Clinical Observation On Treatment Of Diarrhea-predominant Irritable Bowel Syndrome Of Spleen Deficiency And Excessive Dampness Type With Filiform-fire Needle

ObjectiveTo compare the curative effect of filiform-fire needle acupuncture and Pinaverium Bromide in the treatment of diarrhea-predominant irritable bowel syndrome of spleen deficiency and excessive dampness type.Methods60 cases of diarrhea-predominant irritable bowel syndrome of spleen deficiency and excessive dampness type in department of gastroenterology,acupuncture and moxibustion of Zhongshan Hospital of traditional Chinese medicine of Guangdong Province were randomly divided into two groups.30 cases were in filiform-fire needle group,30 cases were in Pinaverium Bromide group.In filiform-fire needle group,two groups of acupoints were selected.Group A took Tianshu,Shangjuxu,Zusanli,Dachangshu and Sanyinjiao,the left side was taken.Group B took the rignt side of these acupoints.The depth of needling was the same as that of conventional needling,and retained the needles for 5 minutes.Two groups of acupoints were used alternately every other day.One day off a week.The control group was treated with Pinaverium Bromide.Filiform-fire needle group and the control group were treated for 2 weeks.We used the irritable bowel syndrome symptom severity scale(IBS-SSS),the TCM syndrome score table,the irritable bowel syndrome patients quality of life rating table(IBS-QOL)and self perceived burden scale(SPBS)to score before and after the treatment.And SPSS 26.0 software was used for statistical analysis.ResultsFinally,there were 30 cases in the filiform-fire needle group and 30 cases in the control group.(1)IBS-SSS score:After the treatment,the IBS-SSS scores of the two groups decreased(P<0.05),and the IBS-SSS score of the filiform-fire needle group was lower than that of the control group(P<0.05).(2)The TCM syndrome score:After the treatment,the TCM symptom scores of the two groups decreased(P<0.05),and the TCM symptom score of the filiform-fire needle group was lower than that of the control group(P<0.05).The TCM single symptom scores of the two groups were decreased after treatment(P<0.05),the filiform-fire needle group was better than the control group in the treatment of diarrhea,abdominal distension,abdominal pain,lethargy,fatigue or cold attack or aggravation(P<0.05),the two groups had the same effect in the treatment of epigastric fullness,borborygmus and mental fatigue(P>0.05).(3)IBS-QOL score:After the treatment,the IBS-QOL scores of the two groups increased(P<0.05),and the IBS-QOL score of the filiform-fire needle group was higher than that of the control group(P<0.05).(4)SPBS score:After the treatment,the SPBS scores of the two groups decreased(P<0.05),and the SPBS score of the filiform-fire needle group was lower than that of the control group(P<0.05).ConclusionFiliform-fire needle and Pinaverium Bromide are all effective in the treatment of diarrhea-predominant irritable bowel syndrome of spleen deficiency and excessive dampness type.Compared with Pinaverium Bromide,filiform-fire needle has better effect in improving clinical symptoms,quality of life and self perceived burden of patients in a short time.
