Objective:To observe the clinical effect of the treatment of lumbar disc herniation by acupotomolysis combined with lumbar spinal nerve stimulation.To provide the basis for optimizing the treatment of lumbar disc herniation.Methods:According to the diagnostic criteria and inclusion criteria of the disease,a total of 99 patients with lumbar disc herniation were selected to be treated in our hospital from January2019 to December 2019.The patients were randomly divided into three groups: overall acupotomy and lysis group(acupotomy group),needle-knife nerve contact surgical group(acupotomy group),and overall acupotomy and lysis combined withacupotomy and lysis group(combination group),with 33 patients in each group.The overall acupotomy and lysis group treated the overall acupotomy and lysis with bow string and mesh based on the theory.Patients in the contact group were treated with Needle-knife nerve contact surgical.Overall acupotomy and lumbar spinal nerve stimulation were used to treat the disease in the combination group.The duration of the whole course of treatment was 1 time/week,and the treatment lasted for 4 weeks.Researchers will record the situation before and after the treatment of patients,and for about a month after the treatment of visual analog pain score(visual analogue scale,VAS),the Japanese orthopaedic association evaluation score(JOA score),the straight leg-raising test Angle(SLR)and before and after the treatment of Interleukin-6 January(Interleukin 6,IL-6)level to evaluate data,and the three groups after treatment,treatment after the end of a month after the overall efficacy of patients with statistical data analysis,All data were statistically analyzed using SPSS20.0.Results:1.There was no significant difference in gender,age,course of disease,pathological lumbar segment,VAS score,JOA score,SLR value and IL-6 value among the three groups before treatment(P>0.05).2.Total effective rate: after treatment,the total effective rate of the combination group was 95.56%,the total effective rate of the contact group was 80.6%,and the total effective rate of the needle knife group was 87.10%.The results showed that: the overall efficacy of the three groups was statistically significant(P<0.05),the combination group can improve the clinical treatment efficiency of lumbar disc herniation.VAS score: after the treatment and 1 month follow-up,the pain degree of the three groups was improved(P<0.05),after the treatment and 1 month follow-up,the pain improvement of the combined group was better than that of the whole needle knife release group and the nerve touch treatment group(P<0.05).JOA score: after treatment and 1 month follow-up,the three groups of methods have clinical significance in improving JOA score and waist function(P<0.05),After treatment and1 month follow-up,the improvement of the overall situation of the waist in the combined group was better than that in the Acupotomy group and the contact group(P<0.05).SLR value: after treatment and 1 month follow-up,three groups of methods have clinical significance in improving the lower extremity mobility(P<0.05),after treatment and 1 month follow-up,the improvement of straight leg elevation angle in the combined group is better than that in the acupotomotor group and the contact group(P<0.05).IL-6 value: After 1 month follow-up,the three methods had clinical significance in regulating the expression of IL-6 in peripheral blood(P<0.05),The level of IL-6 in the combined group was better than that in the needle knife group and the contact group(P<0.05).Conclusion:Overall acupotomy and lysis combined with Needle-knife nerve contact surgical was better than overall acupotomy and hematogenous knife nerve contact surgical in improving patients’ pain,improving function and reducing blood IL-6 concentration.The possible mechanism of action was that the combination of the two methods could better promote nerve edema and inflammatory absorption. |
