Clinical Study On The Treatment Of The Blood Heat And Intestinal Dryness Type Of Stage Ⅱ Anal Fissure By Xiaoyan Ointment Combined With Qingzao Mixture

Objective:To observe the clinical effect of Xiaoyan Ointment combined with Qingzao mixture in the treatment of stage Ⅱ anal fissure of blood heat and intestinal dryness type,so as to provide a safe and effective method of conservative treatment of anal fissure with pure traditional Chinese medicine,and give full play to the value of traditional Chinese medicine in treating anorectal diseases.Methods:60 cases of stage Ⅱ anal fissure with blood heat and intestinal dryness type patients were randomly divided into two groups according to the random table number method,with 30 cases in each group.The control group only adopted topical treatment and was treated with nitroglycerin ointment,while the experimental group adopted both topical and oral treatment by using xiaoyan Ointment and Qingzao mixture at the same time.7 days were viewed as a course of treatment,and both groups were treated for 2 courses.The pain index,the duration of anal spasm pain,the degree of defecation difficulty,the condition of blood in stool,the syndrome score of traditional Chinese medicine,the occurrence and recurrence of adverse reactions were observed and compared between the two groupsResults:After the treatment,4 cases in the experimental group and 4 cases in the control group were excluded due to some uncontrollable causes.A total of 52 cases(26 cases from the experimental group and 26 cases from the control group)met the requirements of the study.Before treatment,there was no significant difference in age,gender,course of disease and other general data before the treatment between the two groups(P>0.05).The results are as follows:(1)Overall Effects:The remarkable efficiency of the experimental group is 76.9%,and the total effective rate is 88.5%;the remarkable efficiency of the control group is 42.3%,and the total effective rate is 73.1%.The remarkable efficiency and comprehensive efficacy of the experimental group were better than the control group,and the differences were statistically significant(P<0.05);(2)Pain Index:Comparing the pain indexes of 7 days and 14 days of treatment in the two groups,the differences were not statistically significant(P>0.05).The experimental group and the control group had comparable efficacy in reducing pain;(3)Duration of Anal Canal Spasm Pain:Comparing the duration of anal canal spasm pain after 7 days and 14 days of treatment in the two groups,the difference was not statistically significant(P>0.05).The experimental group and the control group had comparable efficacy in relieving spasm pain;(4)Degree of Defecation Difficulty:The difference of degree of defecation difficulty after 7 days and 14 days of treatment between the two groups had statistical significance(P<0.05).The experimental group had better performance in relieving defecation difficulty;(5)Condition of Defecation Blood:The difference was not statistically significant(P>0.05)between two groups in the course 7d treatment,while the difference was statistically significant(P<0.05)in the course of 14d treatment.The experimental group does a better job in reducing defecation blood.(6)TCM Syndrome Score:The difference between the two groups had statistical significance(P<0.05)after 14d treatment.In the experimental group,TCM syndrome score decreased more obviously.(7)Adverse Reactions:After calculation,the incidence of adverse reactions in the control group andthe experimental group were 29.6%and7.4%.The difference was statistically significant(P<0.05)and theexperimental group wasmore safe in application;(8)Recurrence:6 months follow-up after treatment showed that there were 4 cases of recurrence among 26 cases in the control group with the recurrence rate of 15.4%and 2 cases of recurrence among 26 cases in the experimental group with the recurrence rate of 7.7%.The difference was not statistically significant(P>0.05)Conclusion:As a conservative method of traditional Chinese medicineforthe blood heat and intestinal dryness type of stage Ⅱ anal fissure,combining xiaoyan Ointment and Qingzao mixturehas the same effect as nitroglycerin ointment in relieving pain and spasmodic pain.However,it does better than nitroglycerin ointment in relieving defecation difficulty,reducing defecation blood,decreasing adverse reactions,improving TCM syndrome score and so on,which is worthy of further clinical application.
