Objective: This topic mainly studies the clinical efficacy of acupotomy transforaminal lumbar spinal nerve root release in the treatment of lumbar intervertebral disc herniation, and compares its therapeutic effect with conventional acupotomy in the treatment of lumbar disc herniation.More targeted, safer and more scientific treatment options. Methods: From January 2021 to January 2022,100 patients with lumbar disc herniation March who were hospitalized in the first ward of the Spine Orthopedics Department of Liuzhou Traditional Chinese Medicine Hospital were randomly divided into the observation group(C-arm X-ray guided acupotomy through the intervertebral foramen).Lumbar spinal nerve release treatment)50 cases and control group(conventional acupotomy treatment)50cases.Before and after treatment, the patients’ perceived pain, symptom improvement, and changes in lumbar spine mobility were observed. All data were compared and analyzed using IBM SPSS statistics 22.0 statistical software.Results:(1)General information: A total of 100 patients with lumbar disc herniation who met the criteria were included in this study,50 in the conventional acupotomy group and 50 in the acupotomy through the intervertebral foramen group.Among them, there were 42 males and 58 females, with a male-to-female ratio of 1:0.724.In terms of age distribution,7were(19-35 years old),45 were(36-59 years old),and 48 were 60 years old and above.Statistical analysis of gender, age, course of disease,VAS score,JOA low back pain score, and ODI index score of the two groups of patients before treatment showed that the results were P>0.05,the difference was not statistically significant, and the studies were comparable.(2)After the treatment, the VAS score,JOA low back pain score, and ODI index score of the two groups were compared with those before treatment, and the difference was statistically significant(P<0.05),indicating that the two regimens can improve the VAS score of patients with lumbar disc herniation,JOA low back pain score, and ODI index score all play a role.(3)After the treatment, the VAS score,JOA low back pain score, and ODI index score of the two groups were compared and analyzed between the two groups before treatment, and the difference was statistically significant(P<0.01),indicating that acupotomy through the intervertebral foramen loosened the lumbar spinal nerve root Removal of lumbar disc herniation in patients with lumbar disc herniation improved VAS score,JOA low back pain score,ODI index than conventional acupotomy.(4)After the treatment, there were no adverse events such as nerve root damage, dural sac tear, local hematoma, wound infection, etc.in both groups. In terms of overall curative effect,1 case(2%)was clinically cured in the control group, and 3 cases were markedly effective.(6%),39 cases(78%)were effective,7 cases(14%)were ineffective, and the total effective rate was86%;in the observation group,6 cases(12%)were clinically cured,21 cases(42%)were markedly effective, and 21 cases were effective(42%),2 cases(4%)were ineffective, and the total effective rate was 96%;the comparative analysis of the effective rate between the two groups was P=0.00 less than0.01,there was a significant difference, indicating that the overall cure rate of the observation group was higher than that of the control group, indicating that acupotomy The overall therapeutic effect of transforaminal lumbar nerve root release for lumbar disc herniation is better than that of conventional acupotomy.Conclusion:(1)acupotomy transforaminal lumbar spinal nerve root release and conventional acupotomy can improve pain symptoms and lumbar spine mobility limitations in patients with lumbar disc herniation.(2)acupotomy transforaminal lumbar spinal nerve root release is more effective than conventional acupotomy in intervening lumbar disc herniation, and can relieve or even eliminate the symptoms of lumbar and leg pain, lumbar dysfunction, etc. It can improve the quality of life of patients and reduce the social burden. It is safer, more accurate and has better curative effect,so it is worthy of clinical promotion and further in-depth research.
