| Objective:1.On the basis of clarifying the connotation of intensive care unit(ICU)nursing interruption events,this study constructed the ICU nursing interruption event questionnaire to further study the occurrence of ICU nursing interruption events and their correlation with the psychological load of nurses.To make hospital managers and nurses better identify the status and characteristics of nursing interruption events,to reduce the occurrence of nursing interruption events,to prevent medical errors to provide experience and reference.2.This paper proposes the intervention strategy of ICU shift interruption events,and provides a new perspective for the clinical search for a safe and effective nursing interruption event management scheme by observing its clinical effect.Methods:1.By means of literature analysis and expert interview,the entry pool of ICU nursing interruption event questionnaire was initially formed.Delphi expert consulting method is adopted to screen and modify the initial nursing interruption event investigation items,and finally determine the investigation items.On the basis of building the ICU interrupt event questionnaire,using the convenience sampling method to extract an ICU nurse in third rate synthetic hospital,Shanxi Province formal investigation for 810 hours,investigate nursing interrupt event occurrence frequency and its correlation with nurses mental load,statistical analysis of data using SPSS25.0 software correlation analysis using the Spearman correlation analysis.2.The causes of 724 nursing interruption events in 120 routine shifts were analyzed.According to the causes,the intervention strategies were put forward and implemented,and the occurrence of nursing interruption events and the time of succession in 120 shifts before and after the implementation of the intervention strategies were calculated.The independent sample t test and chi-square test were used for statistical analysis of the data.Results:1.Form an ICU nursing interruption event questionnaireIn this study,a total of 18 experts completed three rounds of expert consultation,and the effective recovery rates of the questionnaire were 90%,100% and 100%respectively.Among them,15 experts proposed modification Suggestions,indicating that the experts were highly motivated.The results of expert consultation showed that:judgment basis(Ca)=0.92,familiarity(Cs)= 0.76,and authority(Cr)=0.84,indicating a high degree of expert authority.The coefficient of variation(CV)of the three rounds of expert consultation was ? 0.25,and the coefficient of expert coordination of the first,second and third round of the letter consultation was 0.278,0.316 and 0.363(P<0.05)respectively,indicating that the expert opinions tended to be consistent.2.The occurrence of nursing interruption in ICU and its correlation with psychological load of nursesOccurrence of nursing interruption events: a total of 7,685 nursing interruption events were recorded during 810 hours of observation.The average number of nursing interruption events was 9.5 /h.The frequency of weekday nursing interruption events was higher(n=5441;78.0%),mainly from other nurses(n= 2489,32.4%).Nursing records were the most common current task(n= 2430,31.6%),and interruption tasks were mainly patient-related tasks(n= 4372,56.89%).The duration of nursing interruption <1min was the majority(n= 3543,46.1%).The frequency of interruption at the bedside of patients(n= 1984,25.81%)was higher than that of other places.After the interruption,most(n= 6709,87.3%)nurses returned to the original task.Correlation between occurrence frequency of nursing interruption events and psychological load: Spearman correlation analysis showed that there was a significant positive correlation between total psychological load of nurses and occurrence frequency of nursing interruption events(r=0.531,P=0.000).3.Effect analysis of nursing interruption intervention strategy in ICU shift120 shifts before the implementation of the intervention strategy,724 nursing interruption events occurred,and each shift nursing interruption event occurred 6.03 times.After the implementation of the measures,there were 120 shifts,516 interruptions and 4.30 interruptions.The frequency of nursing interruption before and after the implementation was statistically significant(t=6.214,P=0.000).The duration of interruption before implementation(67.39 ± 17.44)s,and after implementation(52.83 ± 19.73)s,before and after implementation,the difference was statistically significant(t=6.056,P=0.000).Before the implementation of the intervention strategy,the number of patients per nurse on each shift was 1.96,and the shift time was 24.03± 5.45 min.After the implementation of the intervention strategy,the number of patients per nurse on each shift was 1.93,and the shift time was 19.15±4.22 min.The difference was statistically significant(t=7.751,P=0.000).Conclusion:1.Delphi method was used to select and revise the questionnaire entries of ICU nursing interruption events,and finally a questionnaire consisting of 3 first-level entries,7 second-level entries and 25 third-level entries was formed.2.Nursing interruption events occurred frequently in ICU,and the occurrence frequency of nursing interruption events was positively correlated with the psychological load level of nurses.3.Nursing interruption events are common in the shift of ICU nurses.The establishment of intervention strategy of interruption events reduces the occurrence frequency of nursing interruption events,shorens the shift time,and improves the quality of nurses' shift. |
