Object:By detecting the expression level of serum fiblin-3 in coronary heart disease(CHD)patients,to explore the correlation between fiblin-3and the severity of CHD and the degree of coronary atherosclerosis was discussed.At the same time,brachial-ankle pulse wave conduction velocity(baPWV)and SYNTAX score of coronary atherosclerosis in CHD patients were measured,And the correlation among serum Fibulin-3 level,brachial-ankle pulse wave conduction velocity and SYNTAX score was discussd.Methods:From June 2018 to May 2019,80 patients with coronary artery angiography were admitted to the Department of Cardiology of the First Affiliated Hospital of Guangdong Pharmaceutical University.According to the results of coronary angiography,the patients were divided into 58 cases of CHD group,including 42 males and 16 females,with mean age(62.91±10.24)years old,and 22 non-CHD groups,including 16 males and6 females,with mean age(58.82±10.24)years old.the CHD group was divided into Stable angina pectoris(SA)group(n=25),acutecoronary syndrome(ACS)group(n=33);single-vessel disease(SVD)group(n=14),double-vessel disease(DVD)group(n=17)and triple-vessel disease(TVD)group(n=27)according to the number of coronary artery lesions.Clinical data of all the participants were collected,and fasting venous bloodsamples were collected to detect clinical indicators,and serum was extracted and stored in-80? refrigerator.The serum Fibulin-3 levels of the specimens were detected in batches by enzyme-linked immunosorbent assay(ELISA),and the baPWV was detected by pulsewave velocity automatic analyzer.Coronary artery disease was assessed by professionals after coronary angiography in the CHD group,and SYNTAX score was obtained with the software.Results:1.The serum Fibulin-3 levels were significantly lower in CHD group than in the non-CHD group(t=6.892,P < 0.01),and the baPWV level significantly higher in CHD group than in the non-CHD group(t=6.2762,P < 0.01).The patients were divided into SA group and ACS group according to the clinical type of coronary atherosclerotic heart disease,the non-CHD group was used as the control group,The serum Fibulin-3 level was significantly different among the three groups(F=34.318,P<0.01),and the Fibulin-3 level of control group was higher than that of SA group and ACS group(P<0.05),while that of SA group was higher than that of ACS group(P < 0.05).The baPWV level was statistically significant among the three groups(F=39.352,P<0.01).The ACS group was higher than the SA group and the the control group(P<0.05),while the SA group was higher than the control group,the difference was statistically significant(P<0.05).SYNTAX scores were significantly different between ACS group and SA group(t=2.061,P<0.05),and ACS group was higher than SA group.2.According to the number of coronary artery lesions can be divided into SVD group,DVD group and TVD group,the non-CHD group was used as the control group,the serum Fibulin-3levels are statistically significant among the four groups(F=17.921,P<0.01),the control group is higher than other three groups(P < 0.05),and SVD group is higher than DVD groups(P<0.05).The baPWV levels also was statistically significant among the four groups(F=16.124,P<0.01),thecontrol group is lower than other three groups(P<0.05),and SVD group is lower than DVD groups(P < 0.05).The correlation between the serum Fibulin-3,SYNTAX score and baPWV was analyzed by Pearson correlation analysis.With the decrease of serum Fibulin-3 level and the increase of baPWV value,there was a negative correlation(r=-0.757,P<0.05);there was no correlation between Fibulin-3 and SYNTAX scores(r=-0.149,P > 0.05);also there was no correlation between baPWV and SYNTAX scores(r=0.125,P>0.05).Conclusions:1.Patients with CHD have decreased serum fibulin-3 level,and acute coronary syndrome group is lower than stable angina pectoris group,indicating that the more serious the coronary atherosclerotic heart disease,the more obvious the decrease of Fibulin-3 level,suggesting that the decrease of serum Fibulin-3 level is associated with the severity of coronary heart disease,which can be used as a predictor of coronary atherosclerotic plaque instability.2.The baPWV and SYNTAX scores of acute coronary syndrome group were higher than that of stable angina pectoris group,indicating that the degree of arteriosclerosis and coronary lesion were higher in patients with acute coronary syndrome,and the increase of ba PWV value was related to serious cardiovascular events.3.There was a negative correlation between serum fibulin-3 level and baPWV value in patients with coronary atherosclerotic heart disease,suggesting that the lower the serum fibulin-3 level,the more serious the degree of arteriosclerosis is.Both of them can be used as predictors to evaluate the condition of coronary atherosclerotic heart disease and the severity of coronary atherosclerosis.
