The healthcare industry matters to both nation’s interest and people’s livelihood.Especially as the continuous development of social economy in recent years,the unbalance between scarce medical resources and people’s health needs has pushed the medical system reform to the next stage.As an important part of the medical reform Standardized Training of Residents(STR thereafter)is a compulsory path before accrediting qualified clinicians and developing hierarchical medical system.The Ministry of Health states that "STR shall focus on enhancing job competencies.The training contents shall include medical ethics,policies and regulations,clinical practices,theoretical knowledge,and interpersonal communication etc.The primary objective is to improve standardized clinical practice competency,giving consideration to clinical teaching and scientific research".As one of the nearly 300 prefecture-level cities,Deyang City shall implement STR in this region and provide clinicians to both general hospitals and primary hospitals.This study extracts the evaluation system of competencies for STR graduates from the results analyzed from STR polices,job requirements and expert interview,basing on the concepts and theories of competency and by collating and analyzing both domestic and overseas research status.Competencies of clinicians will be evaluated in 7 aspects,including achievement motivation,personality traits,learning capacity,professional knowledge,clinical skills,academic research,and doctor-patient communication.This study will focus on the differences of competencies of STR graduates,as well as their influence factors.Although this study implements within Deyang area,its evaluation indicators make copious references to domestic authoritative competency system and literatures from home and aboard.And also Deyang City is a typical prefecture-level city.So the evaluation indicators have typical and common characteristics to some extent.This study collects competency data with electronic questionnaires from 120 graduates who completed STR in Deyang City from 2016 to 2019.Finally,112 valid questionnaires are retrieved.After questionnaire sorting,data is pooled by Microsoft Excel.And then the processed data is analyzed with SPSS 17.0,by using one-way ANOVA statistical analysis and multiple-linear regression at significance level P<0.05.The multi-factor analysis indicates that the competency score of STR clinicians is influenced by various aspects.Generally,the competencies of STR graduates are significantly influenced by their education background and salary during STR,and less influenced by their identity before STR,type of employing hospital,and graduation year.Competency analysis shows that STR graduates have different competency scores if they have different education backgrounds,different graduation years,different employing hospitals,different identities before STR,different policy understandings,and different salaries,in terms of achievement motivation,personality traits,learning capacity,professional knowledge,clinical skills,academic research,or doctor-patient communication.Generally,the STR system in prefecture-level city mainly provides resident doctors for local municipal,county,and township hospitals.Therefore,compatible local STR system and courses shall be developed according to regional economy and actual healthcare needs.In order to achieve the stated goal of training,more local financial support,better STR teachers and evaluation system,and more salary for STR trainees are required.
