Objective:1.To describe the health literacy status of high-risk stroke population in Jilin.2.To analyze the influencing factors of health literacy in high-risk population of stroke.3.Construct a structural equation model of the factors which influence the health literacy in high-risk population of stroke and reveal the intrinsic relationship between the factors and the effect path of health literacy.Method:1.From May 2017 to October 2018,a stratified random sampling method was used to select one street in each of the five municipal districts in Jilin,including Longtan District,Changyi District,Fengman District,Chuanying District and Gaoxin District.And two community health service centers were randomly selected from each street.Finally,a survey was conducted among 588 high-risk stroke population aged 45-69 years in the communities.2.Descriptive analysis,chi-square test and binary logistic analysis of the data are carried out by SPSS 24.0 software.3.Using Amos 24.0 software to construct the path analysis model of influencing factors of health literacy in high-risk population of stroke.Result:1.In this study,a total of 2000 stroke risk screening cards were issued.And 1901 valid questionnaires were collected,among which 588 people were identified as high-risk population at a screening rate of 30.93%.588 subjects who met the high-risk conditions of stroke were included.Among them,279 were males,accounting for 47.45%,309 were females,accounting for 52.55%;208 were aged45-54,accounting for 35.38%;194 were aged 55-64,accounting for 32.99%;186were aged 65-69,accounting for 31.63%;504 were Han people,accounting for85.71%;126 were educated below junior middle school,accounting for 21.43%;89were civil servants and public institutions,accounting for 15.14%;and 29 were married.0 persons,accounting for 49.32%;360 persons,accounting for 61.22%;224persons,accounting for 38.10%,whose monthly income ranges from 2000 to 4999.2.The health literacy level of the high-risk population of stroke in Jilin is 18.03%,which indicates the overall level is low.In addition,the highest level of health skills literacy(10.20%)and the lowest level of basic knowledge and concept literacy(12.41%).3.The health literacy level of stroke high-risk population in Jilin City has significant differences in gender,age,educational level,occupation,marriage,family number,average monthly income,household registration,health status,family function,general self-efficacy and positive and negative emotions(p < 0.05).4.The results of binary logistic regression analysis showed that the main factors influencing the health literacy level of stroke high-risk population in Jilin were marital status,family monthly income,health condition,family function and general self-perception effectiveness and positive and negative emotion.Among them,The health literacy score of the average monthly income during 2000-4999 yuan and more than 5000 yuan(the OR value of average monthly income during 2000-4999 yuan and more than 5000 yuan is 2.620 and 3.477 respectively,and the 95%confidence interval is 1.130-6.072 and 1.514-7.988 respectively)is higher than that of the high-risk population of stroke whose average monthly income is less than2000 yuan.The health literacy score of widowed and divorced(OR value 0.107,95% confidence interval 0.021-0.543),better health(OR value 0.828,95%confidence interval 0.132-5.209)is lower than the high-risk population of stroke who is unmarried and poor health.The health literacy score of family function was good(OR value was 15.389,95% confidence interval was 1.461-162.149),general self-efficacy level was very high(OR value was 4.439,95% confidence interval was1.020-19.326),positive emotion was obvious(OR value was 2.163,95% confidence interval was 1.020-4.590)was higher than high-risk population of stroke who hasserious family dysfunction,low general self-efficacy and obvious negative emotion.5.The goodness-of-fit index of the model after construction and fitting conforms to the evaluation criteria,and the model is more reasonable.Path analysis shows that health self-perception is the most important factor affecting the health literacy level of high-risk stroke population in Jilin City in both direct and overall effect,.At the same time,in terms of the total effect,the order of the effect,according to the extent form strong to weak is health self-perception,marital status,positive and negative emotions,general self-efficacy,family function and average monthly income.The path coefficients are 0.719,0.529,0.237,0.155,0.127 and 0.055,respectively.Conclusion:1.The risk of screening high-risk population of stroke in Jilin City is at a higher level in China,and the health literacy level of high-risk population is lower than that of many regions in the same period.2.The factors affecting the health literacy level of stroke high-risk population in Jilin include marital status,family monthly income,health status,family function,general self-efficacy and positive and negative emotions.3.The goodness-of-fit index of the constructed and fitted model path map is ideal,and the model is reasonable.3.Self-rated health status is the most important factor affecting health literacy in both direct and total effects,while marital status is more important factor that indirectly affects the health literacy level of high-risk stroke population.
