Objective 1.To investigate the effect of noninvasive ventilation(NIV)therapy adherence on the treatment outcome of OSAHS patients with hypertension.2.To investigate the effect of continuous nursing intervention on NIV treatment adherence and therapeutic effect in OSAHS patients with hypertension.Methods The first part of the review before the case recorded method to collect data.We collected general information(name,gender,age,height,weight,BMI,waistline,neck circumference,history of smoking,drinking,etc.).Breathing machine using adherence: by the team members would link the memory card through the card reader,connected with breathing machine corresponding computer software,got patients used breathing machine a month time more than 4 hours the number of days,the average monthly used breathing machine time,remaining AHI and made records.Reviewing and collecting random blood pressure of 212 patients a day,morning blood pressure,highest blood pressure,antihypertensive drugs and dosages.The second part we used the convenience sampling method to select between September 2015 and March 2017 in Tianjin Medical University General Hospital in our sleep center.One hundred and sixty new diagnosed patients with moderate-severe OSAHS by polysomnograph(PSG)were selected and divided into experimental and control groups randomly,each group contained 80 patients.Patients in control group would receive routine care,however,experimental group would receive continuity of nursing care for about one year.Repeated measurement analysis of variance,NIV treatment efficacy and NIV treatment adherence at the 1st,6th,and 12 th months.Before and after the intervention were used to analyze the 12-month quality of life score in the experimental group.Two independent samples t test were used to test the effect of NIV before and after intervention for 12 months.Results 1.Good adherence group's random blood pressure,morning blood pressure,highest blood pressure,antihypertensive drugs and dosages lower than group of poor adherence,P<0.05.2.First month of the two groups after intervention in patients with general information,ESS scores and breathing machine use adherence comparison,but there were no difference in the two groups of patients,P>0.05,not statistically significant.The 12 months of the two groups after intervention in patients with general information,ESS scores comparison and breathing machine use adherence comparison,the experimental group patients effect were better,statistical significantly,P<0.05.3.Two groups of 12 months after the baseline and intervention in patients with OSAHS-QOL scores and OSAHS-QOL compared each dimension scores,while the control group and experimental group compared with baseline are improved,but the experimental group patients with OSAHS,QOL and each dimension scores significantly higher than the control group,P < 0.001.4.Repetitive measure analysis of variance showed that two groups of patients with general information,disease related information and use breathing machine adherence and OSAHS-QOL scores time effect of repeated measurement variance comparison difference were statistically significant(P<0.05).5.Continuity of nursing care could improve the adherence of patients who with OSAHS and wanted to use NIV treatment,which also could increase the patients using the NIV treatment time,what's more,improved OSAHS combined the hypertension of patients' blood pressure.Conclusions The adherence of NIV therapy in OSAHS patients with hypertension may affect the blood pressure control and the type and dose of antihypertensive drugs.Continuation of phased nursing interventions can effectively improve the adherence of NIV treatment in OSAHS patients with hypertension.Improvement of NIV treatment adherence,improved patient's blood pressure control and the quality of life.It is scientific,reasonable,practical and effective to set up the continuation of staged nursing intervention.It is worth promoting in clinical nursing work.
