert consultationquestionnaires were 96.88%(31/32)and 100%(31/31),indicating that the experts were highly motivated.The authority coefficients of experts are 0.92 and 0.94 respectively,indicating that the degree of authority of experts is high and the research results are reliable.The judgment coefficient(Ca)is 0.95 and 0.96 respectively,and the degree of familiarity(Cs)is 0.89 and 0.92 respectively.The score range of each index in the first round of expert consultation was 3.58 ~ 5.00,the coefficient of variation(CV)was 0 ~0.37,and the full score was 12.90% ~ 100.00%.After the second round of expert consultation,the importance score range of each index was 4.03 ~ 5.00,the coefficient of variation was 0 ~ 0.21,and the full score was 22.58% ~ 100%.After two rounds of expert consultation,the coordination coefficients of the first,second and third-level indexes were 0.201,0.200 and 0.319,respectively,and all were higher than that of the first round.The coordination coefficients of the indexes at all levels of the two rounds of expert consultation were statistically significant(P<0.001),indicating that the coordination degree of expert opinions was relatively high.After two rounds of Delphi expert consultation,the mode of training and certification management for specialized nurses in intravenous therapy was finally determined,including 5 first-level indicators,24second-level indicators and 90 third-level indicators.Conclusion:This study through the literature review method,questionnaire survey method and Delphi method,preliminary build a systematic and comprehensive intravenous treatment specialist nurse training and certification management model,the results of the study has the scientific nature,rigor and reliability,the selection of intravenous therapy health nurses in China,training,evaluation,qualification certification and recertification management has a certain guiding significance.
