BackgroundWith the development of society.psychological health was receiving more and more attention.Medical staffs in intensive care unit(ICU)were the "severely afflicted area" of mental health problems,ICU was the special clinical department for emergency and severe cases patients,whose condition was generally heavier and changed rapidly.Thus,ICU works always with high technical requirements,work strength and high medical risks,etc.The medical staff was in high pressure stress state for a long time,unavoidably bringing strong emotional reaction,also more prone to anxiety,depression and other mental disorders.ICU was with higher risk of psychological problems than medical workers at ordinary ward,while the surveys of mental health of ICU medical staff were still relatively rare currently,and there is also an urgent need to analyze the related factors of mental health problems on ICU medical staffs.ObjectivesTo investigate the mental health status of ICU staffs,analyze the causes of psychological problems such as anxiety and depression,screening for the high-risk groups of psychological problems and providing certain reference for psychological evaluation and psychological intervention in these people.MethodsThe questionnaire was prepared based on consulting literature and conducting preliminary survey.The questionnaire can be divided into two parts:general information and psychological behavior scale.A stratified random sampling method was adopted,and a total of 221 cases from tertiary hospital ICU staffs in bengbu city were recruited in this study.Questionnaire survey method was conducted by general information such as gender,age,family background,level of education,marital status,work,night shift,and self-rating depression scale(SDS),and self-rating anxiety scale(SAS)were also investigated.Epidata software was adopted to input data and perform data verification.SPSS 16.0 software was uased to analyze data.Proper tatistical methods was used to compare the mental health state between different clinical characteristics of the crowd.Main ResultsA total of 221 ICU medical staffs were surveyed,including 56 doctors and 165 nurses.The depression score of ICU doctors was 50.02±12.21 and 46.04±13.57 for anxiety,which were both higher than the national norm(P<0.01).The depression score of ICU nurses was 51.93±9.96 and 49.33± 12.30 for anxiety,which were both higher than the national norm(P<0.01).The depression score of ICU staffs was 51.44±10.58 and 48.50±12.69 for anxiety,which were both higher than the national norm(P<0.01).The positive rate of depression for ICU doctors was 42.86%,which was lower than that for ICU nurses(62.42%)(X2=6.549,P =0.010).The positive rate of anxiety for ICU doctors was 39.29%,which was lower than that for ICU nurses(58.18%)(X2=5.999,P =0.014).Univariate analysis showed that age,working years and family stress were correlated with the positive rates of depression and anxiety among ICU doctors.Mulivariate logistic regression showed that family stress was the main factor for affecting the depression of ICU doctors.ICU doctors with higher family stress had a high risk of depression.Besides,family stress was also the main factor for affecting the anxiety of doctors in ICU.ICU doctors with higher family stress had a high risk of anxiety.Univariate analysis showed that family stress,workload and monthly income were correlated with the positive rates of depression and anxiety among ICU nurses.Mulivariate logistic regression showed that family stress,workload and monthly income were the main factors for affecting depression in ICU nurses.ICU nurses with high family stress.heavy workload and low monthly income were at high risk of depression.Marital status,working years,sleep status and workload were the main factors for influencing the anxiety of ICU nurses.ICU nurses with married,short working years,poor sleep quality and heavy workload were at high risk of anxiety.Univariate analysis showed that family stress,night shift,workload and monthly income were correlated with the positive rates of depression and anxiety among ICU staffs.Besides,gender was associated with the positive rates of depression Mulivariate logistic regression showed that the main factors of affecting the depression of ICU medical staffs were gender,family stress,workload and monthly income.ICU staffs with women,family stress,heavy workload and low monthly income people had high risk of depression.The main factors of affecting the anxiety of ICU medical staffs were gender,marital status,family stress,workload and monthly income.ICU staffs with women,married,heavy workload and low monthly income people had high risk of anxiety.Conclusions and suggestionsThere are some severe mental health problems in ICU staffs,the situations of anxiety and depression in doctors and nurses were worse than that of national norm,especially in nurses.Work environment,family and marriage have certain relevance with nurse's mental health problems.Therefore,appropriate psychological intervention should be timely conduct on the ICU staff,especially in nurses with heavy family burden and poor quality of sleep.
