OBJECTIVE: To analyze the relationship between the pre-pregnancy smoking and the degree of coronary artery disease in patients undergoing coronary angiography in our hospital. Methods: A total of 148 patients who underwent coronary angiography in the Department of Cardiology of the Research Center(Linyi People’s Hospital)from May2017 to December 2018 were enrolled, including 56 females and 92 males. The average age was 59.63.±9.48)years old.According to the results of coronary angiography after admission,105 patients with coronary heart disease were diagnosed as coronary heart disease, and the remaining 43 patients were treated as non-coronary heart disease group.General clinical data such as gender, age, diabetes and hypertension history, personal smoking history and parental smoking history, blood lipid levels, blood routine, and uric acid were collected.According to the Gensini score, the coronary heart disease components were 1)mild stenosis group(Gensini score ≤ 20 points);2)moderate stenosis group(20 <Gensini score <50);3)severe stenosis group(Gensini score ≥ 50).The baseline data of the coronary heart disease group and the control group were analyzed and compared. The progeny had coronary heart disease as the dependent variable, and the factors that may affect the progeny with coronary heart disease were used as independent variables.Binary logistic multivariate regression analysis was performed to analyze coronary heart disease.Correlation with various risk factors;using coronary artery lesion stenosis as the dependent variable, other factors that may affect the degree of coronary artery disease as the dependent variable, using multiple logistic regression analysis to analyze the degree of pre-pregnancy smoking and coronary artery lesion stenosis Relationship.RESULTS: The statistical results showed that patients with coronary heart disease had higher risk factors for gender, type 2 diabetes,pre-pregnancy smoking, and personal smoking than the control group(P<0.05).Two-way logistic multivariate regression analysis between coronary heart disease group and non-coronary heart disease group Show patients with pre-pregnancy smoking(OR 7.870,95% CI 2.790 ~ 22.201,P =0.000),personal smoking(OR 7.581,95% CI 2.655 ~ 21.648,P = 0.000),LDL-C(OR10.587,95% CI 1.415 ~79.214,P=0.022),HDL-C(OR 0.163,95% CI 0.040 ~ 0.673,P=0.012)were associated with coronary heart disease,in which pre-pregnancy smoking, personal smoking and LDL-C were positively correlated with coronary heart disease,HDL-C was negatively correlated with coronary heart disease;coronary heart disease group as the dependent variable, including gender, type 2 diabetes, hypertension, personal smoking history, parental pre-pregnancy smoking history, five risk factors and TG,TC,LDL-C,UA and other laboratory biochemical indicators were independent variables, and the data were subjected to ordered logistic regression analysis. The statistical results showed that the parents smoked before pregnancy(OR 0.536,95% CI-2.463 ~-0.864,P= 0.035),personal smoking(OR 0.594,95% CI-2.848 ~-0.34 6,P = 0.043),diabetes(OR 0.112,95% CI-3.242 ~-1.143,P = 0.000)is associated with the severity of coronary artery disease in patients with coronary heart disease, and can be used to predict the degree of coronary artery disease stenosis. Conclusion: Pre-pregnancy smoking is a risk factor for coronary heart disease in pre-pregnancy.Pre-pregnancy smoking has a correlation with the degree of coronary artery stenosis in offspring, and can be used as a risk factor for predicting the degree of coronary artery disease.
