| Objective:By exploring the clinical effects of Huazhuo Jiedu Decoction in treating obesity with insulin resistance patients(turbidity toxin inside syndrome).Evaluating the effectiveness and safety of the prescription.To provide methods and new ideas for traditional Chinese medicine to treat obesity with insulin resistance.Method:From January 2020 to December 2020,60 patients in the Endocrinology Department of the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine.The patients meeting the diagnostic criteria and inclusion criteria.Randomly divided into two groups(control group and experimental group),30 patients as a group.The control group was given routine lifestyle intervention treatment,including reasonable diet,regular and appropriate physical exercise,etc.;the experimental group was added with the control group on the basis of Huazhuo Jiedu Decoction,orally in the morning and evening,100 ml each time.The course of treatment is 12 weeks.The waist circumference,body weight,body mass index,waist-to-hip ratio,fasting insulin,homeostasis model assessment of insulin resistance,blood glucose and blood lipids of the two groups of patients were monitored before and after treatment.The curative effect of this treatment is evaluated according to the patient’s degree of obesity,insulin resistance and TCM syndrome score.Results:(1)Huazhuo Jiedu Decoction can reduce the degree of obesity in patients.After 12 weeks of treatment,the obesity of the two groups of patients improved to varying degrees(P<0.05);The effective rate of obesity treatment in the Huazhuo Jiedu Decoction group was80.00%,in the control group was 46.67%,P<0.05.In the Huazhuo Jiedu Decoction group,the waist circumference was decreased by(4.83±4.98)cm compared with before treatment,P<0.05;the waist-to-hip ratio was lower than before treatment(0.02±0.06),P<0.05.The Huazhuo Jiedu Decoction has a significant weight loss effect,especially in reducing central obesity.(2)Huazhuo Jiedu Decoction can reduce the patient’s insulin resistance.After 12 weeks of treatment,the insulin resistance(FINS and HOMA-IR)of the two groups were improved compared with before,and the Huazhuo Jiedu Decoction group was better than the control group(P<0.05).(3)Huazhuo Jiedu Decoction can improve blood glucose and blood lipid metabolism in patients.After 12 weeks of treatment,the blood glucose(FPG,Hb A1c)and blood lipids(TC,TG,LDL-C)of the Huazhuo Jiedu Decoction group were improved compared with before treatment(P<0.05).(4)Huazhuo Jiedu Decoction can optimize the patient’s TCM symptoms and significantly reduce the syndrome score.After 12 weeks of treatment,in terms of single symptoms,the two groups of patients improved to varying degrees after treatment.In terms of physical obesity,fatigue,fullness of the flank,and turbid urine,the Huazhuo Jiedu Decoction group was better than the control group(P<0.05).The scores of TCM syndromes of the two groups of patients were lower than before,and the Huazhuo Jiedu Decoction group was lower than the control group(P<0.05);For the efficacy of TCM syndromes,the effective rate of Huazhuo Jiedu Decoction group was 86.67%,and that of control group was 46.67%,P<0.05.(5)The two groups of patients had no adverse reactions,such as gastrointestinal discomfort,skin rash,etc.were seen,and the safety was good.Conclusion:Huazhuo Jiedu Decoction has a good comprehensive effect in the treatment of obese with insulin resistance patients,which can effectively reduce the degree of obesity of patients,reduce body mass index,waist circumference,waist-to-hip ratio,etc.;Optimize the patient’s insulin resistance status,and reduce fasting insulin and homeostasis model assessment of insulin resistance;Regulate blood glucose and blood lipid metabolism;Improve the clinical symptoms and signs of patients,and comprehensively regulate the body’s metabolic disorders.In the entire clinical observation,there were no obvious adverse drug reactions and intolerances.The safety is good and it is worthy of clinical application. |
