| ObjectiveBased on the theory of meridians and tendons,this study observed the clinical efficacy of different methods of meridians acupuncture combined with massage in the treatment of upper limb spastic paralysis after stroke,and evaluated the efficacy with relevant scales before and after treatment,in order to explore the therapeutic effect of different methods of meridians acupuncture combined with massage.MethodFrom May 2020 to January 2021,a total of 60 patients with upper limb spastic paralysis in non-acute post-stroke in the outpatient department and ward of the Second Affiliated Hospital of Tianjin University of Traditional Chinese Medicine were selected as the subjects.The patients who met the inclusion criteria were randomly divided into the Meridian tendon node group(Group A)and the Meridian tendon muscle abdomen group(Group B)according to the ratio of 1:1,and the two groups were treated with two kinds of treatment.Group A was treated with acupuncture at meridian tendon nodes combined with massage;Group B was treated with acupuncture at meridian tendon muscle abdomen combined with massage.Two groups of patients were treated 5 times a week,every two weeks for a course of treatment,a total of 2 courses of treatment(a total of 4 weeks).The therapeutic effects were evaluated before the first treatment,3 days after treatment,1 week after treatment,2 weeks after a course of treatment and 4 weeks after two courses of treatment by MAS,Brunnstrom stage,FMA-UE score,MBI score and VAS score.After the endof the treatment cycle,the therapeutic effect evaluation data were statistically analyzed and processed,and the clinical treatment effect of the two groups was compared to draw a conclusion.Result1.The general information and the evaluation data of each scale before treatment were compared between the two groups.The difference was not statistically significant,and the baseline was consistent with comparability(P > 0.05).2.MAS: Compared with the two groups before and after treatment,the difference was statistically significant(P < 0.05),and the two groups can improve the symptoms of spasticity and reduce muscle tension.In terms of the end of the treatment effect,there was no significant difference between the two groups after treatment(P > 0.05);in terms of short-term effect,the curative effect of meridian muscle abdomen group was better than that of meridian tendons node group,and there was interaction effect in group * time node(P < 0.05).3.Brunnstrom: Compared with the Brunnstrom stage(upper limb + hand)before and after treatment,the two groups could improve the motor function of upper limb,the difference was statistically significant(P < 0.05);compared with the Brunnstrom stage of upper limb after treatment,the difference was not statistically significant(P > 0.05);compared with the Brunnstrom stage of hand after treatment,the difference was statistically significant(P <0.05).4.FMA-UE: The difference of scores before and after treatment between the two groups was statistically significant(P < 0.05),which could improve the motor function of upper limb.The difference of FMA(upper limb)score between the two groups after treatment was statistically significant(P < 0.05).The scores of each sub-items of FMA(upper limb)in the two groups before and after treatment were statistically different(P < 0.05);only in the sub item of finger function in the two groups after treatment,the difference was statistically significant(P < 0.05),and the other sub-items were not statistically significant(P > 0.05).5.MBI: The two groups can improve the ADL ability of patients,and the difference of MBI score before and after treatment was statistically significant(P < 0.05);the MBI score between the two groups after treatment,the difference was statistically significant(P < 0.05).6.VAS: Two groups can reduce pain,and the two groups of effective patients before and after treatment of VAS score were significantly different(P <0.05).Compared two groups of VAS score after treatment,the difference was not statistically significant(P >0.05).Conclusion1.Both acupuncture at the meridian tendons node combined with massage and acupuncture at the meridian muscle abdomen combined with massage can improve the spasticity of paralyzed upper limb,reduce muscle tension and relieve pain.There is no significant difference in the curative effect of the two groups after the course of treatment about reducing muscle tension and relieving pain of the affected upper limb,but the short-term effect of meridian muscle abdomen acupuncture combined with massage is better than that of meridian tendons node acupuncture combined with massage in reducing muscle tension.2.Both acupuncture at the meridian tendons node combined with massage and acupuncture at the meridian muscle abdomen combined with massage can improve the motor function of the affected side of the upper limb and improve the ability of daily life of the patients.However,from the observation of the curative effect,the overall curative effect of acupuncture at the meridian tendons node combined with massage is better than that of acupuncture at the meridian muscle abdomen combined with massage.3.The effect of acupuncture at the meridian tendons node combined with massage on improving the motor function of affected hand is better than that of acupuncture at the meridian muscle abdomen combined with massage. |
