ObjectiveThis project combines empowerment management with CRRT specialist nurse training for the first time,aiming to explore the clinical effect and significance of implementing empowerment in specialist nurse training.After summarizing problems found in the study,we put forward suggestions to improve the training of specialized CRRT nurses as well as standardize the empowerment management system of specialized CRRT nurses,so as to expand the team of specialist nurses,reduce the incidence of accidents in the operation of CRRT,ensure the safety of patients and improve the quality of specialized nursing services.MethodsThis study was conducted in a class a general hospital in wuhan,hubei province.We provide three-month CRRT specialist nurse training for nurses who meet the registration requirements.The training mainly includes three stages:theoretical learning,operational skills learning and clinical practice.The examination will be carried out one by one.Only those who pass the examination can enter the next round of training.Finally,those who have passed all the examinations will be reported to the nursing department for approval,then he or she will be empowered while awarded with the school-level CRRT specialist nurse qualification certificate.At the same time,we implement the 12-point empowerment management system to manage specialized CRRT nurses.We reauthorize the qualification regularly every year.Before and after the training,the nurses were investigated,including the level of structural empowerment,the level of psychological empowerment and the level of job satisfaction.Before and after the training,data of CRRT emergency handling was collected.We used SPSS25.0 software for statistical analysis of the data.The structural empowerment scores,psychological empowerment scores and job satisfaction scores of the study subjects were expressed as meanąstandard deviation,data of emergency handling ability was expressed in terms of rates.Pared-samples T test was used to analyze the structural empowerment scores,psychological empowerment scores and job satisfaction scores before and after the training.Chi-square analysis of emergency handling capacity difference before and after the training.One-way analysis of variance was used to analyze the relationship between the nurse's general information and structural empowerment,psychological empowerment and job satisfaction.ResultsA total of 49 nurses successfully completed the training as well as passed the assessment.They were certified as CRRT specialist nurses at the hospital level.By combining empowerment management with CRRT specialist nurse training,the structural empowerment level of nurses was significantly improved(P<0.05).The psychological empowerment level of nurses was improved to a certain extent(P<0.05),but there was no significant difference in the score of work influence dimension compared with that before the training(P>0.05).The job satisfaction level of nurses was improved to a certain extent(P<0.05),but there was no significant difference in the score of external satisfaction scale compared with that before the training(P>0.05).The emergency handling ability of CRRT specialist nurses was enhanced compared with that before the training(P<0.05).Among the influences of general data,age,marital status,education level,working years and rank had influences on the score of structural empowerment(P<0.05);gender,marital status,education level and department factors had influence on the score of some dimensions of psychological empowerment(P<0.05);the factors of working years had an influence on the score of job satisfaction(P<0.05).ConclusionTo combine the empowerment management with CRRT specialist nurse training,we not only set the curriculum scientifically,but also standardize the training process of specialist nurses.After that,we qualified nurses who have passed the final examination.Compared with results before the training,the level of structural empowerment,psychological empowerment and job satisfaction of nurses participating in CRRT specialized training were all promoted to some extent.The emergency handling ability of CRRT specialized nurses was also enhanced.The results indicate that the training of CRRT specialized nurses had achieved certain results.
