Objectives:The purposes of the study are to explore the mortality and its changing trend,rank of cause-specific death rate,the composition of death,the mortality and trends of various components,and the life expectancy after eliminating the main cause of death of residents in the Binhai New Area of Tianjin in 2013-2017,so as to grasp the death spectrum of residents in Binhai New Area and the main diseases affecting the life expectancy of residents,provide scientific basis for the formulation of disease prevention and control policies and the allocation of medical and health resources in Binhai New Area.Methods:The underlying causes of death were classified and coded according to ICD-10.The crude mortality,standardized mortality,and life expectancy were calculated by using"Tianjin All-Cause Monitoring Management System V2.0".The standardized mortality rate was calculated based on the population of the sixth population census in 2010.The cause eliminated life expectancy was calculated by using the life expectancy calculation management system developed by the Chinese Center for Disease Control and prevention.The pearson?~2 test was performed using the SAS statistical software package to compare the difference in mortality between men and women,and the Cochran-Armitage trend test(statistical z value)was performed to analyze the time and age trends of mortality.Results:(1)Death of all causes:In 2013-2017,the total population of Tianjin Binhai New Area was 5,809,800,and the total number of deaths was 35,206,including 20125(57.16%)men,and 15081(42.84%)women.The average death rate of registered residents in Binhai New Area of Tianjin was 605.58/100,000,and the death rate of males(674.51/100,000)was higher than females(533.62/100,000)(P<0.05).The crude mortality rate showed an upward trend in both males and females(P<0.05).The average standardized death rate of registred residents in Binhai New Area of Tianjin was 430.09/100,000 in 2013-2017,and the standardized death rate of males(453.43/100,000)was higher than females(405.95/100,000)(P<0.05).The standardized mortality rate showed a downward trend in both males and female(P<0.05).The semi-logarithmic plot of crude mortality in residents of different age groups showed an obvious“?”distribution.The mortality rate in the 0-year-old group was the highest in the 0-5 age group,mortality is was lower between aged 5and 15 and gradually increased after age of 25.The top five causes of death in Tianjin Binhai New Area in 2013-2017 were malignant tumors,heart disease,cerebrovascular disease,respiratory diseases,injuries and poisoning.The cumulative number of deaths in the top five diseases accounted for 88.91%of the total number of deaths.In2013-2017,The life expectancy of residents in Tianjin Binhai New Area from 2013 to2017 were 81.23 years,80.84 years,81.21 years,81.96 years,and 81.94 years,respectively,and the average life expectancy was 81.47 years.The top three life expectancy increases were heart disease,malignant tumors and cerebrovascular disease.(2)Infectious diseases,maternal and infant diseases,and nutritional deficiencies:In the five years,the average mortality rate was 17.44/100,000,with an upward trend(Z=2.632,P<0.05);the average standardized mortality rate was 13.40/100,000;the mortality in males was higher than in females(P<0.001);mortality showed a"U"shape distribution with age.After removing the death of this kind of diseases,the average life expectancy increased by 0.52 years.(3)Deaths of chronic non-communicable diseases:The average mortality rate was553.91/100,000,with an upward trend(Z=2.562,P<0.05);the average standardized mortality rate was 387.59/100,000,showing a downward trend(Z=-5.000,P<0.05);the mortality in males was higher than in females(P<0.001);96.84%patients with chronic non-communicable diseases were over 45 years old.Cardiovascular diseases,malignant tumors,major respiratory diseases and diabetes are the four major causes of chronic illness in residents of Binhai New Area,accounting for 94.00%of all chronic disease deaths.The life expectancy after removing the deaths of cardiovascular disease,malignant tumor,major respiratory diseases and diabetes increased by 13.04 years,3.56 years,0.51 years,and 0.31 years,respectively.(4)Deaths from injuries:The average mortality rate was 28.23/100,000,and the average standardized mortality rate was 24.14/100,000.Mortality in males was higher than in females(P<0.001).In the composition of the cause of injury,accidental damage accounted for 84.94%,intentional damage accounted for 14.39%,and the remainder was intentional uncertainty.In 2013-2017,after the cause of injury was removed,life expectancy increased by an average of 0.70 years.Conclusions:(1)The average death rate of residents in Tianjin Binhai New Area in 2013-2017 was lower than the death level of Tianjin residents in 2015(mortality was713.88/100,000).In the past five years,the crude mortality rate has shown an upward trend,while the standardized mortality rate has shown a downward trend.(2)The main causes of death in different genders and different age groups are different.Therefore,health education work can be carried out in different categories to maximize the effectiveness of work.(3)The number of deaths caused by infectious diseases and injuries accounts for less than 10%of total deaths,but given the social harmfulness of infectious diseases,sustained attention is also needed.(4)Chronic non-communicable diseases are the main cause of death for residents of Binhai New Area and the main diseases that affect the life expectancy of residents.
