Objective: In this study,a randomized, controlled clinical trial was conducted to observe the clinical efficacy of Heat-reinforcing acupuncture manipulation therapy and conventional twirling reinforcing acupuncture manipulation therapy in the treatment of diarrhea-predominant irritable bowel syndrome(Deficiency of Spleen and Stomach Type). To explore the curative effect and clinical application value of Heat-reinforcing acupuncture manipulation therapy in treating diarrhea-predominant irritable bowel syndrome(Deficiency of Spleen and Stomach Type),and to provide a safe and efficient new method for clinical treatment of diarrhea-predominant irritable bowel syndrome(Deficiency of Spleen and Stomach Type). Methods: 60 patients who met the inclusion criteria were randomly divided into Heat-reinforcing acupuncture manipulation therapy group(30 cases)and conventional twirling reinforcing acupuncture manipulation therapy group(30 cases)by random number table method and envelope method. The same acupoints were used in the two groups:Pishu(BL20),Dachangshu(BL 25),Zhongwan(RN 12),Qihai(RN 6),Tianshu(ST 25),Zusanli(ST 36),Shangjuxu(ST 37)and Sanyinjiao(SP6).The basic treatment of the two groups were health education, and on this basis, the corresponding acupuncture manipulation was used. The two groups were treated once every other day,10 times as a course of treatment,a total of 2 courses of treatment,2 courses of treatment,3 days of rest during the 2 courses. The total score of TCM syndromes and the score of each symptoms,IBS-SSS symptom severity score and IBS-QOL quality of life score before and after treatment in the two groups were taken as observation indexes. Before and after treatment, the observation indexes of the two groups were scored by scale, and the data were analyzed by statistical software SPSS22.0.The clinical efficacy of the two groups was observed.Results:1.There was no significant difference in sex,age and course of disease between the Heat-reinforcing acupuncture manipulation therapy group and conventional twirling reinforcing acupuncture manipulation therapy group(P > 0.05),which was comparable.2.There was no significant difference in the total score of TCM syndromes and the score of symptoms,IBS-SSS symptom severity score and IBS-QOL quality of life score between the Heat-reinforcing acupuncture manipulation therapy group and conventional twirling reinforcing acupuncture manipulation therapy group before treatment(P>0.05).3.After two courses of treatment, the results of comparison of TCM syndromes between the Heat-reinforcing acupuncture manipulation therapy group and conventional twirling reinforcing acupuncture manipulation therapy group showed that:In the Heat-reinforcing acupuncture manipulation therapy group of 30 patients,of which 9patients were cured clinically,9 patients were markedly effective,11 patients were effective, and only 1 patient was ineffective. In the Conventional twirling reinforcing acupuncture manipulation therapy group of 30 patients,of which 6 patients were clinically cured,7 patients were significantly effective,12 patients were effective, and 5 patients were ineffective. The total clinical effective rate of Heat-reinforcing acupuncture manipulation therapy group and conventional twirling reinforcing acupuncture manipulation therapy group was 96.7% and 83.3%.The clinical total effective rate of the Heat-reinforcing acupuncture manipulation therapy group after treatment was better than that of conventional twirling reinforcing acupuncture manipulation therapy group(P<0.05).4.After two courses of treatment, the total scores of TCM syndromes in the Heat-reinforcing acupuncture manipulation therapy group and the conventional twirling reinforcing acupuncture manipulation therapy group were significantly different(P<0.01).There were significant differences in symptoms scores of loose stool, postprandial abdominal distension, abdominal distention and pain and mental fatigue between the two groups(P<0.05).After 2 courses of treatment, the scores of cold chills were significantly different between the two groups(P<0.01).After treatment, there was no significant difference between the two groups(P>0.05).5.After two courses of treatment, the total scores of IBS-SSS symptoms in the Heat-reinforcing acupuncture manipulation therapy group and the conventional twirling reinforcing acupuncture manipulation therapy group were compared. And the results showed that there were some differences between the two groups in improving the severity of IBS symptoms(P<0.05).6.After two courses of treatment, the IBS-QOL quality of life scores of the two groups were compared. The results showed that the IBS-QOL quality of life scores of the Heat-reinforcing acupuncture manipulation therapy group and the conventional twirling reinforcing acupuncture manipulation therapy group were significantly different(P< 0.05).There were statistical differences in the improvement of seven dimensions of dysthymia, behavioral disorders,self-image,health concerns, food avoidance, social function and sexual behavior between the two groups(P<0.05),but there was no significant difference in the improvement of relationship expansion between the two groups(P>0.05).Conclusion:1.Both the Heat-reinforcing acupuncture manipulation therapy group and the conventional twirling reinforcing acupuncture manipulation therapy group can improve the symptoms of diarrhea-predominant irritable bowel syndrome(Deficiency of Spleen and Stomach Type),but the Heat-reinforcing acupuncture manipulation therapy group is better than the conventional twirling reinforcing acupuncture manipulation therapy group in improving clinical efficacy, clinical symptoms and quality of life.2.Heat-reinforcing acupuncture manipulation treatment of diarrhea-predominant irritable bowel syndrome(Deficiency of Spleen and Stomach Type)is safe and effective, which is worthy of clinical application. |
