ObjectiveAs a chronic non-communicable disease, diabetes has caused serious harm to the physical and mental health of the people, especially the elderly. To investigate the cognition of diabetic mellitus related knowledge of community nurses in community health service institutions in Pingshan District of Shenzhen City,to know about the situation of knowledge related diabetes in community nurses, and analyze the related factors that affect their cognition.Meanwhile,we investigated the training needs of community nurses about the knowledge of diabetes, and analyze the related factors that affect their joining training and put forward some suggestions to improve the knowledge of diabetes among community nurses. In order to improve the overall service quality of community health service center, meet the needs of the masses of the people, especially the elderly.MethodsA total of 132 community nurses from 27 community health service centers attached to two public hospitals in Pingshan District of Shenzhen were investigated by cluster sampling method. The questionnaire was composed of three parts: “Basic data questionnaire”,“Diabetes knowledge cognition questionnaire”,“Diabetes knowledge training needs questionnaire”.Diabetes Knowledge Cognition Questionnaire has six dimensions, each dimension is attached to whether the need for training entries.Data were double input by Excel software, and statistical analysis was conducted by descriptive statistics analysis,one-way ANOVA,Spearman correlation analysis and multiple linear regression analysis using SPSS 17 software.ResultsAmong the 132 community nurses in the study, the cognition accuracy rate of diabetes knowledge level was not high, the highest accuracy rate was the knowledge of diabetes mellitus drug treatment,55.3%,the lowest accuracy rate was diabetes monitoring knowledge, only 22%,the overall accuracy rate of diabetes knowledge cognition was only 3%;the score of 4.5% of community nurses was 59 points and below,64.4% was ranging from 60 points to 84 points, and 31.3% was 85 points and above;among the 21 items, the higher score is the basic knowledge of diabetes, and the lower score is the knowledge of diabetes monitoring; The highest training demand in six dimensions was diabetes monitoring knowledge, which was 71.2%,followed by the training demand for knowledge of diabetic complications was 53.8%;In training needs survey,72% of community nurses thought that to improve nurses’ clinical work ability in diabetes specialist training needs is important;in the emphasis of diabetes specialist training,57.6% of community nurses thought that we should focus on the combination of theory and practice.54.5% thought that we should focus on diabetes health education;in term of training harvesting,80.3% of respondents wanted to harvest certificates, and 78% of respondents want to reap the capacity of health education;in the term of best clinical practice mode,58.3% of the respondents believe that it should be the best way of clinical practice according to the trainees’ situation,49.2% of the respondents think that specialist teaching is the best way of clinical practice; With regard to the necessity of specialist training in diabetes,38.6% of the respondents considered that specialist training in diabetes was very necessary,30.3% of the respondents believed that diabetes specialist training is necessary.Through the analysis of single factor variance(one-way ANOVA),it is concluded that there is a significant difference in the cognition level of community nurses with different demographic characteristics, such as working years, marital status, professional title, staffing level, endocrinology work experience, diabetes training experience(P<0.05);through the Spearman correlation analysis, and the number of years of community work, community working life, endocrine work experience, diabetes training experience and diabetes cognitive total score is positive correlation(P<0.001),and staffing level is negatively correlated with the total score of diabetes cognition(P<0.001).Through multiple linear regression analysis, with the influencing factors of community working life, staffing level, endocrine work experience and diabetes training experience are gradually brought in,all factors have an impact on the cognitive score of diabetes knowledge,of which, endocrine work experience coefficient is 0.259(P<0.001),the greatest impact on diabetes knowledge cognition, followed by diabetes training experience coefficient of 0.253(P< 0.001).ConclusionsThe overall knowledge cognition level of diabetes mellitus of community nurses is not high;there are significant differences in cognition level of diabetes knowledge among community nurses with different working years, different marital status, different job titles, different staffing level conditions, different endocrinology experience and diabetes training experience;with the increase of community working life, the improvement of endocrine work experience and diabetes specialist training experience, the total score of diabetes knowledge cognition also increased, however, the cognition level of diabetes knowledge of the community nurses was not high with staffing level of community nurses;the staffing level and the training experience of diabetes had a relatively large impact on the cognition level of diabetes knowledge, which showed the importance of staffing level and training.
