Clinical Study Of Traditional Chinese Medicine In The Treatment Of Anal Sinusitis Based On Turbidity Toxin Theory

Objective:Based on the Turbidity Toxin theory,the clinical efficacy of traditional Chinese medicine in the treatment of anal sinusitis was explored,with the aim of exploring new ideas and new methods for treating this disease.Methods:From March 2018 to July 2019,76 patients with anorectal sinusitis who attended the anorectal outpatient department of Hebei Provincial Hospital of Traditional Chinese Medicine were selected.76 patients met the inclusion criteria,including 44 males and 32 females.The youngest is 22 years old and the oldest is 64 years old.The shortest course of disease was 1 month and the longest was 29 months.This study was approved by the hospital ethics committee,and all patients signed relevant informed consent.Using random number table method,76 patients with anal sinusitis were divided into two groups（western drug group and Chinese medicine group）,with 38 cases in each group.There were no statistical differences between the two groups in terms of gender,age,course of disease,and degree of disease development（P>0.05）.The western drug group（control group）first sitz bath with 400m L of potassium permanganate solution 1:5000 for 15 minutes,and then took one Compound Carraghenates Suppositories into the anus,and treated it once a day before bedtime.The Chinese medicine group（treatment group）first sitz bath with Jiedu Huazhuo lotion 400m L for 15 minutes,then took 8g of Ziyu ointment into the anus,and treated it once a day before bedtime.Both groups take 6 days as a course of treatment,and 3 consecutive courses of treatment.Compare the differences in the effective rate and cure rate between the two groups,and record the improvement of clinical symptoms（belly feeling,pain,pruritus）and signs（touch pain,anal papilla red and swelling,anal sinuses hyperemia,edema）before and after treatment.All data were processed by statistics（SPSS20.0 software）.Measurement data was expressed as（x±s）,t test was used for normal distribution,rank sum test was used for non-normal distribution,andχ² test was used for count data.P<0.05 is the standard for significant differences.Results:1.Total effective rate and cure rate:Of the 38 patients in the Chinese medicine group,37 were effective and 1 was ineffective,with a total effective rate of 97.37%;30 of the 38 patients in the western drug group were effective,8 were ineffective,and the effective rate was 84.21%.Using theχ² test,χ²=4.537,P=0.033.Among the 38 patients in the Chinese medicine group,28cases were cured,10 cases were not cured,the cure rate was 73.68%;18 cases were cured in the western drug group,20 cases were not cured,and the cure rate was 47.37%.Using theχ² test,χ²=5.507,P=0.019.This proves that the overall clinical efficacy of the Chinese medicine group is better than the western drug group.2.Improvement of clinical symptoms（belly feeling,pain,pruritus）:Using theχ² test,there are difference between the two groups on belly feeling and pruritus,and the curative effect of Chinese medicine group is better than western drug group（P₁=0.043,P₂=0.016）.There are no difference between the two groups on pain（P=0.449）,therefore,it can not be proved that there is a difference in improvement of anal pain symptoms between the two groups.3.Improvement of clinical sign（touch pain,anal papilla red and swelling,anal sinuses hyperemia,edema）:Using theχ² test,there are difference between the two groups on anal papilla red and swelling,anal sinuses hyperemia and edema.And the curative effect of Chinese medicine group is better than western drug group（P₁=0.036,P₂=0.029,P₃=0.040）.There are no difference between the two groups on touch pain（P=0.329）,therefore,it can not be proved that there is a difference in improvement of touch pain between the two groups.4.Safety observation:No serious adverse reactions occurred during the treatment of the two groups,and no obvious abnormalities were found in various physical and chemical tests.Conclusion:1.Jiedu Huazhuo lotion combined with Ziyu ointment can significantly improve the total effective rate and cure rate of anal sinusitis.2.Jiedu Huazhuo lotion combined with Ziyu ointment can significantly improve the belly feeling and pain in patients with anal sinusitis.3.Jiedu Huazhuo lotion combined with Ziyu ointment can significantly improve the signs of anal papilla red and swelling,anal sinuses hyperemia and edema.4.Jiedu Huazhuo lotion combined with Ziyu ointment has a positive clinical effect in treating anal sinusitis,and is safe and reliable.
