Clinical Observation Of Self-made Qingfei Zhixue Decoction In The Treatment Of Bronchiectasis Hemoptysis

Objective:This study aims to provide a clinical basis for the treatment of bronchiectasis hemoptysis by "Qingfei Zhixue Decoction" through theoretical research,clinical efficacy observation,safety,etc.,in order to reduce the use of antibiotics,reduce the number of recurrences of such patients,and Provide new ideas for improving the quality of life.Methods:Sixty patients who met the diagnostic criteria of Western medicine for bronchiectasis hemoptysis and TCM hemoptysis were randomly divided into treatment group(n=30)and control group(n=30).The control group was treated with anti-infective,anti-inflammatory and cough-relieving clinical routine treatment:starting from the experience of the disease,routine intravenous infusion of levofloxacin hydrochloride 0.5g,once a day;intravenous drip ambroxol 30.0mg,2times a day;Adjust the results of the sputum culture and drug sensitivity test as appropriate.According to the hemoptysis,hemostatic treatment is given.If the hemoptysis is less than 100 ml within 24 hours,oral Yunnan Baiyao Capsules 0.5g,3times a day;24 hours of hemoptysis 100ml-500 ml,given intravenous drip pituitrin 5U-10 U.The treatment group took the experience side "Qingfei Zhixue Decoction" on the basis of the control group(shengdi 20 g,baihe 15 g,maidong 15 g,bainaishen 15 g,huangqin 9g,zhizi 9g,xuesanqi 10 g,wugongqi 10 g,xuedanggui 10 g,baiji 10 g,cebaiye 10 g,gancao 10g),add or subtract with water decoction,100 ml each time,warm morning and evening after meals,2 weeks for a course of treatment.decoction with water,100 ml each time,warm morning and evening after meals,2 weeks for a course of treatment.The changes of clinical symptoms(cough,expectoration,hemoptysis,chest tightness),signs,important inflammatory markers(WBC,NE%,CRP,ESR)and sputum culture were compared between the two groups before and after treatment.Results: 1.Comparison of clinical efficacy between the two groups: The total effective rate of clinical treatment(96.67%)was higher in the treatment group than in the control group(90%),P<0.05,which was statistically significant.2.After treatment,the symptoms and signs of the two groups were improved(P< 0.05).The improvement of symptoms and signs in the treatment group was better than that in the control group(P<0.05).3.The main clinical symptoms of the treatment group: hemoptysis,cough,cough,chest tightness recovery time were shorter than the control group,in which the recovery time of bloody symptoms in the treatment group was 1.06 days to 3.28 days,and the recovery time of the control group was 3.03 days to 4.63.Days,P<0.05,were statistically significant;the recovery time of hemoptysis in the treatment group was1.53 days to 4.07 days,and the recovery time of hemoptysis in the control group was3.26 to 6.68 days,with a difference,P<0.05,which was statistically significant.4.Compared with before and after treatment,the blood inflammatory markers in the two treatment groups were improved,P<0.05,which was statistically significant,suggesting that both treatments can control infection.However,after treatment,P<0.05 was also different in the two groups,so the treatment group had better control effect.5.Before and after treatment in the two groups,the conversion rate of sputum culture in the treatment group was 77.78%,and the conversion rate of sputum culture in the control group was 60%.However,the effect of the control group was significantly lower than that of the treatment group,which was statistically significant.6.There were no adverse reactions between the two groups before and after treatment,and the clinical safety was reliable.Conclusion: This clinical study shows that in the treatment of bronchiectasis hemoptysis,the treatment group is better than the western medicine treatment group in terms of clinical symptoms,signs,inflammation,and sputum culture conversion rate,and it is safe and has no toxic side effects,which will be used in our future clinical practice.The study provides a new idea to further promote the advantages of integrated traditional Chinese and Western medicine in the treatment of bronchiectasis and hemoptysis.It can not only reduce the use of antibiotics,but also improve the symptoms of patients,improve the quality of life and reduce the economic burden tosome extent.
