Objective:Through a comparative study of acupotomy under the press point approach and acupotomy under the transverse process approach for the treatment of lumbar disc herniation,the effectiveness and safety of two acupotomy approaches for lumbar disc herniation were evaluated.It will provide a reference for the standard clinical solution of acupotomy in the treatment of lumbar disc herniation.Methods:According to the inclusion and exclusion criteria specified by the research project,a total of 68 patients with lumbar disc herniation were collected from October 2018 to September 2019 at Nanjing Traditional Chinese Medicine Hospital affiliated to Nanjing University of Chinese Medicine and Nanjing Yuhuatai District Daishan Community Health Service Center.They were randomly divided into 34 cases of press group and 34 cases of transverse process group using random number table.The press point group chose acupotomy release from the press point approach,and the transverse process group chose acupotomy release from the transverse process.These patients in both groups were treated once a week for 4 weeks totally.The evaluation indicators were Oswestry dysfunction index questionnaire(ODI score),Visual Analogue Score(VAS),and lumbar spine activity.Patients were measured before treatment,2nd week of treatment,and 4th weeks of treatment.The clinical symptoms of the patients were followed up 3 months after the end of treatment,and the adverse reactions of the patients were observed and recorded to evaluate the efficacy and safety of the acupotomy treatment.Results:1.After the end of treatment,there was no statistical difference between these two groups in terms of total effectiveness,indicating that both methods have a good effect on LDH;2.The ODI scores and VAS scores of the two groups of patients decreased at different levels compared with that before treatment,but the reduction in the press point group was more significant.Compared with the transverse process group,the differences were statistically significant(P<0.05).Lumbar spine activity increased compared with that before treatment.The difference between these two groups was statistically significant in the comparison of forward flexion activity(P<0.05),but there was no statistically significant change in the extension activity of these two groups(P>0.05).3.At the 3-month follow-up after treatment,there was a statistical difference in the total clinical effective rate between these two groups(P<0.05).The ODI score and VAS pain score of the press point group were better than those of the transverse process group,with significant differences(P<0.05),indicating that the long-term efficacy of the press point group was better than that of the transverse process point groupConclusions:1.Both methods have a good effect on LDH,but in terms of the long-term effect,the effect of the press point group is better than that of the transverse point group;2.Acupotomy treatment for LDH under the press point approach is effective and safe,and it can obviously relieve pain and improve function in a short time.
