Part ?Systematic evaluation of the effect of dance therapy on the prognosis of patients with essential hypertensionObjective To evaluate the effects of dance therapy on systolic blood pressure, diastolic blood pressure, body mass index, motor function, pain, overall health, and mental health in patients with essential hypertension. Methods Search for database resources such as Pub Med,EMBASE,Medline,Web of Science,CENTRAL,Sinomed,China Knowledge Network,Wanfang,VIP,etc.,and collect randomized controlled trials of hypertensive patients for dance therapy intervention. The search time is from setting up database to 2018.month.Statistical processing was performed using Rev Man5.3 software. Results A total of 6 literatures were selected. The results showed that after the intervention of dance therapy, the systolic blood pressure, diastolic blood pressure, motor function and overall health level were significantly improved in the dance therapy group compared with the non-sports therapy group(P <0.05),and the body mass index, pain, mental health The impact is not significant(P >0.05).Conclusion Dance therapy can effectively reduce systolic and diastolic blood pressure in patients with hypertension, improve the motor function of patients, and promote overall health of patients. However, because the included studies are methodologically uneven and the intervention time is relatively short,a strict, large sample of RCT can be designed in the future to observe the long-term effects of dance therapy on the prognosis of hypertensive patients.Part ?Community square dance on the intervention effect of middle-aged and elderly patients with hypertensionStage 1: Pre-experimentPurpose: Implement a small sample of clinical intervention experiments, explore the feasibility of the community square dance intervention program, improve the details of community square dance intervention,to develop a rigorous and simple community square dance intervention program, explore the sample sample of community square dance, for the later clinical Provide a basis for the development of randomized controlled trial intervention strategies. Methods: It is convenient to select 10 cases of middle-aged and elderly hypertensive patients who were filed by a community health service center in XX City,XX Province from March to April 2018.The subjects did not participate in the square dance activities or never participated in the square before the group was enrolled. In the dance exercise, the subjects were divided into experimental group and control group according to computer-generated random numbers, including 5 in the experimental group and the control group. The subject of the study gave her informed consent and signed an informed consent form to intervene. The control group received routine interventions such as We Chat platform health education and offline follow-up blood pressure. The experimental group added community square dance intervention on the basis of the control group.Blood pressure changes were assessed before and 2 months after intervention.Results:(1)The general data of age, height, weight, blood pressure, and history of hypertension in the two groups were compared at baseline, and the differences were not statistically significant(P>0.05),which was comparable.(2)The experimental group(n=5)had a mean systolic pressure difference of 7.6,the standard deviation of the systolic pressure difference was 3.8,and the control group(n=5)had a mean systolic pressure difference of 3.4,and the standard deviation of the systolic pressure difference. At 4.0,the difference in systolic blood pressure between the two groups was statistically significant(P<0.05).Conclusion:(1)Adjust the square dance intervention time to 19:00-20:00.(2)The recruitment of research subjects is mainly based on researchers, supplemented by volunteer supplements.(3)Health Education Online We Chat platform mode is changed to the opposite side of the line,one-on-one(4)Randomized controlled trial sample size: 31 cases in each groupStage 2: A randomized controlled trial of community square dance on middle-aged and elderly patients with hypertensionObjective: To explore the effects of community square dance on blood pressure, blood lipid levels and sleep in middle-aged and elderly patients with hypertension. Methods: The convenient sampling method was used, the choice from April to July in 2018 XX city community health service center by inputting two middle-aged and old patients with hypertension as the research objects, and 85 patients who met the exclusion criteria were selected according to computer-generated random numbers. It was divided into two groups,44 cases in the control group were treated routinely, and 41 cases in the experimental group were treated with community square dance on the basis of routine treatment. The intervention time was 12 weeks,3 times a week,1 hour each time, and the intervention intensity was moderate. The changes of blood pressure and blood lipid levels at baseline, intervention week 8,intervention week 12,and Pittsburgh Sleep Quality Index(PSQI)were used to evaluate the sleep status of the two groups before and after the intervention. The statistical analysis of the data used descriptive statistical analysis,t-test,chi-square test,Mann-Whitney U test, repeated measures analysis of variance and other methods.Results:(1)General information: 19 people in the experimental group and 18 in the control group withdrew from the study due to housing demolition, loss of follow-up,and inability to participate on time.A total of 48 middle-aged and elderly people completed the study, including 22 in the experimental group.(4 males and 18 females)and 26 in the control group(14 males and 12 females). In addition to gender, the two groups were compared with the general data of age, height, weight, blood pressure, blood lipids, medication compliance, and proportion of antihypertensive drugs in each group. The difference was not statistically significant(P>0.05).(2)The influence of community square dance on systolic and diastolic blood pressure in middle-aged and elderly patients with hypertension: after the intervention, the decrease of systolic blood pressure in the experimental group was significantly higher than that in the control group, with statistically significant difference(P<0.05),and the decrease of diastolic blood pressure in the experimental group was significantly higher than that in the control group, with statistically significant difference(P< 0.05).(3)The effect of community square dance on blood pressure at four time points in middle-aged and elderly patients with hypertension:(1)intra-group comparison: systolic blood pressure and diastolic blood pressure at each time point were different(P<0.05),and the effect of different intervention methods on systolic blood pressure changed with time, but had no significant effect on diastolic blood pressure(P>0.05).(2)comparison between groups: there was a difference in the effect of reducing systolic blood pressure between different intervention methods(P<0.05),but no significant effect on diastolic blood pressure(P> 0.05).(4)The effect of community square dance on blood lipids in middle-aged and elderly patients with hypertension: there was no statistically significant difference in serum lipid before intervention between the two groups(P>0.05).After 12 weeks of intervention, except for the decrease of LDL-C in the intervention group, there were no statistically significant differences in other blood lipid indexes between the two groups before and after intervention(P>0.05).(5)The effect of community square dance on sleep in middle-aged and elderly patients with hypertension: sleep disorder accounted for 50% of the total before intervention, intervention after sleep disorder accounted for 33% of the total,PSQI scores similar between the two groups have statistical significance(P<0.05),the sleep quality and sleep time two dimension differences statistically significant(P < 0.05).Conclusion:(1)Community square dance can effectively reduce the blood pressure of middle-aged and elderly patients with hypertension.(2)The effect of different intervention methods on reducing systolic blood pressure was statistically significant. The effect of reducing systolic blood pressure in the experimental group was significantly greater than that in the control group. The time of intervention also had an impact. After the intervention, the systolic blood pressure of the subjects gradually decreased. At the 8th week of intervention, the systolic blood pressure of the experimental group decreased rapidly, and reached the lowest at the 12 th week. There were interaction effects between different intervention methods and time. With the prolongation of intervention time, the systolic blood pressure of the experimental group and the control group decreased by different degrees, with the largest decrease in the experimental group. The difference of diastolic blood pressure between the experimental group and the intervention time was statistically significant. At the 8th week of intervention, the diastolic blood pressure increased, and at the 12 th week of intervention, the diastolic blood pressure reached the minimum.(3)Community square dance can effectively reduce low-density lipoprotein cholesterol in middle-aged and elderly patients with hypertension, but has no significant effect on cholesterol, triglyceride and high-density lipoprotein cholesterol.(4)Community square dance can effectively improve the sleep quality of middle-aged and elderly patients with hypertension, and reduce the time to fall asleep.
