ObjectiveOn the basis of the investigation and understanding of the current situation of community health workers' long-term care assessment for disabled elderly,the intervention program(O2O training model)to improve the long-term care assessment of disabled elderly people(training model)was formulated and its effect was verified in order to improve the long-term care evaluation of the disabled elderly in our community.The evaluation work provides theoretical basis and practical guidance.MethodsThis study is divided into two parts:The first part is a survey of knowledge,attitude and practice of community health workers on long-term care for disabled elderly.Based on the theory of knowledge and credit,a preliminary draft of the questionnaire was formed on the basis of fully consulting the domestic and foreign literature and expert consultation.The preliminary revision of the items was carried out through cognitive interview.After the validity and reliability of the questionnaire were measured by pre investigation and expert validity,the long-term care of the community health workers for the disabled elderly was formed.The questionnaire of the estimation of the knowledge bank.From July 2017 to October 2017,20 community health service centers in Weifang(including community health service stations)and 250 community health workers who were willing to accept the survey were selected by convenient sampling method,and the self design "community health workers' disability" was used.The questionnaire on the long-term care assessment of the elderly was conducted to investigate the status of community health workers' knowledge,attitude and behavior on long-term care assessment for disabled elderly.The second part is the training effect of community health workers for the long-term care evaluation of disabled elderly.On the basis of literature research,theoretical analysis and cross-sectional investigation,the intervention program(O2O training mode)was formulated through expert group meetings.In the first part of the study,80 community health workers were selected in accordance with the standard of the study.The informed consent was signed according to the voluntary principle,and the random number table was used to divide into the intervention group and the control group.The intervention group adopted the O2 O training mode while the control group adopted the traditional on the job training mode.The effect of intervention was evaluated immediately after training,1 months after training and 4 months after training.The data were recorded by Epidate3.1 software,and the data were analyzed by SPSS-19.0,t test,chi square test,variance analysis,and variance analysis of repeated measurement.ResultThe first part is the survey results of the knowledge bank.(1)A questionnaire for the community health workers in the long-term care assessment of disabled elderly was compiled.The questionnaire includes three parts: the informed consent,the basic information questionnaire and the questionnaire of the long-term care assessment of the elderly.Among them,the questionnaire of knowledge,attitude and behavior of disabled elderly was divided into three dimensions: knowledge,attitude and behavior,with 57 entries.The knowledge dimension has three options,namely,right,wrong and unknown.The answer to one question is one point,the wrong answer or the zero point;the attitude dimension adopts the Likert5 grade scoring method,and the 1-5 points express the very disagreement,disagreement,no opinion,agreement,and very agreeing.The behavioral dimension also adopts the Likert5 grade scoring method,and 1-5 indicates never,seldom,occasionally,frequently and always.The total score of the questionnaire was 117 points,the higher the score,the better the current situation.(2)Results of the basic data:(1)general data results: among the 236 community health workers surveyed,142 were nurses,94 were doctors,144 were women(144,61%);the majority(80,33.9%)were among the age 21-30(80,33.9%);education was mainly(106,44.9%),and most of the community health workers worked in 2-5.Years(80,33.9%);the majority of clinical experience in community health workers(129,54.7%);(2)the basic results of community health workers' long-term care assessment for disabled elderly: the survey results of community health workers' awareness rate of long-term care assessment tools for disabled elderly showed the daily living scale The awareness rate of(Activities of Daily Living,ADL)was the highest(93.6%).The awareness rate of the home care index questionnaire(family adaptation,partnership,growth,affection,resolve,APGAR)was the lowest(24.2%);95(38%)used the long-term care assessment tool for the elderly with negligent ability,and the long-term care assessment of the elderly was not used.There were 141(59.7%)of the tools;the main channel for nurses to obtain long-term care assessment for disabled elderly was the network,the access of doctors to periodical literature,79 training for long-term care assessment for the elderly,157 community health workers who did not receive related training,and only 1 of the existing training activities.6.5% people are very satisfied,and 41% of the community health workers are generally satisfied with the existing training satisfaction;86.9% of the community health workers think it is necessary to carry out the related long-term care assessment of the disabled elderly.(3)The total score of the total score of the long-term care assessment for the disabled elderly was 67.20 ± 8.68,the average score of the knowledge dimension was 23.72 ± 3.89,the average score of the attitude dimension was 31.89 ± 5.09,and the average score of the behavioral dimension was 11.60 ±2.36.(4)The results of Pearson correlation analysis showed that community health workers' knowledge and attitude(r=0.374,P<0.01),knowledge and behavior(r=0.296,P<0.01),and between attitude and behavior(r=0.351,P<0.01)were positively related to the long-term care assessment of the disabled elderly.The results of the second part of the study on the effect of training:(1)After the training,the total score,knowledge score,attitude score and behavior score of the intervention group and the control group were improved,and the score of the intervention group was significantly higher than that of the control group,but the scores of each dimension gradually decreased with the time of the training.(2)The results of repeated measurement of variance analysis showed that there were significant differences between the intervention group and the control group on the knowledge score,the attitude score,the behavior score and the total score of the total score of the knowledge and credit line(P<0.01);the knowledge score,the attitude score,the behavior score and the total score of the knowledge and letter were on the main intervention effect in the intervention group and the control group.The difference was statistically significant(P<0.01);there was an interaction between the total score of knowledge and belief and the score of behavior in time and intervention methods(P>0.05).ConclusionThe level of knowledge,attitude,attitude,knowledge and behavior,behavior and attitude were positively correlated with the low level of the long-term care assessment of the disabled people in the community health work.The related training is less and the training effect is not ideal.Therefore,it is necessary to strengthen the training work on the long-term care assessment of the disabled elderly by community health workers,optimize the training model and improve the level of long-term care assessment for the disabled elderly.The O2 O training model,which is based on situational teaching and network self-study as the supplement,is better than the control group for the long-term care assessment of the disabled elderly.However,after a certain period of training,the assessment level has declined,so regular retraining should be emphasized.
