Objective To discuss the changes of ankle brachial index (ABI),brachial-ankle pulse wave velocity (baPWV) and high sensitive C-reactive protein (hs-CRP) in 195 patients with different risk factors of athersclerosis; Analysis its early diagnosis of coronary heart disease.Methods 195 patients possible diagnosis or diagnosed of coronary heart disease with at least a merger of athersclerosis risk factors who were hospitalized and coronary angiography in cardiovascular physicians of NingXia medical university affiliated hospital On November 2008-2009 November period were selected. Coronary angiography by the Affiliated Hospital of Ningxia Medical Center Catheter Room of the heart ,and lesion stenos is in vascular interventional treatment by the two experts to determine. Appeared at least one coronary artery atherosclerotic plaque that was that there was been coronary artery disease, accordingly as selected hospitalized patients were divided into coronary artery disease group (150 cases) and non-coronary artery disease group (45 cases). Then according to at least one coronary stenosis≥50%, coronary artery disease group was divided into coronary heart disease (CHD) group (97 cases) and atherosclerosis (As) group (53 cases), two sub-groups. And 39 cases of healthy people were take as control group. Recorded the athersclerosis risk factors such as age,gender,triglyceride(TG),total cholesterolemia(TC),high density lipoproteinc cholesterol (HDL-C), low-density lipoprotein cholesterol(LDL-C),systolic of blood pressure (SBP),diastolic of blood pressure(DBP) ,glucose,body mass index(BMI),smoking history and measure ABI,baPWV,hs-CRP of serum. Contrast the above three groups of indicators of differences in. Using SPSS13.0 Statistical Package for the above indicators, statistical analysis. The measured data indicated x±s that among the groups using single factor analysis of variance. To coronary angiography results as the gold standard, make ABI, baPWV, and hs-CRP of the ROC area under the curve, thus making the diagnosis CHD the reference value. Using multiple linear regression analysis of baPWV and hs-CRP related to risk factors.Results①All patients between men and women ABI, baPWV, and hs-CRP was no significant difference compared (P>0.05); Comparison among the three groups no significant difference in ABI (P>0.05), BaPWV compared three groups of statistically significant difference(P<0.01); Non-coronary artery disease group and normal group hs-CRP was no significant difference(P>0.05); However, coronary artery disease group hs-CRP levels were significantly higher than the other two groups, there were statistically significant differences between the two groups(P<0.01), the level of hs-CRP showed no significant difference between the two subgroups of coronary artery disease group.②With coronary angiography as the gold standard drawing ABI, PWV, and hs-CRP on the predictive value of CHD subjects ROC curve , ABI had no significant predictive value for CHD; CHD with higher baPWV for the predictive value of the cut-off point for the 1700 cm/s ; baPWV the severity of coronary lesions with low predictive value; CHD pairs of hs-CRP has a moderate predictive value, its cut-off point of 0.67 mg/L, hs-CRP did not predict the value on the severity of coronary artery disease. Two joint diagnosis of coronary artery disease 99% positive predictive value, negative predictive value of 92%.③The risk factors of baPWV are age(P<0.01), SBP(P<0.01),FBG(P<0.01); Factors affecting hs-CRP for the age(P<0.01),BMI(P<0.01),LDL-C(P<0.05),FBG(P<0.05).④After controlling for other risk factors, baPWV and hs-CRP no significant correlation between.Conclusion①ABI pairs of CHD have no significant predictive value;②BaPWV with gradually increasing degree of coronary artery disease, baPWV≥ 1700cm / s can be that the merger CHD, As risk factors for the merger of the influencing factors in patients with baPWV mainly age, FBG;③Hs-CRP in a coronary artery disease is significantly higher among people; Hs-CRP≥0.67mg / L can be considered when the merger CHD, but hs-CRP and CHD showed no significant relationship; hs-CRPeffects of main factors are age, BMI, LDL-C, FBG;④BaPWV joint hs-CRP in early diagnosis of coronary heart disease have a higher value;⑤After controlling for other risk factors, baPWV and hs-CRP is no significant correlation.
