| ObjectiveThe purpose of this study was to observe the effect of Guilong Decoction on the levels of serum tumor necrosis factor-alpha and interleukin-10 in patients with type 2 diabetic peripheral neuropathy due to yang deficiency and blood stasis,and to evaluate the traditional Chinese medicine symptoms,clinical symptoms,inflammatory indexes,and vibration perception thresholds.Observation of nerve conduction velocity,etc.,to explore whether Guilong Decoction’s mechanism of action and reduce inflammation,and thus play a role in neuroprotection,provide a basis for the clinical application of Guilong Decoction.MethodInpatient department of endocrine department of Guangzhou Traditional Chinese Medicine Hospital and 99 outpatients of Yang deficiency and blood stasis type 2 diabetic peripheral neuropathy meeting the inclusion criteria were randomly divided into treatment group 1(Guilong Decoction)and treatment 2 group(Guilong Decoction + Mecobalamin tablets)and control group(Mecobalamin tablets),33 cases in each group,treatment period was 8 weeks,during which the original basic treatment was maintained,including dietary exercise control,hypoglycemic regimen,and all effects on the study results were disabled.Drugs,including Chinese medicine preparations.The TCM symptom scores,peripheral neuropathy overall symptom scores,Toronto clinical neuropathy scores,serum tumor necrosis factor-alpha,serum interleukin-10,lower limbs sensory threshold,and nerve conduction velocity of the lower extremities were observed before and after treatment in the three groups of patients.Results1.TCM syndrome score changes and efficacy assessment:The scores of the patients after treatment in the three groups were improved compared with those before treatment(P<0.01).Among them,the treatment 2 group was superior to the treatment 1 group after treatment.And control group(P<0.05,P<0.01),and treatment 1 group was superior to control group(P<0.05);in terms of efficacy of TCM symptom improvement after treatment,the total effective rate of treatment group 1 was 90%,treatment 2 The total effective rate in the group was 96.7%,and the total effective rate in the control group was 86.7%.Each group was compared with each other.The treatment group 1 was superior to the control group(P<0.05),and the treatment group2 was superior to the other two groups(P<0.05).2.The TSS score changes and curative effect evaluation: After treatment,the overall symptom scores of peripheral neuropathy in each group decreased compared with before treatment(P<0.01).The comparison between the three groups showed that the treatment 2 group was better than the treatment 1 group and the control group.(P<0.05,P<0.05).There was no significant difference between the treatment 1 group and the control group(P>0.05).In terms of efficacy evaluation,the total effective rate of the treatment 1 group was 90%,and the total effective rate of the treatment 2 group was 96.7%.The total effective rate in the control group was 90%,but there was no significant difference between the three groups(P>0.05).3.The TCSS score changes and curative effect evaluation: The clinical neuropathy score in Toronto after treatment was lower than that before treatment(P<0.01).After treatment,the two groups were compared,and the treatment 2 group was better than the control group(P<0.05).The treatment1 group and treatment group 2 There was no significant difference between the control group and the control group(P>0.05,P>0.05).In terms of curative effect,the total effective rate of the treatment 1 group was 90%,the total effective rate of the treatment 2 group was 96.7%,and the total effective rate of the control group was 83.3%.There was no significant difference between the three groups(P>0.05).4.The VPT changes: Compared with before treatment,the VPT of both lower extremities decreased after treatment in both groups(P<0.01),and the difference between the two groups after treatment was statistically significant(P<0.05)There was no significant difference between treatment group 1 and control group(P>0.05).5.The nerve conduction velocity of both lower extremities results:Compared with before treatment,the tibial nerve conduction velocity and the common peroneal nerve conduction velocity in both lower limbs were improved in all three groups(P<0.01),and the treatment group 2 was better than the treatment group 1 and the control group(P<0.05,P<0.05).There was no significant difference between the treatment group 1 and the control group(P>0.05).6.The changes in serum TNF-α: The serum levels of TNF-α in the three groups after treatment decreased compared with that before treatment(P<0.01).The decrease of serum TNF-α in the two groups was better than that in the treatment 1 and control groups(P<0.05,P<0.01).The treatment1 group was superior to the control group(P<0.05).7.The changes in serum IL-10 : Serum interleukin-10 levels increased after treatment in all three groups(P<0.01).After treatment,the increase rate of treatment group 2 was better than that of treatment group 1 and control group(P<0.01,P<0.01).The increase rate of treatment group 1 was better than that of control group(P<0.05).ConclusionGuilong Decoction can effectively improve the patient’s clinical symptoms,sensory threshold,nerve conduction velocity,the mechanism may be associated with lower proinflammatory cytokines,anti-inflammatory cytokines,reduce the inflammatory response,and thus play a neuroprotective role. |
