Objective1.By means of literature review,questionnaire survey and qualitative interview,the current situation of hospice care ability of undergraduate nursing students and their needs and expectations for hospice care education are comprehensively understood.2.To construct a concept-based instruction scheme of hospice care for undergraduate nursing students in accordance with the current situation of nursing education in China,and carry out practice research and effect evaluation.MethodsThis study was divided into two parts:The first part was the study on the current situation of hospice care ability and cultivation expectation of undergraduate nursing students:1.Compiling and reliability and validity test of the questionnaire of nursing students' ability of hospice care.The members of the research team consulted the domestic and foreign literatures related to hospice care ability of undergraduate nursing students,initially constructed the entry pool of the questionnaire on hospice care ability of undergraduate nursing students,and further revised the contents of the questionnaire through the expert group meeting cognitive interview and pre-survey.198 undergraduate nursing students who met the inclusion criteria were selected for convenience.Pearson correlation coefficient was adopted for project analysis using the critical ratio method.The structural validity of the questionnaire was tested by exploratory factor analysis.Analysis on the current situation and influencing factors of hospice care ability of undergraduate nursing students in practice.The questionnaire on hospice care ability of undergraduate nursing students was used as a survey tool to facilitate the investigation of 296 senior nursing students from 6 universities,so as to comprehensively understand the status quo of hospice care ability and its influencing factors,and provide valuable reference for the establishment of the concept-based instruction scheme in this study.2.Study on the cultivation expectation of hospice care ability of undergraduate nursing studentsObjective sampling method was used to select 7 hospice care experts to conduct semi-structured interviews.to understand their views and expectations on hospice care ability training for undergraduate nursing students,and to further supplement the contents of the concept-based instruction scheme.The second part was the construction and implementation of the concept-based instruction scheme of hospice care for undergraduate nursing students:On the basis of the previous literature review and the results of the survey and interview analysis,the draft of the concept-based instruction scheme of hospice care for undergraduate nursing students was determined,and 8 experts in related fields were selected to demonstrate the teaching program and further ensure the scientificity and feasibility of the program through the preliminary test.Adopts the principle of voluntary recruitment of a university bachelor degree nursing students of junior year,and randomly divided into intervention group and control group,intervention group of nursing students to participate in the routine teaching at the same time to participate in the network interaction and training based teaching with concept of hospice care,nursing students of control group only in routine teaching and network interaction in the undergraduate nursing students before and after the intervention hospice care ability questionnaire death attitude portrayed scale(revised edition),and evaluate the effect of intervention using purposive sampling method to select interview part of nursing students to participate in this research,fully understand the nursing students to participate in peaceful therapy care training gain experience.ResultsThe results of the first part:1.After the expert group meeting and cognitive interviews,a total of 16 items were included in the final revised questionnaire.According to the preliminary survey,the Cronbach's a score coefficient of the questionnaire was 0.916.indicating good reliability.The correlation analysis results showed that the correlation coefficient between the scores of each item and the total score of the questionnaire was 0.652?0.828,the I-CVI of the questionnaire was 0.83?1.00,and the S-CVI was 0.96.After exploratory factor analysis,a total of three common factors of>1 were generated,namely,effective care skills,culture and ethical values,cross-professional team cooperation,and the contribution rate of cumulative variance was 70.737%.The Cronbach's ? coefficient of the questionnaire was 0.872,and the Cronbach's a coefficient of the three dimensions was 0.890,0.858 and 0.840 respectively.Retest was conducted at an interval of 2 weeks,with retest reliability of 0.793 and retest reliability of 0.813?0.854 for each dimension.The score of hospice care ability of the senior undergraduate nursing students was(41.52±9.59),which was in the lower middle level.The score of effective care skills was(14.48±3.91),the score of culture and ethical values was(15.49±3.34),and the score of cross-professional teamwork was(11.55±2.96).The factors influencing the students' ability of hospice care were the place of origin,whether they had cared for the critically ill patients,and whether they had attended hospice care nursing training(P<0.05).2.Through the analysis of experts to undergraduate nursing students hospice care ability training expectations,two themes were extracted and eight themes:undergraduate nursing students should have the ability to hospice care(to establish a scientific attitude towards death,the basis of hospice care knowledge and skills,able to solve practical problems in the field of hospice care)and undergraduate nursing students of hospice care teaching expectations(the teaching objective is to cultivate the comprehensive ability of nursing students in the practice of hospice care,the characteristic of teacher team is the cooperation of nursing education,online and offline blended learning as the approach,taking hospice care service demand as the training based,adopting diversified and personalized evaluation methods).The results of the second part:1.Combined with the previous research results,the teaching plan was initially constructed,and the concept-based instruction scheme of hospice care for undergraduate nursing students was finally formed after the demonstration by 8 experts in related fields and the preliminary test.The scheme included 9 core teaching concepts:connotation and development,life and death awareness and education,roles responsibilities and capacity,teamwork and communication,assessment and symptom management,spiritual needs and care,psychological needs and care,family and social support,ethics and moral dilemma,16 core concepts and related teaching objectives and teaching forms.2.Before intervention,intervention group and control group in nursing students balance(P>0.05)comparable baseline data.For a period of 9 weeks after the concept teaching,through two independent sample T test analysis showed that intervention group hospice care ability of nursing students score higher than the control group,the difference was statistically significant(P<0.05),one of the effective care skills and cultural and ethical values score higher than the control group,the difference was statistically significant(P<0.05),but transdisciplinary cooperation ability was no statistically significant difference(P>0.05).After intervention,the scores of nursing students in the intervention group in the dimension of death fear and death avoidance were lower than those in the control group,and the difference was statistically significant(P<0.05).The scores of nursing students in the intervention group in the dimension of natural acceptance were higher than those in the control group,and the difference was statistically significant(P<0.05).However,there was no statistically significant difference in scores between the two groups in the dimension of approaching acceptance and escape(P>0.05).ConclusionStatus quo survey combined with literature review qualitative interviews and expert argumentation final build of undergraduate nursing students concept-based instruction scheme,will help the undergraduate nursing students set up the correct view of death,and improve the hospice care ability of nursing students and professional identity,and promote nursing students take the initiative to build knowledge system of hospice care,can provide professional nursing training of hospice care in China provide a new thought of practice and reference.
