ObjectiveTo observe the clinical effect of plum-blossom needle on hand Sanyang and conventional acupuncture on upper limb spasm after stroke,and to explore its mechanism,so as to provide a simple,safe and effective treatment scheme for patients with upper limb spastic paralysis after stroke.MethodsFrom June 2020 to December 2020,this study included 60 patients who were hospitalized in the Department of Acupuncture and Rehabilitation of Guangdong Second Hospital of Traditional Chinese Medicine and met the inclusion criteria.They were randomly divided into treatment group and control group,with 30 cases in each group.Two groups of patients were given basic internal medicine and routine rehabilitation treatment On this basis,the treatment group was given routine acupuncture combined with plum-blossom needle tapping the Sanyang Meridian of the hand,and the specific tapping range was from the back of the hand to the shoulder,while the control group was given routine acupuncture.Acupuncture and plum-blossom needle tapping are treated once a day.After continuous treatment for 6 days,they rest for 1 day,and 14 days is a course of treatment.The modified Ashworth scale(MAS),simplified Fugl-Meyer scale(FMA)and Barthel index score(BI)of the two groups were evaluated before and after treatment,and SPSS25.0 software was used for statistical analysis and comparison of curative effects.Results.1.There were no rejected or dropped cases during the treatment in this study.A total of 30 cases in the treatment group and 30 cases in the control group were included in the statistical analysis.2.Baseline comparison:Before treatment,the age,sex,course of disease,nature of lesion,modified Ashworth scale,upper limb simplified Fugl-Meyer scale score and Barthel index score of the two groups were compared,with no significant difference(P>0.05),indicating that the two groups are comparable.3.Comparison of scales:After 2 weeks of treatment,the modified Ashworth grades of the treatment group and the control group were improved compared with those before treatment,and the improvement degree of the treatment group was better than that of the control group(P<0.05).After treatment,the simplified Fugl-Meyer scores of upper limbs in the two groups were significantly different from those before treatment,and the difference between the two groups before and after treatment was statistically significant(p<0.05).After treatment,Barthel index scores of both groups were higher than before,but there was no significant difference between the two groups before and after treatment(P>0.05).4.Comparison of curative effects:After the course of treatment,6 cases were cured,2 cases were markedly effective,16 cases were effective,6 cases were ineffective,and the total effective rate was 80.00%.In the control group,2 cases were cured,0 cases were markedly effective,17 cases were effective,and 11 cases were ineffective.The total effective rate of the control group was 63.33%,and the difference in clinical efficacy between the two groups was statistically significant(P<0.05).ConclusionThe combination of plum-blossom needle and routine acupuncture treatment and routine acupuncture treatment can improve the spasticity of upper limbs of patients with spastic paralysis after stroke to a certain extent,which is conducive to promoting the rehabilitation of stroke patients.In terms of reducing upper limb spasm grade and improving upper limb motor function,the curative effect of plum-blossom needle tapping hand Sanyang meridian combined with conventional acupuncture treatment group is better than that of conventional acupuncture group.Both groups of research methods can improve the activities of daily living of patients with spastic paralysis of upper limbs after stroke,but they fail to show which group is more effective.
